    Compassionate expert respiratory recovery care

I'm very pleased with this facility: clean, welcoming, and staffed by caring, knowledgeable professionals. Nurses Tiffany and Jen, respiratory therapist Ernst, office manager Precious and the therapy team went above and beyond - they helped my loved one recover (weaned from trach and feeding tube), provided 24/7 respiratory support, and kept us well informed. Engaging activities, excellent communication, and compassionate care made us feel safe and supported.

    Summary of reviews

Overview: Reviews describe a facility with clear clinical strengths alongside operational inconsistencies. Many families and former residents praise the caregiving team, respiratory services, and rehabilitation programs; at the same time a subset of accounts raises concerns about timeliness of clinical response, communications, and certain aspects of facility maintenance. The overall pattern is one of strong specialty care (particularly for respiratory and ventilated patients) paired with variability in day-to-day operational reliability.

Care quality and clinical services: The center is repeatedly identified for its respiratory expertise and capacity to manage ventilator-dependent residents, including 24/7 respiratory-therapist coverage and short-term rehab availability. Multiple reviewers describe successful clinical outcomes such as progress in weaning from tracheostomy and feeding support and meaningful mobility gains with physical therapy. Conversely, there are operational concerns related to timeliness: examples include delayed diagnostic testing, delays initiating antibiotics or other treatments, and reports of slow escalation or transfer decisions. One serious allegation concerns wound and infection-management follow-up; this is best treated as an item for verification during a direct inquiry with facility leadership and regulators.

Staff and communication: Staff are commonly characterized as warm, compassionate, and professional; specific aides, nurses, and managers receive named commendations for bedside care and family communication. Many families describe frequent consultations and clear updates. However, reviewers also report inconsistent communication practicesโ€”difficulty reaching staff by phone, delayed notifications about status changes, and variable responsiveness to call bells. These issues appear tied to staffing variability and shift-to-shift differences in coverage rather than to uniformly poor attitudes.

Activities, dining, and therapy: Recreational programming is a clear strength: reviewers note a range of activities (games, outings, fashion shows), meaningful engagement, and active therapy teams. Therapy staff receive positive mentions for contributing to recovery and functional improvement. There is limited, largely positive comment about dining and food quality in the reviews; specifics on menu variety and meal service patterns are scarce. Several reviewers praised smooth financial and admissions processes, which can ease transitions for families.

Facilities and management: The facilityโ€™s physical environment is often described as clean, odor-free in common areas, and set in attractive grounds. At the same time, multiple comments indicate parts of the building or equipment are dated and would benefit from updates; occasional reports of nonfunctional room items (television, heater) point to maintenance follow-through as an area for improvement. Management receives mixed assessments: some reviewers commend strong coordination, adherence to infection-control protocols, and effective leadership during COVID-19; others describe gaps in oversight that correlate with communication and timeliness problems.

Notable patterns and guidance for families: The most consistent strengths are respiratory specialization, skilled rehab services, and a caregiving staff that many families find compassionate. The most consistent weaknesses are operational: variable staffing levels, inconsistent responsiveness, delays in diagnostic/treatment processes, and spotty family-notification practices. Prospective residents and families should tour the unit, observe staffing at different times of day, and ask specific questions about monitoring protocols for high-acuity residents, wound and infection-management procedures, average response times to call bells, diagnostic and transfer escalation pathways, and how therapy schedules are maintained. Verifying recent inspection results and asking management for examples of how the facility has addressed past communication or timeliness issues can provide additional context for decision-making.

About Montgomery Subacute and Respiratory Center

Montgomery Subacute and Respiratory Center is a 60-bed skilled nursing facility located in Plymouth Township, northwest of Philadelphia, with easy access from Pennsylvania, New Jersey, Delaware, and other nearby areas, which means families have a few different ways to get there without too much trouble, and the center puts a strong focus on specialized care for folks with respiratory needs, handling things like ventilator management, pulmonary rehabilitation, and hemodialysis, while also offering wound care and dementia care, and it's received national recognition for quality care, including the Bronze - Commitment to Quality Award from AHCA/NCAL, but even so, the facility has had 22 inspection deficiencies, including citations for not always telling residents or families about issues right away, not always protecting residents from wrongful use of their belongings or money, and one infection control violation, which is something people should weigh when considering a place. The center runs under Marquis Health Consulting Services, operating as a for-profit LLC, and Marquis Limited LLC has been involved since December 2022, which brings experience in managed care, and ongoing support from Marquis Health Services. The average daily census runs about 54 residents, and the nursing staff provides about 5.28 hours of care per resident per day, though the nurse turnover rate is around 34.9%, and there's on-site staff to supervise and provide immediate care. There's an experienced leadership team with a Director of Nursing, a Human Resource Director, and the facility recently added Dr. Pandya, a pulmonologist, to support the specialized respiratory and ventilator care programs. Montgomery offers a range of services like short-term rehab for those coming out of the hospital, long-term and intermediate care for frail residents, as well as subacute care and comprehensive rehabilitation, so there's a bit of everything from basic post-acute recovery to more complex respiratory therapy, including Urgent SNFโ„ข for 24/7 stabilization, Care Navigation, Rehabbing Careโ„ข, and a continuum of care options for those needing help in between the hospital and returning home. The building has been set up with amenities aimed at making recovery more comfortable and feels a bit homey, which some residents may appreciate. While the facility has shown commitment to quality through national awards, families should also consider the facility's past deficiencies and staff turnover when making decisions. Montgomery Subacute and Respiratory Center accepts Medicare, Medicaid, and most insurances, and more information about their programs or services can be found on their website.
