    Caring staff, clean comfortable facility

I chose this facility for my mother and quickly felt reassured - the staff are caring, compassionate, and responsive, and administration is knowledgeable and organized. The building is clean and comfortable, rooms have private bathrooms and daily housekeeping, meals are home-cooked and tasty, and activities and outings keep residents engaged. Rehab staff helped my mother regain strength for a safe discharge home, and overall the warm, supportive team gives our family real peace of mind.

    Summary of reviews

Reviews for Autumn Breeze Healthcare Center are highly polarized: some families and clinicians praise compassionate caregivers, effective rehabilitation, and engaging activities, while other accounts raise substantive operational concerns. The overall pattern indicates uneven performance across clinical care, housekeeping, and administration rather than uniformly low or high quality. Prospective residents and families are likely to encounter both strong individual staff members and systemic shortcomings.

Care quality and safety are frequent themes. Positive reports highlight an effective rehab team that has helped residents regain strength and return home, and several direct-care staff receive praise for compassion and competence. Conversely, a number of accounts describe inconsistent delivery of basic care tasks (including bathing and continence support), confusion in wound-care and discharge planning, delayed escalation of medical issues after falls or clinical decline, and unreliable alarm/call-system response. These operational gaps have been associated with subsequent hospital transfers and raised concerns about monitoring and timely clinical intervention.

Staffing and conduct descriptions are mixed. Reviewers note compassionate CNAs, helpful social workers, and responsive therapists, but also describe understaffing, especially on weekends and during shift changes, high staff turnover, and variability in professionalism and attentiveness. Communication breakdowns between shifts and with families are commonly cited, as are long phone hold times and delays in returning calls. Some families report productive interactions with specific staff members and administrators; others describe disorganized coordination or a brusque communication tone.

Dining and programming receive both praise and criticism. Several reviewers appreciate home-style meals, appealing desserts, and organized outings or memory-care activities. At the same time, others point to inconsistent meal quality, missed or delayed meals, and inadequate dietary accommodations for conditions such as diabetes. Recreational programming and memory-care engagement are described positively in some units but appear uneven across the facility. Issues with televisions, reading materials, and room-level amenities are noted in some reports.

Facility condition and management practices show variability. Positive comments describe clean, well-maintained rooms with daily housekeeping, while negative accounts raise sanitation and odor-control issues, pest-control concerns, and maintenance problems such as leaks, temperature irregularities, and intermittent water outages. Property-management weaknesses (lost or missing laundry and personal items) and serious concerns about billing and discharge coordination — including wound-care billing confusion — are described. There are also reports of restricted hospice-provider options and rigid visitation procedures that families found limiting.

In sum, Autumn Breeze displays a mix of strengths and operational risks. Strengths include dedicated frontline staff, a capable rehab program, and engaging activities in some units. Recurrent weaknesses involve staffing consistency, basic care delivery, clinical monitoring and escalation, sanitation and maintenance, and administrative coordination. Families considering the facility should arrange an in-person tour, ask for recent inspection results, review staffing ratios and call-system reliability, inquire about wound-care and discharge procedures, confirm dietary accommodations, and clarify hospice and visitation policies. Doing so will help determine whether the facility’s strengths align with the prospective resident’s clinical and personal needs.

    About Autumn Breeze Healthcare Center

Autumn Breeze Health and Rehab is a medium-sized skilled nursing facility offering both short-term rehabilitation and long-term care to its residents. With a total of 109 beds, this for-profit, corporate-owned center provides services as part of the broader healthcare community in its area. Autumn Breeze Health and Rehab participates in both Medicare and Medicaid programs, accepting residents who may require financial assistance through these avenues for their healthcare needs.

The facility is not part of a continuing care retirement community, but it does offer a variety of services designed to accommodate the needs of individuals recovering from hospital stays as well as those who require ongoing nursing care. Short-term rehabilitation at Autumn Breeze Health and Rehab is intended to help patients regain strength and independence after major health events such as strokes, heart attacks, infections, or accidental injuries. Nearly 52% of residents have been able to return home following their rehabilitation, reflecting the transitional focus of these services. The nurse staffing level averages about 3 hours and 13 minutes per resident daily, encompassing both medical and personal care support, such as medication administration and assistance with daily activities like eating, dressing, and mobility.

Residents who need more comprehensive and consistent nursing care, including help with self-care activities and daily medical needs, can access long-term care at Autumn Breeze Health and Rehab. The facility also provides specialized care, including respiratory support when required, and strives to ensure residents maintain their ability to move, eat, and perform necessary actions with as much independence as possible. A high proportion of residents receive preventive health measures, such as flu vaccinations, demonstrating the facility’s emphasis on overall wellbeing.

In terms of care quality, the facility maintains regular nurse staffing levels throughout the week, with some variability on weekends. Monitoring by health agencies ensures that health and safety standards are adhered to, with nursing and ancillary staff providing care in line with comprehensive care plans that account for individual goals and preferences. Special attention is given to essential areas such as abuse prevention, administration of psychotropic medications, and the establishment of a homelike and comfortable environment for all residents.

Autumn Breeze Health and Rehab is subject to routine and unannounced inspections. Issues addressed during these inspections may include staff training, the maintenance of safe and clean living spaces, proper care planning, protection from abuse or neglect, and the implementation of facility-wide readiness assessments for both day-to-day operations and emergencies. The facility is also responsible for adhering to National Fire Protection Association standards to provide a secure environment for its residents and staff.

As part of its commitment to resident welfare, Autumn Breeze Health and Rehab has systems in place to monitor for and respond to major injuries such as falls, as well as to manage infection control and respond appropriately to medical emergencies. The operations and training programs for staff are geared toward maintaining a consistent standard of care and meeting the diverse needs of a population with complex medical and personal care requirements. The ultimate aim of Autumn Breeze Health and Rehab is to foster effective rehabilitation outcomes while offering a safe, compassionate setting for long-term residents.
