    Strong leadership and compassionate care

I stayed here for rehab and was very pleased - new ownership has brought strong leadership, a caring administrator and DON, and a clear focus on quality. The nursing and therapy teams were compassionate and knowledgeable, the building spotless and welcoming, and staff went above and beyond with activities, meals, and follow-up. I would highly recommend this facility for post-acute care.

    Summary of reviews

Founders Plaza presents a mixed profile: many families and residents praise the facility's cleanliness, welcoming atmosphere, and especially its rehabilitation and therapy services. The site includes roomy communal areas and a large dining room, and admissions staff and some clinical staff are frequently described as helpful and efficient. Activity offerings are extensive and varied, with religious services, bingo, crafts, exercise, and social programming that can be beneficial for residents, including those with dementia.

Clinical and caregiving strength appears concentrated in certain teams. Numerous accounts highlight compassionate nurses, attentive aides, and a therapy department that produces measurable progress for short-term rehabilitative stays. At the same time, reviewers describe notable variability in staff skill and availability: weekend and overnight coverage, call-bell responsiveness, and support-staff consistency are recurrent concerns. Several families raised issues about adherence to medical orders and medication handling, which indicates a need to verify medication-administration protocols and auditing practices prior to placement.

Dining and activities receive mixed feedback. Many residents and families appreciate the social programming and in-house religious offerings; dementia programming that emphasizes social interaction is a positive feature. Meal quality, however, is inconsistent in reviewer accounts โ€” while some describe enjoyable meals, others characterize the menu as overly reliant on processed items or not meeting expectations. Prospective families should inquire about current dining menus, special-diet accommodations, and how kitchen staffing or budget constraints affect food quality.

Facility condition and operations show strengths in cleanliness, maintenance, and communal space design, but some reviewers noted privacy concerns related to constant in-room camera use. Administrative areas are another mixed area: admissions staff and certain leaders receive praise for accessibility, yet others described communication breakdowns, billing delays, and instances where families felt transparency and accountability were lacking after critical incidents. There are also repeated comments about staff turnover and variability that can affect continuity of care.

For prospective residents and families: verify weekend and night staffing levels, ask about medication-administration safeguards and recent regulatory/quality audits, review the incident-reporting and family-notification process, sample the menu, and observe activity sessions. If privacy concerns are important, discuss camera policies and room-monitoring practices up front. Overall, Founders Plaza appears to offer solid rehab services and an active social program within a generally clean environment, but consistent oversight of clinical practices, staffing patterns, and administrative communication would be prudent areas to confirm before making placement decisions.

About Founders Plaza Nursing & Rehab

Founders Plaza Nursing & Rehab is a nursing and rehabilitation facility that's set up to help older adults who need daily medical care and help with basic tasks, and even though the place has some modern touches and features like emergency alert systems, outdoor spaces and movie nights, you'll also find practical things such as furnished rooms, private bathrooms, kitchenettes, and air conditioning, and there are transportation options available too if anyone wants to get out and about or has a doctor's visit, and there's a professional chef making meals all day with a dining room that offers restaurant-style service, including meals for special diets like diabetes or allergies. The staff can help with things like laundry, housekeeping, bathing, grooming, dressing and transferring from bed or chair, and there's medication management and 24-hour supervision for people who need it, plus skilled care for those with complex needs including therapy, wound care, and occupational therapy, and if someone needs help with walking or wheelchair use, personal care assistants pitch in. Residents can join in daily programs like arts, crafts, games and movie nights, or take part in fitness, religious, and health education activities, or just relax in the garden or library, while community events and resident-run activities are also offered. Safety measures are observed, with visitor restrictions sometimes in place, regular deep cleaning, and a strong focus on hand hygiene and use of PPE, with COVID-19 updates following CDC and CMS advice, and they accept Medicare, Medicaid, and long-term care insurance. Founders Plaza is licensed for 106 beds, though only 29 are in use as of June 2025, and on average there are about 77 residents each day, and while nursing staff are available 12-16 hours daily, nurse hours per resident are a bit below the state average. There have been 23 deficiencies on inspection reports, including issues with pharmacy services, infection control, and resident care planning, but the nurse turnover rate is slightly below the Texas average. Fresh residents are welcomed regardless of their background, and management has been under Wylie Health Care LLC since September 2021. Amenities include Wi-Fi, cable TV, washers and dryers, a fitness room, salon/barbershop, parking, safety features, and maintenance, and there's access to podiatry and other health services. Policies encourage social distancing where needed, and staff get ongoing training for safety. The focus stays on rehabilitation and long-term care, with services tailored to each resident's needs, and while not every detail about Founders Plaza Nursing & Rehab is available, the aim remains to deliver support to seniors in a setting equipped for both daily living and medical care.
