    Neglectful care; some compassionate staff

I'm disappointed. The facility is old and basic care is lacking-staff attentiveness is poor (caregivers idle or watching soap operas), residents left unattended in hallways, beds lowered to the floor, no water offered, and I ended up hiring a full-time sitter while still paying high fees because care gets passed to the sitter. That said, many individual staff and the social worker were caring, hardworking, and supportive, and I truly appreciate them.

    About Azalea Trace Nursing Center

Azalea Trace Nursing Center is a very large senior living community with 110 beds and about 104 residents, running at nearly full occupancy most of the time. This facility holds a 4-star rating from the Centers for Medicare & Medicaid Services, ranking as the 23rd highest rated community in its city, and has an average score of 6.4 out of 10 from different reviews. Residents can choose from a variety of studio room layouts, often with private bathrooms, kitchenettes, cable TV, air conditioning, and Wi-Fi. The center provides skilled nursing care, rehabilitation services, and both short-term and long-term care, including outpatient rehab and support for those needing memory care due to Alzheimer's or dementia. There's a strong focus on health and safety, with nurses on staff, a 24-hour call system in every room and bathroom, and routines for monitoring pain, pressure sores, and other health issues.

Staff help with activities of daily living, like dressing, bathing, and medication management, and support people who have incontinence or mobility needs, offering both independent living and assisted living options. Meals are prepared by chefs and include restaurant-style dining, snacks, and meal options for allergies or diabetes. Residents can enjoy amenities like an arts room, game room, fitness space, library, spa, sauna, and outdoor walking paths as well as lots of organized activities-everything from movie nights to exercise programs, music, and community events. Family and resident councils help make sure everyone has a say in how things run. Azalea Trace gives information and help about Medicaid eligibility and long-term care planning, plus resources for caregivers with training options and certification details.

The facility accepts many payment types, including Medicare, Medicaid, private insurance, military or VA benefits, and personal payment like cash or checks. It's a non-profit center, fully sprinklered for safety, not located in a hospital, and is part of a larger network of nursing homes. Residents and families get move-in coordination and support for transportation, errands, laundry, and housekeeping, and the staff, who speak English, are frequently described as kind and helpful. Azalea Trace also offers a wide range of medical services, such as physical, occupational, and speech therapy, tube feeding, IV therapy, oxygen therapy, and mental health support, and they hold a few awards for their friendly environment and quality of service. There's an emphasis on making life engaging, healthy, and as independent as possible for each resident, with care that matches different needs and a friendly, active community that encourages both comfort and dignity.
