    Warm family community, attentive staff

I'm very pleased with this warm, family-like community - spotless facility, pretty decorations, and engaging activities that my loved one enjoys. The staff (Tamara, Lisa, Monique, CNAs, nurses, DON/ADON) are caring, attentive, and communicative; they anticipate needs, keep us informed, and genuinely go above and beyond. Rehab and therapy have been strong and the overall atmosphere put our family at ease - I would recommend it.

    Summary of reviews

Park Manor Health and Rehabilitation, LLC elicits a mixed but clear pattern of strengths and operational weaknesses. Many families and residents highlight a consistently compassionate bedside presence: nurses and CNAs receive frequent praise for personalized attention, kindness, and continuity of care. Rehabilitation services and therapy staff are commonly described as effective and engaging, and activity programming — including twice-daily events and special celebrations — contributes to a family-like, homelike atmosphere. Several reviewers also compliment the facility’s decor and perceived cleanliness, as well as visible, approachable leadership from the director of nursing and assistant DON.

Clinical strengths described include attentive pain management, proactive therapy teams, and staff willingness to anticipate resident needs. For families seeking an environment with social engagement and steady therapy focus, the facility has many of the attributes people value: personable caregivers, organized activities, and leadership that is accessible for questions.

However, the reviews also raise a number of operational concerns that prospective families should review carefully. There are recurring accounts of inconsistent responsiveness to resident requests and call lights, and several comments point to sanitation and incontinence-care issues. Meal quality and delivery reliability are inconsistent across accounts — some families praise the food while others describe missed or delayed meals. There are also indications of uneven staff conduct and communication tone, with reports ranging from very kind interactions to staff perceived as brusque or poorly trained.

More serious clinical and safety themes emerge in portions of the feedback: concerns about transfer- and equipment-related safety practices, medication-management gaps, and variability in staff training and competency across shifts. Some comments describe lapses in family communication regarding clinical changes, and a few reviews note alleged missing belongings and medication discrepancies; these are matters worth verifying directly with administration. Additionally, visitation policies have been described as restrictive in at least one account, which may matter for families wanting flexible access.

For prospective residents and families: visit the unit during peak care times, observe staff–resident interactions, and ask for documentation on staffing ratios, recent inspection or incident reports, medication-administration protocols, infection-control and sanitation audits, and the therapy program structure. Request specifics on how the facility handles call-light response times, transfer training for staff, and procedures for reporting and resolving family concerns. Asking to meet the DON or ADON and to see care-plan communication examples can help clarify whether the facility’s strengths align with your expectations and whether the operational concerns raised in reviews have been addressed.
