    Helpful staff, clean facility, recommended

I'm grateful for the caring, helpful staff-front office even assisted with Medicaid paperwork. The newer, clean building in a nice neighborhood offers decent meals and occasional outings; it's convenient for family visits and suitable for hospice needs. I highly recommend it.

    Summary of reviews

Reviews reflect a facility with a mixture of practical strengths and significant operational concerns. Positive remarks focus on location convenience for families, administrative help with Medicaid paperwork, and individual caregivers — particularly some CNAs — who are viewed as compassionate. Several reviewers also noted that parts of the building are newer and that the site can be appropriate for hospice or short-term comfort-focused stays. Occasional outings are provided and a subset of families found the environment clean and acceptable.

Major themes of concern center on care quality and responsiveness. Multiple families described delays in attending to basic personal-care needs and incontinence management, episodes of prolonged time in bed or in chairs for some residents, and instances of pressure-injury or other skin-integrity problems. There are also recurring accounts of insufficient responsiveness to call lights and urgent requests, and reports suggesting uneven practices around transfers and mobility assistance that raise transfer-safety questions.

Staffing and conduct issues appear repeatedly. Reviewers cited high turnover and frequent use of temporary staff, inconsistent task follow-through, and variable professionalism and communication tone from some employees. At the same time, families who had direct, ongoing contact with certain CNAs or front-office staff reported helpful behavior; this indicates variability across shifts and individuals rather than uniformly poor or uniformly excellent staffing.

Dining and engagement programming are described as inconsistent. Comments range from acceptable to poor food quality, with specific concerns about limited options for therapeutic diets (for example, diabetic-friendly meals) and a prevalence of less varied menu choices. Activity offerings were characterized as limited, with only occasional outings and modest on-site programming.

Facility maintenance and housekeeping are mixed. Several reviewers reported housekeeping shortcomings and odor concerns in hallways and resident rooms, while others described clean areas and newer facility sections. Furniture and room upkeep were identified as needing attention in some units. Administrative responsiveness is similarly inconsistent: families praised assistance with paperwork in some cases but also reported poor follow-up, difficult phone communication, and at least one filed state complaint.

Overall, the pattern in these reviews suggests a facility that can provide competent, convenient care in certain circumstances — particularly for hospice or when individual staff members are consistently assigned — but one that also has systemic issues around staffing consistency, cleanliness/housekeeping standards, responsiveness to basic care needs, and clinical follow-through. Prospective residents and families should consider on-site visits (including meal service and a walk-through of resident rooms and hallways), inquire about staffing ratios, wound and incontinence care protocols, medication-administration procedures, and the frequency and types of activity programming before making placement decisions.

About Misty Willow Healthcare & Rehabilitation Center

Misty Willow Healthcare & Rehabilitation Center sits on nicely landscaped grounds with a modern building that holds 124 certified beds and usually cares for about 99 residents every day, offering both short-term and long-term care that focuses on nursing and rehabilitation, and what people will find here are skilled nurses on staff around the clock, therapists for physical, occupational, and speech needs, and an in-house therapy program that uses a hands-on approach aimed at helping residents regain strength and mobility, all guided by care plans made by a team working with each resident, their family, and healthcare providers, and folks get meals included with their stay, laundry and housekeeping services, regular maintenance, and care provider support, while the communal dining room and well-appointed accommodations, which include private and semi-private rooms, help make things comfortable and the daily routine easier. There's a busy activities calendar here, with social, spiritual, and entertainment events running from morning to night, plus a variety of organized programs designed to help people stay active. Special care units and specialized care programs are set up for folks who need a bit more support. Misty Willow Healthcare, Inc. manages the center for Liberty County Hospital District No 1, while The Ensign Group is affiliated. Staff help with daily needs and the dining menus focus on nutrition. Parking is available on-site. Medicare gave Misty Willow a 5-star overall quality care rating, though inspection reports have found 16 deficiencies, including issues with resident privacy, infection control, and pressure ulcer care, and nurse turnover sits at 56.8%, which is higher than the state average, and staff provide a bit fewer nursing hours than the average in Texas, so while there are some strengths at Misty Willow, there are also areas noted for improvement, and anyone can look at their Medicare page for more details.
