    Kind staff, clean safe home

I placed my mom here and I'm very pleased - the staff are kind, professional, and attentive, with excellent RNs, CNAs and PT; the facility is very clean, safe, and feels home-like. Meals, activities, hair services and a roomy room (window by the bed) keep her happy and confident about long-term care.

    Summary of reviews

The reviews for Holston Rehabilitation and Care Center are sharply mixed, revealing a facility that elicits strong praise from some families and equally strong alarm from others. On the positive side, multiple reviewers singled out nursing staff — RNs and CNAs — and the physical therapy team as exceptional. Several reports describe caring, patient, and professional nurses and therapists who delivered successful rehabilitation after falls. The business office receives consistent praise for professionalism. Many reviewers also noted a warm, home-like atmosphere for some residents, with attractive communal spaces (piano, TVs, aquarium), regular activities and entertainment (Elvis performances, corn hole), and rooms that some found clean, spacious, and comfortable. Proximity to neighborhood services was another practical benefit mentioned.

Contrasting sharply with these positives are very serious negative reports. Some reviewers allege neglect, including instances of residents being left in soiled bedpans for extended periods and the development of bedsores. A subset of reviews contains severe accusations — claims of inappropriate medication use for dementia, elder abuse, and retaliation against those who reported problems — and at least one family reported removing a resident due to safety concerns. These are grave allegations that, if accurate, indicate critical failures in care and oversight. Even among less extreme complaints, there are recurring themes of understaffing, delayed responses to resident assistance needs, inconsistent care quality, and occasional cleanliness lapses (for example, hair found in meal trays).

The pattern suggests a facility with notable strengths in therapy and in pockets of caregiving excellence, but also with systemic issues that undermine consistent quality. Understaffing appears to be a central driver of many negative experiences: delayed assistance, missed care tasks, and cleanliness problems are often connected to not having enough staff on duty. Communication gaps, especially involving social work or administrative follow-up, are another recurring frustration for families. Dining receives mixed mentions — the dining room and atmosphere can look nice, but food quality is criticized by several reviewers and there was at least one safety concern related to meals.

Facility-related observations are similarly mixed. The building is described as older and some rooms are not up-to-date, though a number of reviewers found rooms adequately clean and comfortable. Activities and engagement are frequently cited as strengths, contributing to a pleasant day-to-day experience for some residents. However, because experiences vary widely between reviewers, it appears that outcomes may depend heavily on staffing levels, the particular unit or shifts, and the individual caregivers assigned.

In summary, Holston Rehabilitation and Care Center receives both high praise and very serious criticism. Key strengths are its nursing and therapy staff in many cases, an active activities program, and a professional business office. Key risks reported by reviewers include alleged neglect and abuse in extreme cases, chronic understaffing leading to delayed care and cleanliness issues, poor communication from social work/administration, and inconsistent food quality. Prospective families should be aware of this variability: verify staffing and care processes, ask about pressure ulcer prevention and medication management protocols, request recent inspection records, speak directly with nursing leadership about response times and incident reporting, and, if possible, tour the facility during different shifts to assess consistency of care and cleanliness firsthand.

About Holston Rehabilitation and Care Center

Holston Rehabilitation and Care Center sits over on Memorial Boulevard in Kingsport, Tennessee, and folks have known it for years since it's been serving the Kingsport and Tri-Cities area since 1967. The place just got newly renovated, and it stands as a 204-bed nursing home and residential care facility, with a focus on rehabilitation and long-term care, and you get both private rooms and spacious therapy gyms there. They've got transportation services, laundry, housekeeping, Wi-Fi, and on-call doctors, along with on-site maintenance for those unexpected needs, and the staff is used to growing and changing with the times because health needs tend to shift as folks age. You'll find a lovely interior courtyard along with an atrium that people sometimes use for private gatherings, plus an active department that runs activities to help keep minds and bodies engaged.

The skilled nursing care runs 24 hours a day, and a team of Physical, Speech, and Occupational Therapists works with people who stay short-term or for longer periods, and you'll notice Holston's focus rests a lot on care and respect for each patient. The center does its best to meet both social and health needs, with certified beds and a nurse staffing level reported as 3.91 hours per resident per day-though like many places, the nurse turnover was 54.1%. This facility has a for-profit setup run by Venza Care Management, and Susan Strauss has held managerial control since January of 2022. The place has had 16 total deficiencies reported in inspections, including 2 for infection, and it follows infection control and quality of life procedures.

Several staff members have worked there for decades, which shows some stability, and the center offers Certified Nursing Assistant training programs and sometimes hosts education seminars, conferences, and training for professionals and consumers. Holston Rehabilitation and Care Center holds membership in the Tennessee Health Care Association (THCA) and works closely with local hospitals like Ballad Healthcare, Holston Valley, and Indian Path, so hospital transitions aren't too far off. The philosophy there centers on trying to treat everyone with dignity, keeping up with the growing needs of the community, and providing comfort, especially for people who need either short-term help after a hospital stay or longer-term nursing care. The updated community spaces, patient-centered services, and resources for long-term care make a difference for both residents and their families.
