    Compassionate, professional care and support

I placed my loved one in this newer, small facility and have been very pleased. The staff are incredibly caring, professional and highly trained - nurses, aides and therapists went above and beyond with personalized, health-focused care. Communication was excellent, bedside sitters and end-of-life support were compassionate and always available, and the building felt clean and presentable with thoughtful safety measures. I'm grateful for the comfort and support and would strongly recommend this team.

    Summary of reviews

Reviews of Brookhaven Nursing & Rehab describe a facility with a mixture of strong clinical rehabilitation services and notable operational inconsistencies. Many families praised the therapy teams (physical, occupational, and speech), and several accounts describe compassionate, dedicated nurses and aides who provided attentive rehab and personal care. A number of reviewers specifically commended administrators or directors for being available and communicative, and some residents occupied private, presentable rooms in a relatively new facility environment that some found clean and pleasant.

Alongside those positives, a clear pattern of systemic weaknesses emerges. Medication-management problems — including delays in administering prescribed pain medications, supply issues, and occasional substitution with less effective alternatives — were repeatedly described; these were often tied to slow clinical response and family frustration. Staffing levels and responsiveness were also a frequent concern: reviewers cited long waits for assistance, perceived understaffing, and inconsistent handoffs that contributed to delayed care. Communication and accountability appeared uneven across shifts and departments, with families noting chart confusion and difficulty getting clear, consistent information from staff and administration.

Clinical-safety and ancillary-service issues were raised in multiple reviews. Several families reported wound-care and escalation gaps that required higher-level intervention, and there were mentions of falls and transfer-safety problems, suggesting opportunities to strengthen fall-prevention and monitoring practices. Laundry and transportation coordination were described as unreliable at times, and some reviewers experienced unresolved follow-up on those services. Sanitation and housekeeping feedback was mixed: while some reviewers found the facility clean, others described localized cleanliness and odor concerns in bathrooms and common areas. Meal service quality and consistency were another recurring theme — reviewers reported cold meals, difficult-to-chew food, and late deliveries.

Family interactions and policy issues also affected perceptions of care. Some reviewers described restrictive visitation or social-service gatekeeping and concerns about staff conduct or tone during interactions with residents and visitors. A few accounts referenced serious adverse outcomes and concerns following a resident's death; these appear to be isolated but significant to those families and point to the importance of clear communication and incident follow-up. Additionally, ventilation issues related to the placement of smoking areas and restrictive window policies were mentioned as affecting air quality and comfort.

Overall, Brookhaven appears to deliver effective rehabilitative therapy and has staff who are capable and caring; however, prospective residents and families should be aware of recurring operational challenges. Key areas to evaluate during a visit include medication-administration protocols, staffing ratios and response times, wound-care and fall-prevention procedures, meal service operations, laundry and transport arrangements, and the facility’s approach to family communication and incident follow-up. Experiences reported by reviewers vary substantially, so direct observation and specific questions to management about these operational areas are advisable when considering placement.

    About Brookhaven Nursing & Rehab

Brookhaven Nursing & Rehab provides a wide range of care options for seniors, offering rooms set up for different needs including active adult living, independent living, assisted living, memory care, home care, adult day services, long-term care, skilled nursing, home health care, and hospice care, and the building's set up with wheelchair-accessible showers, baths, and tubs, helping folks with daily routines. The facility has 90 certified beds and skilled nursing staff around to deliver care, working to keep up infection control standards and focusing on safety with supervision to reduce accidents and hazards, going over these things in regular inspections. Residents can use both indoor and outdoor common areas, with activities happening on and off the property, including devotional gatherings, and they serve meals with options like kosher and vegetarian, though there have been some nutrition and dietary issues reported, mostly about food handling and the taste or temperature of the food. Brookhaven provides services like non-ambulatory care, incontinence help, and care for people with higher medical needs, and if someone needs to go to an appointment, they've got transportation using accessible vans. Nurses and staff are available to help with personal needs, but the nurse staffing level comes out to 3.11 hours per resident each day, which is a bit lower than the state average, and while nurse turnover is about 51.7%, that's still lower than most places in the area. You'll find respite or short-term stays, rehab therapies to help people recover, and everything's led by Ltc Management Services Llc, with James and Judy Lincoln as owners, each holding half ownership since December 2014. They say they're dedicated to improving life for residents and staff, and the staff tries to offer personalized services based on each person's needs, always under the eyes of physicians when it comes to quality of care and overall well-being. Brookhaven's got a presence on Facebook, and if residents or families want to come see the place, they can take a virtual or in-person tour, get in touch easily by phone or email, and inquire about the details and daily routines if they're curious.
