    Excellent rehab and compassionate staff

I stayed at Brookside for rehab and was very pleased - the PT/OT (including in-room therapy) helped me regain strength and speech, and staff were professional, kind, and consistently communicative. The facility is clean and attractive, with helpful social services, activities (monthly church service, Friday luau, library) and strong hospice/support during treatment - I felt well cared for and would recommend returning.

    Summary of reviews

Brookside Healthcare elicits a strongly mixed set of impressions. Many families and patients praise bedside caregivers and therapy staff: physical and occupational therapy are frequently described as effective and instrumental in recovery, and multiple accounts emphasize compassionate, attentive nurses and aides who provide individualized rehabilitation and supportive end-of-life care. Social work and some admissions personnel are also named as helpful, and the facility offers organized activities (including regular social events and worship outreach) and coordination with outpatient services such as dialysis. The facility holds a CMS five-star rating, and several visitors commented on attractive common spaces and well-run therapy programs.

At the same time, a consistent pattern of operational concerns appears across reviews. Medication management is a recurrent issue, with examples of delayed or missed doses and abrupt medication changes that families found concerning; these point to systemic weaknesses in medication-administration and education processes. Reviewers also describe inconsistent clinical assessment and monitoring in some cases, raising questions about care-plan adherence and early recognition of acute changes. Response times to call bells and assistance requests are reported as variable, and some families describe inconsistent bathing and restroom-assistance practices, which suggests gaps in routine care responsiveness.

Cleanliness and sanitation emerge as another area of division. While parts of the building and common areas were described as clean and attractive, other comments cited sanitation concerns in resident rooms, bathrooms, and corridors, as well as intermittent odor concerns. Dining is similarly mixed: several accounts praise the food and meal support during therapy, while others describe unappealing menu options and inconsistent food quality. Activity programming and social engagement are generally identified as strengths when present, with specific events and in-house programming noted positively.

Management and staffing trends are notable contributors to variability in experience. Reviewers described periods of high staff turnover, reliance on agency personnel, and perceived leadership instability; these dynamics were linked by families to inconsistencies in care, communication, and morale. Administrative responsiveness and complaint handling were judged inadequate by some visitors, including concerns about admissions interactions and limited flexibility in family access or room visitation. Communication tone also variedโ€”some staff and departments were described as professional and timely, whereas others were characterized by brusque or dismissive interactions. A few serious individual-level claims about staff conduct and post-event handling were raised and should prompt careful inquiry by prospective families.

For prospective residents and families: Brookside offers clear strengths in rehabilitation, therapy, and some compassionate direct-care relationships, along with active programming and coordination with medical services. At the same time, variability in medication management, cleanliness, staffing continuity, and administrative responsiveness make it important to verify current practices. Recommended due diligence includes asking about recent staffing turnover, medication-administration protocols and medication education, visitation policies, routine sanitation audits, and the facilityโ€™s processes for escalation and complaint resolution. Observing therapy sessions, meeting the unit nursing leadership, and requesting documentation of staff continuity for anticipated shifts can help align expectations with on-the-ground operations.
