    Caring staff; mom is painting

I'm grateful for this caring, home-like facility - the nursing and support staff are friendly, kind, and hardworking. I receive daily phone updates from the care team, my mom is happy and even painting again, and the rooms are clean. I'm very impressed and thankful; a few small improvements would make it perfect.

    Summary of reviews

Overall impression: Reviews present a mixed picture. Many families express appreciation for frontline caregivers and the day-to-day experience of residents, while others raise substantive concerns about facility leadership, policies, and staff conduct. The positive comments focus on direct care, communication, cleanliness, and activities; the critical comments focus on management decisions and operational transparency.

Care quality and staff: The strongest theme in the positive feedback is the quality of direct caregiving. Reviewers frequently describe nursing and support staff as compassionate, attentive, and friendly; families note a coordinated care-team approach and consistent, daily phone updates. These reports indicate responsiveness to family communication needs and a level of interpersonal care that contributes to residents’ satisfaction. At the same time, a number of reviewers raised concerns about staff conduct and property security, including serious allegations of theft. Those concerns undermine trust for some families and suggest the facility may need stronger controls and clearer incident-handling protocols.

Dining and activities: There is limited specific commentary about dining in the provided summaries, so no clear pattern emerges regarding food quality or meal service. Activities receive positive mention; several reviewers appreciated creative and engagement opportunities (for example, art/painting), and described rooms and common areas as home-like and well maintained. Cleanliness of resident rooms and a comfortable environment are recurring positives.

Facilities and visitation: Physical facilities are described as clean and home-like, which families valued. At the same time, several reviewers highlighted issues with the visitor experience: visitor restrictions, enforced mask/face-covering policies described as strict, restricted or confusing front-entrance access, and a lack of clear signage or guidance for visitors. These operational elements appear to be a source of friction and confusion for families and should be addressed through clearer signage and visitor communications.

Management and policies: Management and leadership are the most frequent source of negative commentary. Reviewers cite perceived inexperience or poor decision-making by leadership, concerns about rehiring practices and personnel decisions, and inconsistent adherence to infection-control policies (notably around COVID protocols). Some families expressed legal and rights-related concerns tied to these operational choices. There are also mentions of limited insurance acceptance (specifically Humana), which may affect placement decisions for prospective residents.

Notable patterns and recommendation: Prospective residents and families should weigh strong reports about day-to-day caregiving, cleanliness, and activity programming against reported management and policy issues. The positive reports suggest a facility capable of providing attentive, person-centered daily care, while the operational concerns point to risks in leadership consistency, visitor access, and internal controls. Families considering placement should ask facility leadership for specifics on visitor policies, infection-control protocols, staff hiring and monitoring practices, property-protection measures, and accepted insurance plans, and should request references from current families to corroborate recent experiences.

    About Vian Nursing & Rehab

Vian Nursing & Rehab sits in Vian, Oklahoma at 305 North Thornton and offers a variety of care services for seniors who need skilled nursing care after an illness, surgery, or hospital stay, and whether someone needs help for a short time or needs long-term care, the facility has staff ready to help with daily activities like bathing, dressing, and taking medicine, and with its for-profit Limited Liability Company ownership, it's designed for patients needing extra support, and it's got 73 certified beds available out of 133 as of June 2025, with Medicare and Medicaid coverage. People here get 12-16 hour nursing care each day, a 24-hour call system in case someone needs help fast, and access to medical professionals who create personalized care plans that pay close attention to health changes, including wound care and medication management, plus specialized therapy services like physical, occupational, and speech therapy. For those who can't get around on their own, the staff is trained for safe transfers and tailored support, and people who need palliative care or respite care for caregivers can get that, too. The facility includes a spa and sauna for anyone who wants to relax, a fitness room and wellness programs for staying active, and outdoor gardens and walking paths with common spaces for socializing or just sitting outside in good weather, which some folks like to do, especially during the afternoons, and when it comes to daily life, residents lead activities themselves, and you'll find music programs, movie nights, arts and crafts in an arts room, a game room, and a library where folks often spend their afternoons reading or chatting, and there are plenty of activities sponsored by the local community, which helps people stay involved and connected. Staff is careful to give each resident a plan that fits their own needs, and the team works to make the community feel welcoming and safe, making sure folks get the right help with moving around, therapy, and keeping up with daily routines, so each person gets the support they need, however basic or complex that support may be. Renovations and improvements in the recent past have helped keep the spaces comfortable, while also making the outdoor and indoor environments easier to enjoy, so those who live there can find comfort whether they want to join in activities or just relax and recover at their own pace. Information about Vian Nursing & Rehab is a little limited online, but people looking for specific floor plans or payment details can call to learn more, and many families choose this facility because the staff provides care with attention and respect, helping residents feel at home while getting help with whatever health challenges they face.
