    Friendly professional clean excellent rehab

I am very pleased with Belmont Terrace - the staff are friendly, professional, and genuinely attentive. The facility is clean, bright, and smelling fresh; housekeeping, laundry and dietary have clearly improved and renovations look great. Therapy and rehab are excellent, nurses work well together and management is responsive - I would recommend them for skilled nursing and rehab.

    Summary of reviews

Reviews of Belmont Terrace Nursing and Rehabilitation Center present a mixed but distinct pattern: many families and patients describe clear improvements under new ownership and management, while others report ongoing operational and clinical concerns. Positive comments emphasize stronger leadership presence, a stable permanent staff, effective rehabilitation and therapy programming, and more attentive in-house medical oversight. Several reviews specifically note renovated rooms and common areas, supportive teamwork among clinicians, personalized care plans, and a welcoming atmosphere that some families describe as family-like.

At the same time, there are recurring facility-level issues that prospective residents and families should consider. Sanitation and odor concerns, as well as spotty personal-care and incontinence-care practices, appear in multiple accounts and suggest variability in housekeeping and bedside care standards. Reviewers also report inconsistent staff professionalism and communication tone—ranging from very helpful and friendly caregivers to dismissive or unhelpful interactions—which can affect family confidence and day-to-day resident experience.

Clinical responsiveness and safety practices show variability. Several reviews describe delays in responding to call lights or assistance requests and note coordination gaps around supplies and equipment; these point to potential weaknesses in response protocols and transfer/safety procedures. Concerns about medication-management and medical oversight were raised by some families, while others praised attentive nursing and on-site medical staff. A few reviews include serious individual allegations regarding staff conduct and past regulatory or leadership actions; those claims warrant independent verification through facility inquiry or public inspection records.

Dining and amenities are another area of inconsistency. Multiple reviewers cited unsatisfactory meal quality and limited menu options, whereas others praised dietary improvements under the new management. Activity engagement and therapy offerings received generally positive remarks, with several families finding residents active and benefiting from rehabilitative services.

In summary, Belmont Terrace shows evidence of substantive recent improvements—especially in management visibility, therapy services, and physical renovations—but retains operational risks that prospective residents should assess carefully. Recommended next steps for families: arrange an in-person, unannounced visit to observe cleanliness, staff–resident interactions, and mealtime; ask the administrator about staffing consistency, response-time protocols for call lights and falls, pest-control measures, and how clinical oversight and medication management are documented; and review recent state inspection reports for objective information about corrective actions or citations. These steps will help determine whether the facility’s positive changes have been sustained and whether any remaining concerns have been effectively addressed.

About Belmont Terrace Nursing and Rehabilitation Center

Belmont Terrace Nursing and Rehabilitation Center sits as a for-profit senior care facility with 151 certified beds and is tied to Encore Health Partners, and folks who come here can get skilled nursing care, nursing home services, assisted living, memory care, and short-term rehabilitation, so if someone needs help after a hospital stay or has health needs that require a lot of daily attention, staff offer comprehensive services. The facility takes Medicaid for those who qualify and people can tour the place, meet staff and see the rooms by request, and there's a form for tours or referrals on their site, plus a brochure, photo gallery, and virtual tour for people who want to see things before visiting. Residents have daily activities, amenities, and programs aimed at well-being, including respite care for families who need a break and continuing care as medical needs change. There's a strong focus on individualized care planning, with regular resident assessments, and staff use specialized protocols to prevent pressure sores and to support infection control, although past inspections have found some infection-related issues, and it's labeled as a Special Focus Facility Candidate, meaning it's had some care challenges in the past. Staff turnover is high, with 80% of nurses leaving, and staffing averages 3.19 nurse hours per resident each day. Belmont Terrace promotes a welcoming environment that values inclusivity and non-discrimination, and the community keeps refining services, not only for daily medical care or rehabilitation but for memory support and long-term comfort, so people who stay here often have a range of medical and personal needs. Amenities and activities serve to keep residents engaged, and respite care is one of the options, which means families can get some help when needed.
