    Clean, friendly staff; excellent therapy

I placed my loved one here after a rehab stay and was very impressed. The facility is clean, home-like and well organized; staff are caring, responsive and friendly; therapists deliver excellent PT/OT and rehab; there's a pleasant porch gathering spot and a quiet, welcoming atmosphere. My family settled quickly - I would bring someone back and recommend it, with only minor room for improvement.

    Summary of reviews

Reviewer feedback for Three Rivers Health & Rehab Center is mixed, with clear strengths in rehabilitation and direct-care relationships but recurring concerns about operational consistency. Positive comments emphasize effective PT/OT services, a knowledgeable nursing and therapy team, and a generally home-like atmosphere in parts of the facility. Several reviewers described compassionate, friendly staff and a quiet setting with useful communal spaces such as a porch gathering spot. A number of families and residents also noted responsive maintenance and engaging activities that support socialization.

Care quality appears uneven. Many reviewers praised nursing and therapy staff for professionalism and effective rehab outcomes, describing staff as caring and responsive. At the same time there are multiple indications of variability in responsiveness and oversight: delayed responses to resident needs, understaffing on particular shifts, and concerns about clinical-communication between providers and families. The therapy program is a consistent strength, while routine caregiving and timely attention to call bells are described as inconsistent.

Dining and activities received mixed feedback. Some reviewers complimented the food and the facility’s activity offerings, describing food they enjoyed and programs that encouraged social interaction. Other reviewers found meal quality poor and inconsistent. The activity program and communal spaces were generally viewed positively when present, supporting social engagement for residents.

Facility condition and infection control are points of divergence. Several reviewers described clean, pleasant rooms and a well-maintained, home-like atmosphere in renovated areas; others raised sanitation concerns, odor issues in parts of the building, and uneven cleanliness between wings. These contrasting impressions suggest variability by unit or shift. There are also explicit concerns about infection-prevention practices and clinical oversight that prospective families should explore further.

Management and operational patterns: some reviewers noted an improved atmosphere under new ownership (since March 2023), while others raised serious operational questions, including concerns about staffing levels, communication (including unanswered phones), and billing practices. The presence of allegations regarding billing or extended stays for financial reasons is a significant claim and should prompt direct inquiry and documentary review if relevant to a prospective resident.

Overall, the facility shows clear strengths in rehabilitation services, individual staff relationships, and certain renovated areas, but it also displays inconsistent performance on sanitation, staffing, responsiveness, and meal service. Prospective residents and families would benefit from on-site visits at different times of day, targeted questions about staffing ratios and infection-prevention protocols, review of recent inspection reports, clarification of billing practices, and conversations with the therapy team and current families to assess consistency across units and shifts.
