    Welcoming attentive family-oriented excellent care

I felt welcomed from day one - the staff were caring, attentive, and treated my loved one like family. Long-tenured nurses and visible department leads (special thanks to LaTasha Wright, Nicole Averette and Tiffany) clearly explained meds, therapy and care plans; the small, clean facility with lovely grounds, good food and responsive CNAs made daily care and rehab smooth. Communication with family was excellent and I'm very satisfied with the personalized, resident-focused care.

    Summary of reviews

Reviews for Hunter Creek Health and Rehabilitation present a mixed but recurring pattern: many families and residents describe genuinely attentive, compassionate care and effective rehabilitation services, while others report operational inconsistencies that affect safety and comfort. Positive accounts emphasize a small, home-like atmosphere, long-tenured caregivers, clear nursing leadership, and prompt discharge planning and benefits assistance. Admissions and administrative staff are frequently described as welcoming and helpful, and the facility's outdoor landscaping and therapy programs are noted as strengths.

Care quality and staffing show substantial divergence. Numerous accounts praise CNAs, nurses, and department leads who provide personalized care, explain treatment plans, and respond promptly; therapy teams and discharge planners receive consistent commendation for helping residents return home. At the same time, reviewers describe irregularities in caregiver responsiveness and conduct, including delays in answering call buttons, periods where staff are not readily available, and occasional unprofessional hallway behavior. High staff turnover is mentioned alongside reports of long-tenured staff, suggesting variability between shifts and roles.

Clinical safety and daily-care issues are recurring themes. Several reviewers raised concerns about medication handling and administration accuracy, with examples implying gaps in medication-management processes. Inconsistent bathing and hygiene scheduling, and more general incontinence-care concerns, were also described. A few accounts reference serious outcomes or concerns following a resident's death; these underscore the importance of clarifying clinical protocols and escalation pathways with facility leadership.

Dining, activities, and resident life are described unevenly. Some families praise individualized meal service and a welcoming dining experience (including ready beverages and food preferences accommodated), while others find meal quality unsatisfactory. The activities program and activities director earn positive mention for engagement and a festive atmosphere. The facility's small size is often cited as enabling staff to know residents personally, which many families value.

Facility condition and management practices show mixed performance. Outdoor areas and landscaping are commonly described as neat and attractive. Conversely, interior maintenance and sanitation receive criticism in a subset of reviews, including reports of mold, maintenance shortcuts, shabby furnishings, and odor concerns. A small number of reviews allege missing personal items and financial irregularities; these allegations merit direct inquiry during any evaluation. There are also comments suggesting some online ratings may be influenced by staff or vendors, which reviewers feel complicates interpretation of posted scores.

Overall, Hunter Creek appears to offer strong, person-centered care and effective rehabilitation for many residents, supported by engaged admissions and therapy staff. However, variability in staff responsiveness, medication practices, hygiene scheduling, interior maintenance, and professional conduct are notable patterns. Prospective residents and families would be advised to tour the facility, ask specific questions about medication administration protocols, staffing levels by shift, hygiene and maintenance practices, security for personal belongings, and to speak directly with nursing leadership about any clinical or end-of-life care concerns before making placement decisions.

About Hunter Creek Health and Rehabilitation, LLC

Hunter Creek Health and Rehabilitation, LLC is a for-profit skilled nursing facility with 78 certified beds that usually has about 74 residents each day, and it's known for providing both long-term care and short-term rehabilitation for folks who need some help between a hospital stay and returning home, and you'll find that they're able to offer physical therapy, occupational therapy, speech therapy, wound care, restorative nursing, IV therapy, cardiac recovery, and orthopedic care, so residents get the care they need whether they're here for a while or just passing through to get back on their feet, and they've got professionally trained nurses, certified nursing aides, and therapists who say they're committed to quality care, and their nurse turnover rate is 39.5%, which is somewhat better than the Alabama average, and the staff is around enough that the reported nurse hours per resident per day sits at 4.10, which is a bit above the state average. The rooms are set up with cable TV and complimentary Wi-Fi, and residents use a restaurant-style dining area or get in-room dining if needed, with beautician, laundry, housekeeping, and transportation services included, so no one's left without the basics. There are outdoor courtyards and inviting common areas that make the place pleasant enough, plus daily activities trying to keep people active and involved, and it's clear some effort goes into making things homelike, though one will notice they've had a few state deficiencies in recent years, like a delayed standard inspection and a couple of findings about respiratory care, nutrition, and infection control, none of which caused actual harm, but showed a potential for problems. They're operated by NHS Management and have managers named Casey Boyd, Marcia Duffy, Matthew Projahn, and Lynn Rasco, and they accept Medicaid and Medicare, making care possible for a range of folks who need skilled nursing and rehab, as well as intermediate care for people who can't get by alone anymore.
