    Compassionate attentive staff, dignified care

I placed my mom here and I'm deeply grateful - the nurses, CNAs and admin are compassionate, attentive, and respectful (special thanks to Nurse Patti and Vicente). Staff keep residents engaged with music, exercise and activities, coordinate care smoothly (even hospice via FaceTime), and rehab helped my loved one regain function. The place is clean, communication is excellent, and the team treats residents like family - I highly recommend their caring, dignified approach.

    Summary of reviews

Overall impression: Reviews portray a facility with consistently strong, person-centered caregiving and notable clinical strengths alongside operational and facility-condition variability. Praise centers on nursing and direct-care staff who are described as compassionate, attentive, and respectful; families frequently cite individualized attention, effective rehabilitation outcomes, and coordinated end-of-life care (including remote hospice communication). Many commenters highlight a home-like atmosphere, helpful admissions support, and interior cleanliness and common areas that are well cared for.

Care and staff: The dominant theme is high-quality hands-on care. Nursing, CNAs, and therapy teams receive repeated commendation for responsiveness, dignity-preserving approaches, and assisting residents to regain function. Communication with families is often described as strong, with administrators and some leaders noted for proactive, solution-oriented involvement. However, there is variability: some accounts indicate lower engagement from particular shifts or staff members and occasional concerns about responsiveness and professional conduct. These comments suggest staffing and supervisory consistency as an area for ongoing attention.

Dining and activities: The facility offers an a la carte menu and a range of activities—exercise classes, Bingo, movies, trivia, live music and holiday events—which many residents and families find meaningful. Activity programming appears to contribute to a positive social environment in several units. At the same time, reviewers note unevenness in activity delivery: weekend programming and memory-care engagement can be limited or inconsistent, and some families perceive rushed or poorly planned events when staffing is thin.

Facilities and maintenance: Interiors and common spaces receive favorable remarks for cleanliness and upkeep, including a neat courtyard and gazebo. Conversely, the exterior and some physical plant elements are described as dated and in need of renovation; reviewers also highlight aging beds and equipment. There are isolated sanitation and maintenance inconsistencies reported in specific areas, suggesting uneven housekeeping or deferred repairs that prospective residents should inspect during a tour.

Safety, security, and management: While administration is praised in many accounts for responsiveness and hands-on problem solving, other reviews describe variability in administrative professionalism and communication. Of particular concern are accounts indicating gaps in security and emergency-response protocols; there are instances that families found serious enough to involve outside authorities, which points to the need for clarity on incident-response procedures and staff training. Prospective families should ask about current security measures, incident escalation policies, and recent improvements.

Notable patterns and guidance for visitors: The strongest and most consistent positives are the quality of bedside care, rehabilitation success, and a family-oriented culture among many staff. Recurrent operational themes to evaluate in person include staffing consistency (especially for activities and weekends), condition of resident equipment and the exterior, and the facility's emergency-response and security processes. A recommended approach for prospective residents is an in-person tour focused on memory-unit programming, recent maintenance projects, staffing ratios by shift, and a review of security/incident protocols and family communication practices.

    About Wurzbach Nursing and Rehabilitation

Wurzbach Nursing and Rehabilitation sits in San Antonio, TX, offering skilled nursing and rehabilitation with 140 certified beds and an average of about 84 residents a day, and the staff there work around the clock, day and night, to help people who need long-term care, health oversight, and recovery services of all kinds, focusing on health and safety, although inspections have turned up deficiencies over the years, like issues with infection control, accident hazards, supervision, and even some problems making sure residents are safe from harm or wrongful use of belongings, which remains an area that needs attention. The community is part of Caraday Healthcare and works with big names like Baylor Scott White Health and St. David's HealthCare, and a nationally recognized geriatrician leads medical care as Chief Medical Officer, aiming to keep care standards high, though the nurse turnover rate has been at 63.3%, and the staffing averages about 3.05 nurse hours per resident per day, which is something families might think about. Residents get skilled nursing, memory care, and lots of rehabilitation services-there's help for people after joint replacement, stroke, spine and back trouble, wounds, breathing problems, and serious illnesses like diabetes, CHF, or COPD, along with constant tracheostomy care for those who need it. The facility also includes a secure memory care unit where a team trained in dementia and Alzheimer's care supports residents, and the daily schedule is gently structured to help people feel safe and cared for, with on-site therapy-physical, occupational, speech, and wellness-all part of the regular offerings, giving personalized support for each person's needs.

Meals are provided each day, meeting dietary needs including specialized nutrition when required, and residents who need help with daily living-like getting dressed or keeping clean-find staff there to assist. The building has several common spaces, a beauty shop, and rooms where residents can take part in devotional activities or join offsite outings, plus there's always a full list of social, educational, and entertainment events happening, designed so anyone can join if they feel up to it, whether it's for enjoyment, emotional support, or to help with mental and physical activity. People living at Wurzbach Nursing and Rehabilitation can expect diabetic care, incontinence help, medication management, wound care, palliative and hospice support if needed, and a focus, wherever possible, on fostering a sense of independence along with socialization and friendships. For residents or families who run into concerns, there's access to a long-term care ombudsman who offers confidential help. While the community tries to balance safety with activity and has many engaging programs, the history of care and safety violations in inspections is something to keep in mind, as the facility continues working towards improvement in those areas.
