    Attentive, compassionate care; spotless facility

I've been very pleased with the spotless facility and genuinely caring, responsive staff - nursing, therapy and the front office all go out of their way and even know residents by name. New management has been excellent, patient with questions and quick to fulfill requests; activities, outings and strong rehab support really improved my loved one's quality of life. It's a welcoming, supportive community and I would recommend it for attentive, compassionate care.

    Summary of reviews

The reviews of Colonial Manor Nursing Center describe a facility with polarized experiences: many families and residents praise compassionate caregivers, effective short-term rehabilitation and therapy services, and an engaged activities program, while other accounts raise substantive operational concerns. Positive comments commonly cite staff who know residents by name, supportive front-office personnel, weekly housekeeping in private units, hospice integration, and occasions of high-quality meals prepared by committed cooks. These strengths indicate the facility has functional clinical capability and community programming that benefit some residents, particularly for rehab stays and families who observe consistent staffing.

Conversely, a recurrent theme is inconsistency. Staffing levels and turnover are frequently cited as sources of variability in day-to-day care and responsiveness. Reviewers describe long response times, missed clinical checks or medications, and uneven documentation practices โ€” including advance-directive and DNR paperwork gaps โ€” that have, in a few instances, led to emergency interventions and significant family distress. There are also multiple mentions of variable staff conduct and communication tone, suggesting staffing stability and supervision are ongoing operational challenges.

Housekeeping and the physical environment emerge as mixed. Several accounts praise clean, well-kept rooms and portions of the facility, while others describe sanitation and odor concerns, inconsistent room upkeep, and areas that appear dated or in need of further remodeling. Dining impressions are likewise inconsistent: some residents report excellent lunches and cooks who take pride in meals, while others note small portions and uneven food quality.

Management and administration receive both commendation and criticism. Some reviewers state that newer management and owner involvement have led to measurable improvements and timely responsiveness; others describe scheduling issues, poor phone responsiveness, and a perceived lack of administrative accountability. These contrasting reports suggest recent leadership changes and uneven implementation of policies across units.

Activities and community life are generally described positively, with organized outings, faith-based services, and daily programming noted as beneficial to quality of life. Short-term rehab guests and apartment residents most often highlight therapy access and weekly housekeeping as advantages. However, families considering long-term placement should weigh activity availability against the facilityโ€™s variability in staffing and housekeeping.

Notable patterns for families and prospective residents: the facility shows capacity for excellent care when staffing and management are stable, but there are documented operational weaknesses that can materially affect safety and comfort when those systems falter. During a tour or decision process, ask specific questions about current staffing ratios, medication-administration audits, advance-directive handling, housekeeping schedules, recent ownership or management changes, and recent state inspection results. Observing mealtime service and speaking directly with nursing leadership about incident protocols can help clarify whether current conditions meet your expectations.

    About Colonial Manor Nursing Center

Colonial Manor Nursing Center sits in Tulsa, Oklahoma at 1815 E Skelly Dr and offers a range of care for older adults, and the place has a long history of serving seniors who need different kinds of help, so there are options like independent living for active seniors, assisted living, memory care, and skilled nursing services for those who need more support, and this is also a nursing and rehab center with both long-term and short-term care. People can get around-the-clock assistance with daily activities like bathing or dressing, and registered staff give regular nursing and medical care, so there's always someone there to help or answer a call for help day or night, and that includes skilled nursing, wound care, physical therapy, occupational therapy, speech therapy, dentistry, and other specialized services. The memory care areas are secure and designed to be dementia-friendly with special programs and memory activities for people living with Alzheimer's or other forms of dementia, and the staff provides support to help these residents feel safe and comfortable.

The community does its best to keep things lively by offering both planned activities and trips as well as events run by the residents themselves, so folks can join things that interest them, make friends, and stay engaged whether they want to join a club, read a book in the small library, keep up with a fitness routine in the exercise room, or get some fresh air in the garden and outdoor space, plus there's a wellness center aimed at helping both body and mind. Residents can live in fully furnished rooms that come with private bathrooms, kitchenettes, cable, air-conditioning, telephone, and Wi-Fi, and the independent living spaces often include on-site meal programs with one to two meals a day, and some housekeeping and laundry services may be included or offered for a fee. There's also a dining room, beauty salon, salon, computer center, gaming room, activity spaces, and transportation services for doctor appointments or outings.

Colonial Manor Nursing Center has 120 certified beds with about 53 residents on an average day, and the nurse staffing averages 3.39 nursing hours per resident every day, but nurse turnover is pretty high at 63%, which is something to keep in mind, and the caregiver-to-senior ratio meets state licensing rules. They take Medicaid, Medicare, social security disability, and some federal and state programs for residents who qualify.

Facility inspections gave Colonial Manor Nursing Center an A- for quality, and while the center works to follow all safety and infection-control standards, reports over the years have found 11 deficiencies, and some of those relate to protection from abuse or neglect, infection control, and nutrition standards, so while a lot of support's provided, it's good to be aware of the oversight points and the areas being worked on.

Colonial Manor Nursing Center belongs to the Diakonos Group and is operated as a for-profit Limited Liability Company, and it works with organizations like A Place For Mom. The community has a rating around 3.0 based on 29 reviews, and besides basic nursing care, there are services like respite stays, inpatient rehab, nutritional counseling, help with medications, pharmacy, feeding assistance, and respiratory therapy. Residents and families can take part in councils to share concerns or ideas, and the staff aim to create a supportive, caring place where each person can live as safely, comfortably, and independently as possible, and even though the facility isn't without its challenges, it has served Tulsa area seniors for many years by offering a wide variety of services to meet different needs.
