    Excellent care, clean bright home

I moved my dad here and I'm very pleased - the facility is clean, bright and feels like home. The staff (Shannon and the therapy team especially) are attentive and compassionate, activities are plentiful, and restaurant-style dining and rehab services have really improved his quality of life. Communication is prompt, the owners are involved, and I would recommend this well-run, caring community.

    Summary of reviews

Overview: The Oaks at Liberty Grove presents as a relatively new, well-appointed assisted-living community with many elements families commonly seek: modern apartments (private studios and one-bedrooms), bright common areas, an attractive courtyard/patio, and multiple resident amenities such as a barber/beauty salon, library, and organized activities. Reviewers consistently praise the facility’s physical environment and cleanliness, and many describe a warm, home-like atmosphere with engaged staff and visible leadership during certain periods.

Care quality and staff: A dominant positive theme is compassionate, attentive caregiving and a number of staff members and directors who are described as respectful, personable, and hands-on. The therapy team (PT/OT) and short-term rehabilitation services receive frequent commendation for their effectiveness. At the same time, a recurring operational concern is high staff turnover, which reviewers link to gaps in continuity of care, variable service quality, and periodic short-staffing. Several accounts describe inconsistent administration and communication from leadership, with some families experiencing prompt, effective responses and others reporting slow or inadequate follow-up. More serious clinical concerns raised include gaps in medication administration controls and training, and isolated but serious allegations relating to medication theft and financial practices; these claims warrant verification through facility leadership and regulatory sources.

Dining and activities: Dining is a pronounced strength for many reviewers: restaurant-style service, daily menus with specials, flexible dining hours, and in-room meal delivery are highlighted. An executive chef and weekly menu rotation appear to support variety, and numerous families find portions and homemade items appealing. However, dining quality is not uniformly described; a number of reviewers noted inconsistent food quality and variability in meal execution. The activities program is broadly viewed as robust, with arts and crafts, exercise classes, games, music, and social programming contributing to an active community life.

Facilities and services: Facility amenities and cleanliness are frequently praised—updated decor, wide open communal spaces, and responsive maintenance are repeatedly noted. On-site clinical supports such as medicine distribution and periodic in-house clinician visits are positives. That said, reviewers identified practical shortcomings including inadequate temperature control in some resident rooms and concerns about the safety and reliability of transportation/medical-van services. Several families stated the community is better suited to residents needing assisted living or rehabilitation-level care rather than those requiring consistent, higher-acuity two-person assistance.

Management and notable patterns: The reviews show a clear split between periods of perceived strong, resident-focused management and periods of instability or problematic administration. When leadership is engaged and communication is timely, families report excellent experiences, strong hospice coordination, and memorable end-of-life support. Conversely, recurring themes of management-related weaknesses include inconsistent responsiveness to concerns, billing and financial-transparency questions, front-desk professionalism variability, and insufficient staffing or training in clinical procedures. Prospective families should note this pattern of uneven experience and seek current, specific information about staffing levels, medication administration protocols, turnover rates, and transport safety before making a placement decision.

Bottom line: The Oaks at Liberty Grove offers many features typical of a high-quality assisted-living community—pleasant facilities, an active activity schedule, solid therapy services, and a core of compassionate caregivers. However, persistent operational issues (staff turnover, management inconsistency, medication-administration controls, and some service variability) create a mixed picture. Families considering the community should tour during activity and meal service, ask for written staffing ratios and medication-control policies, confirm temperature-control and transport procedures, and request references from current residents’ families to evaluate recent leadership and care stability.
