    Warm attentive staff excellent therapy

I placed my mom here and we're very pleased. The staff are genuinely caring, attentive and trustworthy; the facility is clean and home-like, the food is above average, and physical therapy is excellent. Communication and hospice coordination (timely meds, attentive care) have been reliable, the administrator is hands-on, and mom has made friends and settled in - a warm, supportive place I recommend.

    Summary of reviews

Overall impression: Reviews reflect a mix of strong clinical and interpersonal strengths alongside persistent operational weaknesses. Many families praised the clinical care teamโ€”particularly nursing, hospice coordination, and the rehabilitative therapy programโ€”while other feedback highlights variability in day-to-day operations such as staffing, cleanliness, and maintenance.

Care quality: Clinical care elements received consistent positive comments. Families described timely pain-management in coordination with hospice, routine pressure-area prevention practices, and an effective physical-therapy/rehabilitation program. At the same time, there are repeated notes about uneven on-floor care: delays in responding to needs, inconsistent rounding, and occasional failures in callback or follow-up communication that affect perceptions of reliability.

Staff and management: Staff are frequently characterized as caring, welcoming, and skilled; several notes highlight nurses and aides who go beyond basic duties and an administrator who is visibly engaged with residents and families. However, reviewers also described variability in individual caregiver conduct and performance. Understaffing is cited as a contributing factor to inconsistent responsiveness and occasional lapses in routine care. Some families also described friction or difficulty with referral and intake processes.

Dining and activities: Dining received generally favorable mentions, with comments about above-average food and productive mealtimes. Social programming and peer connections are strengths for many residents: reviewers noted a family-like atmosphere, social engagement, and successful transitions that supported residentsโ€™ well-being.

Facilities and cleanliness: Physical-condition concerns are prominent. Reviewers described an aging exterior and interior, visible wear, and uneven housekeeping standards. Specific operational sanitation issues include odor concerns in rooms and common areas and instances of equipment or room cleanliness that did not meet expectations. Room layout is another practical constraint: some rooms are small and share bathrooms, which may limit privacy and storage for personal items.

Notable patterns and guidance: The dominant pattern is variabilityโ€”strong clinical and therapy services coexist with recurring operational issues (staffing consistency, maintenance, sanitation, and property management). Prospective residents and families should plan an in-person visit that includes observing a mealtime, speaking with the therapy team, asking about staffing ratios and shift coverage, reviewing laundry/property policies, and discussing cleaning and infectionโ€‘control protocols. Confirming administrator engagement and clarifying the referral/intake workflow can help set expectations and identify whether the facilityโ€™s strengths align with the residentโ€™s priorities.

    About Southbrook Healthcare

Southbrook Healthcare sits at 832 Isabel St in Ardmore, Oklahoma, and has been taking care of residents for over 45 years, always making sure there's registered nurses on-site every day and doctors on call around the clock, so people get the help they need any time, day or night, and the facility's also got all its beds certified for both Medicare and Medicaid, so folks can feel sure about getting covered care. Operated by Elmbrook Management Company, which runs several other nursing homes and an assisted living center in southern Oklahoma, Southbrook Healthcare holds a place as a skilled nursing facility where you'll find inpatient nursing, long-term care, and assisted living, so folks can move from short-term rehab to more long-term support as their needs change, and they keep the admissions open around the clock, which makes transitions a little easier. People here can get specialized nursing assistance, support for Parkinson's, Alzheimer's, and other dementia, along with help from Certified Medical Assistant Annica Stone and Director of Nursing Claudia Motal, all under the primary oversight of administrator Craig Kelley, plus there's always skilled care staff available and physicians checking in on the skilled nursing patients at least twice a week.

People who need physical, occupational, or speech therapy can use the fully equipped rehab unit, which has modern therapy tools, and a screening process starts right at admission to look over what each person might need; some stay short-term to recover after a hospital visit before heading home, while others live here longer and get daily support. The nurses help with personal care, pain management, and specialized tasks like dialysis or hospice care, and staff build out treatment plans tailored for every resident, rather than taking a one-size-fits-all approach. Amenities are practical-hospitality services, comfortable common lounging areas, private rooms for special family events, restaurant-style dining reviewed by a registered dietitian, daily activity programs like music, fitness, trivia, games, and even volunteer visits, and you'll notice there's also a "Send a Greeting" service for mail delivery so residents keep connections alive. Social services offer help for both residents and families, and you'll notice they want to encourage residents to keep their dignity and independence as much as possible, with a schedule full of choices, outdoor gatherings, and even a resident council where folks can voice their opinions or take part in planning. Family visits are welcome every day from 7 am to 9 pm, and people can stay later with a heads-up, plus prospective families can always set up a tour to see things in person for themselves.
