    Small family-like community excellent care

I'm very pleased with this small, family-like community - staff are professional, hands-on, and genuinely attentive, and administration, especially Michael, is knowledgeable and responsive. My loved one gets excellent rehab and compassionate care, residents stay active with fun activities, the place is clean and welcoming, and the food is enjoyable; I would recommend it.

    Summary of reviews

The reviews present a strongly mixed picture of White Oak Healthcare. Many families and visitors describe an environment in which individual staff members and a visible administrator provide compassionate, personalized care; positive notes include skilled licensed nurses and LPNs, a familyโ€‘like atmosphere, active rehabilitation services, and a range of social activities. Several accounts highlight meaningful residentโ€“staff rapport, smallโ€‘facility advantages such as greater direct contact, personalized care plans, and improvements underway to the physical plant.

At the same time, a recurring set of operational weaknesses appears across reviews. Chronic understaffing and high staff-to-resident ratios are cited as a root cause for long wait times, delays in bathing and toileting assistance, and missed or delayed medication administration. These service delays are paired with sanitation and maintenance shortfalls in some areas of the facility, with specific concerns about lingering odors, pestโ€‘control issues, and inconsistent housekeeping coverage. Where staffing is limited, families report extended response times for routine and urgent needs.

Staff quality is described in polarized terms: several reviews praise individual caregivers and an engaged administrator for compassionate, aboveโ€‘andโ€‘beyond care, while others describe problematic staff conduct and poor communication tone. This inconsistency suggests variability in training, supervision, or staff retention across shifts. Management responsiveness is likewise mixed โ€” some families report timely, helpful customer service and strong leadership, while others describe poor afterโ€‘hours accessibility, unanswered calls on weekends and holidays, and instances where complaints were not adequately addressed. Reviews also identify variable clinical-testing practices and limited transparency about testing decisions and followโ€‘up, and there are allegations relating to theft and disputes over testing protocols that raise concerns about grievance resolution and clinical governance.

Dining and activities show similar variation. Multiple accounts praise enjoyable meals, active programming (bingo, music, crafts, gardening), and a calming social vibe. Other families report inconsistent meal quality and service. The facilityโ€™s physical environment is described both as attractive and clean by some visitors and in need of renovation and improved cleaning coverage by others, indicating uneven maintenance across units or over time.

Taken together, the pattern suggests a facility with notable strengths in individualized, compassionate care delivered by committed staff and leadership, yet uneven operational reliability. The most consistent actionable themes are staffing levels, medication administration controls, sanitation/housekeeping consistency, afterโ€‘hours communication procedures, and clearer clinicalโ€‘testing policies. Prospective residents and families may wish to conduct an inโ€‘person visit, ask for current staffing ratios and turnover data, review medicationโ€‘administration and testing protocols, confirm housekeeping schedules and pestโ€‘control measures, and verify afterโ€‘hours emergency contact procedures before making a placement decision.

    About White Oak Healthcare

White Oak Healthcare sits on Ridge Road in Bolindale, Ohio, and is a small stand-alone nursing home with 62 beds where you'll find a mix of skilled nursing, assisted living, independent living, and memory care services all under one roof, so if someone needs long-term residential assistance or short-term rehabilitation after an illness or surgery, they can stay here, and the staff-who seem to have a kind and supportive reputation-help with everything from walking and wheelchair support to medication and wound care, plus help with bathing and toileting; folks with diabetic or incontinence care needs aren't turned away either. Inspections happen at least every 15 months for food, safety, and care plans, and the facility takes Medicaid, making it possible for a range of residents to live here, whether they're looking for respite care, longer stays, or specialized memory care with 24/7 support and activities that help with memory and engagement. The building isn't attached to a hospital, but staffing includes nurses and aides around the clock, along with physical, occupational, and speech therapy, so both short-term rehab guests and residents get help aimed at regaining independence, with care plans tailored to each person.

Rooms come fully furnished, with private bathrooms, kitchenettes, air conditioning, safety features, cable TV, and washers and dryers nearby, while housekeeping, regular linen changes, and maintenance are included, and everyone gets access to nourishing, home-cooked meals that accommodate special diets, served restaurant-style, with snacks available if someone needs them. There's a sense that residents have some say in their routines and daily life, and folks spend time together in common areas like the dining room, game room, computer center, fitness room, small library, outdoor garden, and salon/barbershop, and Wi-Fi makes it possible to stay connected. Social events, trips, and resident-run activities fill up the calendar, so people get choices if they want crafts, exercise classes, or just to join a group in the garden, and if someone needs to get to a doctor's appointment or go on a community outing, transportation is provided. It's not a fancy place but is meant to feel home-like, welcoming a variety of residents, and focusing on comfort, safety, and kindness while offering both independence and support when someone needs it.
