    Attentive compassionate staff and care

I was very pleased with the attentive, compassionate staff and the good care my father received. They always greeted us with smiles (they even attended his service), the activities director was personable, and residents were friendly. I would recommend this community.

    Summary of reviews

Overall impression Reviews present a mixed picture. Several families praised individual caregivers for being friendly, personable and compassionate, and reviewers noted an active activities program led by a personable activities director. At the same time, a set of serious operational concerns recurs across accounts: understaffing, lapses in basic personal-care routines, and failures in clinical monitoring and escalation. Care quality Positive comments emphasize attentive bedside interactions and examples of staff going beyond routine duties to support families. However, other accounts describe inconsistent delivery of basic care tasks and delayed recognition of clinical deterioration. These patterns point to variability in day‑to‑day care consistency rather than uniformly poor or uniformly excellent performance. Prospective families should assess current staffing ratios, staff mix (licensed nurses versus aides), and written care-plan enforcement when considering placement. Staff and management Staff members are often described as courteous and compassionate, and there are examples of good family engagement. Conversely, multiple accounts describe slow or unsatisfactory administrative responses to incidents, limited follow-up communication with families, and concerns about billing transparency after a resident’s death. These suggest weaknesses in incident-response protocols, family communication pathways, and financial-administration practices that are worth reviewing directly with leadership. Safety and clinical systems Patterns in the reviews raise concerns about safety systems: insufficient supervision that may increase wandering/elopement risk, inconsistent medication‑management processes, and variable clinical decision‑making around transfers to higher levels of care. There are also references to delayed escalation that resulted in hospital transfers. These point to gaps in monitoring, escalation criteria, and transfer coordination rather than isolated interpersonal issues. Activities, dining, and facilities The activities program receives consistently positive mention, led by an engaged activities director and supported by social interaction among residents. There is limited information about dining quality in the available summaries. Facility-level comments include sanitation and personal-care inconsistencies; families should inspect common areas and resident rooms in person and ask about cleaning schedules and incontinence-care protocols. Notable patterns and guidance Two themes stand out: (1) staff-level strengths in interpersonal care and activities programming, and (2) operational weaknesses in staffing, monitoring, incident response, and administrative transparency. For decision-making, families should request current staffing ratios and turnover rates, review incident-response protocols and examples of how the facility handled recent clinical escalations, ask for documentation of rehabilitation intensity and goals, and obtain a clear, written explanation of billing policies. If safety or intensive therapy needs are primary concerns, consider verifying the facility’s capacity to meet those needs before placement.

    About Brookside Nursing Center

Brookside Nursing Center, located at 310 Brookside Dr. in Madill, OK, is a skilled nursing and long-term care facility that's been serving the community since 1964, and you'll find a range of care types here including skilled nursing, rehabilitation, Assisted Living, Independent Living, Memory Care, Continuing Care, Home Care, and Respite Care, so people with different medical needs can get support under one roof. The facility has 140 certified beds and offers skilled services such as medication management, wound care, inpatient rehab, and even specialty services like dermatology and long-term acute care hospital support, and all care is aimed at the resident's specific needs-there's a focus on providing person-centered individualized care that starts from the day you arrive with a rehabilitation screening.

Here, resident well-being is important, and the staff, led by administrator Leigh Bearden and supported by Nola Jones in administration, work to provide nursing, rehabilitation, and social support, while also being available for referrals around the clock, so families can reach out any time. The nursing turnover rate is a bit above average at 74.5% compared to the state average of 56.5%, and nurse hours per resident per day are at 3.52, which is just under the state average of 3.8, so that's something to consider, and the center has had 11 documented deficiencies in inspection reports, including issues with Resident Assessment and Care Planning, but they continue to focus on improving care coordination and planning for discharge to meet residents' needs.

The center's amenities are modern and focus on comfort, with restaurant-style dining overseen by a registered dietitian, fresh and nutritious breakfasts each day, along with coffee, tea, and other beverages available throughout. Private spaces can be reserved for special events, and mail gets delivered daily except on weekends and holidays, so residents keep connections with loved ones. Brookside encourages social and recreational activities, offering musical performances, fitness and stretching, current events reading, religious meetings, trivia, games, outside entertainment, and active resident councils, plus opportunities for volunteer involvement to keep everyone engaged.

For those needing memory care, there's dementia care services and a secure environment, and the therapy staff provides physical, occupational, and speech therapies for those recovering from illness or injury. Scheduled tours let families see the facility, meet the staff, and even enjoy a meal to get a sense of the community, and visiting hours are recommended from 8 a.m. to 8 p.m., with 24-hour access if arranged in advance. Owned fully by Tom Coble and affiliated with Elmbrook Management Company, Brookside Nursing Center makes information about its services available through a user-friendly online interface, so people can view photos, schedule tours, or request information easily, and if you need to make a special referral, Kyla Reinhart handles those by fax. The staff is friendly, many are long-serving, and the facility stays focused on maintaining dignity, independence, and comfort for every resident through their personalized nursing program and care that adjust as needs change.
