    Dedicated caring staff, clean facility

I'm very pleased with the dedicated, caring staff and the attentive, family-like care my loved one received. The place is clean, warm and home-like - beautifully decorated lobby, veteran tributes, strong rehab and social programs, and responsive management. I'd recommend it for short- or long-term stays and would gladly send my father here again, though there's still room for small improvements.

    Summary of reviews

Washington Square Healthcare presents a mixed picture: reviewers include strong endorsements of clinical and rehabilitative strengths alongside repeated operational concerns. Positive commentary centers on thoughtful clinical teams, robust therapy and rehab programming, and a clear emphasis on veteran-centered services and VA affiliation. Several families describe warm greetings, a well-appointed lobby and dining room, and comfortable, home-like resident rooms. The facility has received longstanding recognitions, and many reviewers highlight engaged activity programming, respite and hospice availability, and areas where housekeeping and reception staff perform well.

Care quality appears variable across shifts and units. Numerous accounts describe attentive, proactive nurses and aides who communicate with families and adapt care plans; conversely, other accounts describe delays in responding to call lights, lapses in monitoring (for example, blood-sugar and other clinical checks), and concerns about medication-safety processes. These differences point to inconsistent staffing levels and uneven clinical oversight as primary drivers of variability in resident experience. There are specific family concerns related to timely attention after falls and to transfer safety, which suggest opportunities to strengthen fall-prevention protocols and equipment maintenance.

Staff behavior and communication are another polarizing area. Many reviewers praise compassionate, family-inclusive caregivers and strong management when present; others report instances of perceived inattentiveness, unprofessional conduct, or inadequate identification of staff roles. Phone responsiveness, call-station monitoring, and timely family notification of clinical changes are recurring themes for improvement. An allegation involving boundary-crossing behavior was also raised; such claims underscore the need for consistent staff accountability and clear visitor/interaction policies.

Dining and nutrition receive mixed feedback. The facility offers an inviting dining atmosphere and therapeutic meal services, but some reviewers reported repetitive menus, inconsistent adherence to prescribed diets (for example, diabetic meal plans), and dissatisfaction with texture-modified food preparation. Strengthening menu variety and ensuring dietary orders are consistently followed would address several families' concerns.

Facilities and cleanliness likewise show a split: many reviewers find common areas clean and well-maintained with respectful tributes to veterans, while others point to sanitation lapses, pest-control concerns, and room-level deep-cleaning needs. Housekeeping performance appears uneven across units, suggesting targeted quality checks could improve consistency. The physical environment, when well maintained, supports rehabilitation and social engagement; when maintenance lapses occur, they contribute to safety and dignity concerns.

Management and responsiveness present a contrasted view. Positive reports describe engaged leadership, continuous-improvement efforts, and supportive family partnerships. Negative accounts focus on slow administrative responses to complaints and concerns about escalation to external authorities. Taken together, the pattern suggests a facility with significant strengthsโ€”particularly in rehabilitation, veteran services, and pockets of strong caregivingโ€”but with operational variability that affects family confidence. Prospective residents and families should weigh the facilityโ€™s veteran-focused services and therapy capabilities against the documented variability in staffing consistency, communication practices, dietary and hygiene routines, and administrative follow-through. Visiting in person, asking about current staffing ratios, fall-prevention protocols, dietary procedures, and complaint-escalation pathways will help clarify how these patterns apply to particular units or rooms.

    About Washington Square Healthcare

Washington Square Healthcare sits as a large senior living community with a lot of different options for folks who need help at different stages. The facility supports all levels of care, so people can live more independently or get full-time skilled nursing if they need it, and there's also memory care for dementia or Alzheimer's. The place offers active adult and independent living, assisted living, memory care, home health care, adult day services, long-term care and skilled nursing, and even hospice and respite care for short stays or if a family caregiver needs a break. There are about 20 certified beds coming open in June 2025, and the community accepts Medicare, Medicaid, and private insurance, and even helps with funding advice if that's needed, and it's also the only VA-approved skilled nursing center in Trumbull County.

Staff include a team of nurses, physicians, social workers, and nutritionists, and people get 24-hour supervision with 12 to 16-hour nursing care. The place helps with daily tasks like bathing, getting dressed, medication, transfers, and even more hands-on non-ambulatory help if that comes up. Skilled nursing is there for those with serious health needs, offering things like wound care, infusion therapy, surgery recovery, disease management, and full rehab after an illness or injury. There are also programs focused on rehabilitation to help people get back on their feet and return home if that's possible. Meals are prepared by a professional chef, with dining options that let folks eat all day, and special menus cover allergies or diabetes.

Rooms come furnished and might have private bathrooms, kitchenettes, cable TV, phones, air conditioning, and high-speed internet, but not every room has every feature. There are common areas for people to spend time together, including a fitness room, library, spa or wellness space, arts and activity rooms, and spots for movies and games, and you'll find outdoor walking paths and gardens for fresh air and sunshine. Housekeeping and laundry are handled, and families can count on move-in help and ongoing support services. Social activities are built in, with resident events, music programs, outings, and group activities to keep things lively. There's transportation and parking for folks who need to get out for appointments or errands. Washington Square Healthcare includes things like legal protections for gender identity, orientation, and income source in their terms.
