    Clean cozy facility, outstanding rehab

I'm very pleased with the extremely clean, cozy facility, friendly and professional staff (Liz and Dow stood out), and the welcoming Christian atmosphere. The food, activities and outstanding rehab therapy helped my dad return home independently; service dogs are welcome and Odie brightened visits. A small caveat: puppies aren't permitted, but I still highly recommend this place.

    Summary of reviews

The collected reviews present a polarized picture of Oaks Healthcare Center: several families describe high-quality, rehab-focused care and a clean, comfortable environment, while others describe operational concerns that have tangible effects on resident experience. Strengths most often cited include an active therapy program with measurable recovery outcomes, a pleasant physical environment, and staff members who are friendly and professional in many interactions. The facility's acceptance of service animals and the presence of a welcoming therapy dog were repeatedly noted as positive contributors to resident well-being. Multiple reviewers also praised meal variety, accommodations, and a faith-oriented, inviting atmosphere.

At the same time, systemic operational issues recur across the feedback. Understaffing and frequent staff turnover are prominent themes and are associated with delays in basic clinical monitoring and medication administration. Several accounts describe missed vital-sign checks and difficulties obtaining timely analgesia or other medications; these point to weaknesses in clinical oversight and medication-management processes rather than isolated events. Relatedly, reviewers raised concerns about insufficient resident supervision and limited socialization or activity engagement, which some families felt left residents isolated for periods.

Communication and management performance are mixed. Some families reported that administrators and nursing leadership were responsive and actively working to improve service, and a number of reviewers recommended the facility based on positive resolutions. Conversely, other families experienced poor communication about visitation hours, difficulty getting clear answers from staff, and instances they characterized as unprofessional conduct. There are also mentions of privacy and confidentiality process gaps and concerns about corporate governance or financial priorities influencing operations.

Dining and facility upkeep show both positive and negative signals. Many reviewers remarked on a clean, updated building and pleasant common areas, but there are also isolated sanitation and dining-hygiene concerns that warrant verification during a visit. Prospective residents and families should consider the polarized feedback as an indicator to ask targeted questions: current staffing levels and turnover, staffing ratios on different shifts, clinical-monitoring protocols (vitals, medication administration), supervision and activity programming, visitation policies, complaint-resolution procedures, and infection-control/sanitation checks.

In summary, Oaks Healthcare Center demonstrates clear strengths in rehabilitation services, facility cleanliness, and caring staff interactions in many cases, while simultaneously showing operational gaps—chiefly around staffing, clinical monitoring, supervision, and consistent communication—that have materially affected some residents and families. A direct, focused tour combined with specific questions about staffing and clinical safeguards will help determine whether the facility’s current practices meet an individual resident’s needs.

    About Oaks Healthcare Center

Oaks Healthcare Center sits in Poteau, Oklahoma and has 158 certified beds, and it offers a wide range of care options so folks can find what fits them best, like independent living, assisted living, memory care, home health care that's Medicare-certified, and hospice care, along with non-medical home care and adult day services for those that want to stay connected but still need help. The staff does skilled nursing and long-term care, and the nursing team averages about 3.43 nurse hours per resident per day, which means someone's usually around to check on you, and if you need therapy or skilled rehab services, they work with you right there. They have outdoor courtyards for folks who like a little fresh air, and the common areas feel warm and welcoming, not hospital-like, which helps everyone relax a bit and visit with others. Oaks Healthcare Center's got a clear focus on community, kindness, and open communication, where family, faith, and relationships matter and the staff talks about guiding everyone with love, joy, and peace, calling it the "Fruit of the Spirit," which some people find comforting.

Norma Cox keeps things running as the administrator, and Crystal Hilton serves as Director of Nursing, with Dow Wilson named as secondary administrator, and the center works with Care Providers Oklahoma for daily operations. Bedlam Properties LLC, Matrix Provider Solutions LLC, and Mark Lietzke have managed the place since December 2024, and ownership comes through the Grant Rhodes Revocable Trust and Jeffrey W Young Revocable Trust, each holding half, with indirect involvement from Jonathan Rhodes and Jeffrey Young. Oaks Healthcare Center operates as a for-profit company, and they say they're dedicated to creating a workplace where professional, personal goals and family time are respected for the staff, and that they're committed to being easy for residents and families to reach and talk to if they need anything.

The center also posts pictures of their spaces and amenities, so people can get a look before they visit, and they invite people to see the photo gallery for more details. The facility has faced several health and safety citations, including 7 infection-related deficiencies and 11 total deficiencies noted on an inspection in February 2025, with issues like infection control, providing and documenting COVID-19 vaccines, and following food safety rules. While Oaks Healthcare Center offers a large number of services and works to create a calm, supportive environment, families should review the most recent inspection reports to understand these challenges.
