    Positive transition to caring community

I moved my mother here and it's been a very positive transition. The building and grounds are lovely, rooms are bright with large windows, and the small, homey community (about 50 residents) really fosters personalized, person-centered care-staff know residents by name, are visible, compassionate and frequently go above and beyond (special thanks to Kelly Hutchins). Administration and nursing leadership are proactive, physical therapy and services are excellent, communication and activities are consistent, and the facility is clean and well kept. My family and I feel supported and would recommend it.

    Summary of reviews

Concordia Manor elicits strongly mixed impressions. Many family members and residents describe a small, home-like environment in which staff are visible, personable, and attentive to individual preferences. Positive comments emphasize a tight-knit community of roughly 50 residents, staff who recognize residents by name, active social programming, and staff who make efforts to support family involvement and transitions. Several reviewers highlighted specific strengths such as responsive nursing presence, available physical therapy, bright common spaces, and a generally welcoming atmosphere for visits and activities.

At the same time, there are consistent operational concerns that prospective families should evaluate. The physical plant is frequently described as dated and in need of renovation; rooms are often small with limited furniture and, in some configurations, multiple residents share a bathroom, which limits privacy. Reviewers raised parking and exterior maintenance issues, including cigarette butt accumulation near entrances, indicating opportunities for improvement in property upkeep and exterior cleanliness.

Care delivery and staffing show variability. Many reviewers praise individual nurses and certain caregivers, but there are recurring reports of staffing shortagesโ€”particularly on nights and weekendsโ€”and difficulty locating staff when needed. This variability is linked to concerns about timeliness of routine care, incontinence-care practices, and pressure-injury prevention. Several accounts point to supply-management problems (for example, limited spare wipes or undergarments) and cases of delayed or incomplete clinical follow-up, including slow lab-result communication. Infection-control and outbreak management were also raised as areas to discuss during a tour.

Dining and daily living services appear inconsistent. Descriptions range from acceptable to poor: some families report adequate, family-friendly meals while others cite menu deviations and unsatisfactory food quality. Activities programming and social engagement are a relative strength; the facilityโ€™s small size supports frequent social interaction and personalized invitations to participate.

Management and administration receive mixed feedback. Multiple reviewers noted administrators and the director of nursing as actively trying to improve operations; proactive communication and visible leadership were cited as positives. Conversely, billing transparency is a persistent concernโ€”families describe unclear itemization and higher private-pay rates without clear explanations. Communication between staff and families is reported as uneven: while some families receive regular updates and supportive guidance, others experience delays or difficulty obtaining clinical information.

Overall, Concordia Manor may suit families who prioritize a small, community-oriented setting with personable staff and individualized attention. Prospective residents should plan a comprehensive on-site visit that includes: reviewing staffing patterns (including nights/weekends), observing room layouts and bathroom arrangements, asking about pressure-injury prevention and incontinence protocols, clarifying supply and laundry processes, requesting sample menus and food-service policies, reviewing recent lab/clinical-communication practices, inspecting exterior maintenance, and obtaining detailed, itemized billing examples. These targeted questions will help determine whether the facilityโ€™s strengths align with the prospective residentโ€™s care needs and expectations.
