    Caring staff and excellent therapy

I've been very pleased with the caring, knowledgeable staff and an accommodating administration who really listen-especially helpful with Alzheimer's care. The clean, updated facility and pleasant courtyards, excellent rehab/physical therapy, tasty food, variety of activities, and strong family support make me feel safe and comfortable. Very satisfied overall; a few small tweaks would make it perfect.

    Summary of reviews

The reviews reflect a facility with clear strengths and recurring operational weaknesses. Many families praised individual caregivers, the rehabilitation program, and the appearance of the facility and grounds; at the same time a number of persistent concerns about daily care processes, cleanliness, and safety were described. Overall impressions are mixed, with recent management and staffing changes noted as producing improvements by some families.

Care and staffing: Several reviews highlighted compassionate and helpful staff members and staff with dementia-care knowledge, and the physical therapy/rehab program received consistent positive comments. However, a recurrent pattern involves inconsistent staffing levels that contribute to delays in personal-care tasks (bathing, repositioning, assistance with transfers) and longer call-response times. Reviewers described risks to skin integrity and wound-prevention practices when repositioning and bathing schedules are not consistently followed, and some accounts raised concerns about nutrition and hydration management and timely clinical attention after incidents.

Dining and activities: The facility's activity program and ancillary staff are frequently cited as strengths — bingo, church services, outings, a proactive activity director, and a responsive dietician were noted. Several families reported good meal quality and accommodating dietary staff. Conversely, there are intermittent complaints about meal delivery and temperature, and at times service continuity has been inconsistent.

Facilities and cleanliness: Multiple reviewers praised recent remodeling, well-kept courtyards, and attractive common areas. Counterbalancing this, there are sanitation concerns in certain rooms and bathrooms, reports of odor issues in areas, and isolated pest-control concerns. Shower facilities and some resident rooms were described positively, but cleanliness and maintenance appear uneven across the building.

Management, communication, and safety: Admissions and tours are generally described as welcoming, and some families observed positive changes under newer management. At the same time, several reviews cite inconsistent communication from administration, unresolved property and billing issues, and concerns about property security and missing belongings. Infection-control practices and mask use were highlighted as inconsistent in some accounts. Safety processes such as fall prevention, transfer techniques, and oversight during high-acuity needs were identified as areas needing attention.

Notable patterns and considerations for families: The overall pattern is mixed — strong rehabilitation services and some very dedicated staff coexist with operational gaps in staffing, hygiene assistance, cleanliness, property control, and certain safety processes. Prospective residents and families should ask about current staffing ratios, documented routines for repositioning and bathing, fall-prevention protocols, infection-control policies (including mask policy), property-security procedures, and recent corrective actions taken by management. Visiting at different times of day and requesting recent facility performance or inspection records may help assess whether reported improvements have been sustained.

About The Fairmont Skilled Nursing & Therapy

The Fairmont Skilled Nursing & Therapy sits in Oklahoma City and is a nursing home known for a warm, home-like feeling with attention aimed at every resident, trying hard to help folks feel at ease with their own needs, and you'll find staff there who put effort into making life better for people, giving support with patience and care. The facility's got space for up to 125 beds, with an average of about 106 residents each day, and folks live in a place run by admins like Laquita Christon and managed by Amity Care, LLC since June 2019, with Michael Dimond and Kenneth Greiner as key managers, and you'll find ownership split between the Don Greiner Trust and Kenneth Greiner, making it a for-profit partnership that falls under Grace Living Centers. The Fairmont Skilled Nursing & Therapy offers all sorts of services folks might need in later life, including skilled nursing care, physical, occupational, speech, and respiratory therapies, along with a therapy gym and a restorative program for those coming from hospital stays after strokes, joint replacements, or respiratory illnesses, and certified therapists help residents regain movement and independence where they can.

There's help for people with Alzheimer's through a memory care unit, and the facility provides cognitive rehabilitation, wound care, respite care, and care for those needing help with medicine or daily self-care, offering long-term care and short-term rehabilitative options alike, and laundry, housekeeping, and dietary services are part of the package. Several outdoor courtyards encourage fresh air and community, and the staff aims at patient-centered care while also supporting family caregiving and guidance about long-term care insurance. The place is focused on care but there have been problems, like 17 deficiencies found over past inspections with $12,735 in fines, some related to issues of abuse, neglect, or exploitation, though most weren't immediately threatening, and the nurse turnover rate is a bit higher than the state average, with 3.75 nurse hours per resident each day, which is a little under what's common statewide. The Fairmont lists 19 reviews online, has a steady routine with staff and nurses on duty, offers specialized care plans for folks' rehab needs, and maintains its mission to serve with compassion and dignity, providing a comprehensive support system without a lot of fuss.
