    Excellent care, compassionate, clean facility

I placed my mom at Locust Ridge and I'm very happy with the care. Staff are consistently friendly, caring, and helpful - nurses, aides, housekeeping and administration go above and beyond; therapy is focused and effective, and the facility is freshly remodeled and very clean. The family-like atmosphere and clear communication from leadership reassured me, and I would recommend this facility.

    Summary of reviews

Locust Ridge Nursing and Rehabilitation elicits strongly mixed impressions. A substantial portion of feedback highlights compassionate, attentive staff who create a family-like atmosphere and support strong rehabilitation outcomes. Reviewers commonly note effective PT/OT services that contributed to functional improvement, proactive administrative and social-work assistance, and helpful Medicare guidance. Several comments single out specific staff and nursing leadership for hands-on involvement and responsiveness, and some areas of the campus (notably a recently remodeled therapy suite) receive consistent praise for cleanliness and upkeep.

At the same time, there are recurring operational concerns that prospective families should evaluate. Multiple accounts describe inconsistency in staff conduct and responsiveness across shifts, including issues with personal-care frequency (for example, bathing and room-check schedules) and wound-care follow-through. These concern patterns suggest variability in clinical-care continuity rather than a uniform practice across the facility. Second-shift coverage and occasional personality conflicts were cited as factors linked to uneven experiences.

Facility condition and dining receive mixed feedback. While housekeeping and portions of the building have been described as very clean and freshly updated, other descriptions indicate sanitation concerns and areas that appear run-down or in need of maintenance and aesthetic updates (bare walls and dated decor were mentioned). Dining quality also appears inconsistent; some families appreciated meals and even staff-provided accommodations, while others found the food service could be improved.

Management and communication present both strengths and gaps. The business office, social work team, and some administrators are credited as proactive and helpful, and routine communications such as newsletters have been valued. Conversely, there are reports of delayed postings or uneven information flow to families, which may reflect inconsistencies in administrative follow-through rather than an absence of outreach altogether. Reviewers also note an overall improvement trend under newer ownership/management, suggesting that some of the negative experiences may be linked to past conditions or transitional issues.

Recommendation: Locust Ridge may be a strong option when the priorities are compassionate direct care and active rehabilitation, but the variability described across reviews indicates it is important to verify current operational status in person. When touring, ask about staffing ratios by shift, wound-care protocols and documentation, bathing/room‑check schedules, recent maintenance or renovation plans for specific units, dining menus and food-service routines, and how the facility communicates updates to families. Observing multiple units and speaking with the therapy, nursing leadership, and business/social-work staff will help determine whether the facility’s strengths align with a prospective resident’s needs.

About Locust Ridge Nursing and Rehabilitation

Locust Ridge Nursing and Rehabilitation sits out on Elm Corner Road in Williamsburg, Ohio, in a single-story building that's been recently remodeled, and it offers both assisted living and nursing home care in a place that's easy to find and simple to get around, and folks living there can have either private rooms or spend time out in enclosed courtyards or enjoy the easy-to-use restrooms and on-site parking, and the whole building is air-conditioned for comfort during warm months, which is important around here. This facility specializes in care for seniors who need skilled nursing, long-term care, and help after a hospital stay, and they provide both short-term rehabilitation and long-term living options, as well as respite care for folks just needing a short break. There are nurses-registered nurses, licensed practical nurses, and certified nursing assistants-on staff around the clock along with therapists for physical, occupational, and speech therapy, and they also handle wound care, palliative and hospice care, and post-operative recovery, always making sure someone is available, day or night. The community puts a special focus on Alzheimer's and dementia care, so they have wander guard alarms on doors to keep residents safe, along with secured behavioral and mental health units for those with more challenging needs, plus a team experienced in memory care that knows how to help during difficult times. Locust Ridge offers plenty of amenities, like in-house smoking areas, an on-site salon, restrooms accessible to people with physical disabilities, enclosed courtyards, and even offers transportation options for spouses who can't get out on their own. Meals come from dietician-recommended menus, and there are weekly outings, social events, and activities planned to keep folks engaged, along with in-house specialists for podiatry, optometry, dental needs, and more, so residents don't have to travel for appointments unless they want to. The staff-a team with a long track record here-tries to make sure the place feels like a family, and many stay for years, which makes the community feel familiar and welcoming. Locust Ridge holds a deficiency-free rating from the Ohio Department of Health and is licensed for Medicare Services Part A and Part B. With a rating of 4.3 stars based on 45 reviews, the facility's known just as much for its steady atmosphere as it is for its wide range of services, and visitors will find comprehensive, personalized care plans for both quick recovery and ongoing needs, and a structured routine that gives a sense of security. The site is open for tours and visits, with good parking and all the conveniences you'd expect from a community-based care facility, and every effort's made to foster a comfortable, safe setting that values both expert medical care and opportunities to connect with others.
