    Attentive staff create warm community

I'm grateful for the caring, dependable staff - kitchen, laundry, housekeeping, CNAs, nurses and the helpful PT/rehab team - who were attentive, efficient, and made me feel safe. Ambassador Mirian De Souza's kindness and Director of Nursing Victoria R.'s knowledgeable, hands-on leadership, plus welcoming, culturally diverse management and excellent customer service, created a warm community; the food was a favorite.

    Summary of reviews

The reviews present a facility with substantial variability in performance: some families and residents describe attentive, skilled care from specific individuals and departments (notably nursing leadership, a resident ambassador, and the rehabilitation team), while a substantial portion of accounts raise operational and environmental concerns. Strengths most often cited include knowledgeable nursing leadership, effective PT/OT rehabilitation services, helpful laundry and housekeeping staff, and a culturally diverse, welcoming atmosphere. These positive elements are frequently tied to particular staff members who families felt went above and beyond.

Clinical care and staffing are described unevenly. Praise for certain CNAs, nurses, and therapists coexists with repeated descriptions of inconsistent staffing levels, especially overnight, and delayed responses to call bells and alarms. Reviewers indicate gaps in feeding assistance and personal-care routines, and several accounts raise concerns about medication management and reconciliation processes. There are also reports of delayed escalation to emergency services and inconsistent family notification when clinical events occur, creating concerns about continuity of care and clinical oversight.

Dining and nutrition issues recur as a distinct theme. While some residents and families complimented meal options and kitchen staff, others reported spoiled or improperly stored meals and overall inconsistent food quality. These statements point to weaknesses in food-safety, storage, and meal-service continuity rather than uniform kitchen performance.

Facility environment and maintenance are frequent areas of concern. Reviews indicate sanitation and odor concerns in some areas, pest-control concerns, problems with heating/cooling, structural vibration when heavy vehicles pass, and intermittent room-level maintenance issues. Limited private-room availability, shared bathrooms in some configurations, and restricted options for personal-display or in-room personalization were also noted as affecting resident comfort and privacy.

Management, communication, and organizational stability show mixed feedback. Some reviewers praise responsive administrators and a hands-on director of nursing; others report frequent management turnover, inconsistent policy enforcement, and poor communication from case managers. Several families described difficulties coordinating care transitions, arranging supervised outings, or receiving timely updates about clinical changes. These themes suggest variability in unit-level leadership and care-coordination processes.

Notable patterns across the reviews include a clear split between well-regarded individual staff and systemic operational weaknesses. The most consistent facility-level issues are inconsistent staffing and responsiveness, cleanliness and pest-control problems, food-safety and meal-service gaps, medication-management concerns, and limits on privacy and supervision. There are also isolated but serious accounts raising concerns following a resident's death and other adverse outcomes; these warrant direct inquiry by prospective families.

For prospective residents and families, an in-person tour with targeted questions is advisable: ask for current staffing ratios, overnight coverage plans, infection-control and pest-control documentation, medication-safety procedures, meal-storage and temperature-control policies, private-room availability, and examples of how the facility communicates clinical changes to families. Also inquire about the facility's recent leadership changes and corrective actions taken to address sanitation, maintenance, and care-coordination shortcomings. The facility shows pockets of strong, compassionate care but also operational risks that should be evaluated directly before placement.

About Stonehedge Rehabilitation and Skilled Care Center

Stonehedge Rehabilitation and Skilled Care Center sits at 5 Redlands Road, West Roxbury, MA 02132, close to Jamaica Plain, so it's handy for folks who want to stay connected with that area, and it's got this schedule tour feature for anyone wanting to come in and see the place ahead of time. The facility works as a skilled nursing facility, or SNF, meaning it offers both short-term rehabilitation and long-term nursing care for older adults and people dealing with serious health issues. The community provides skilled nursing, memory care, substance use supportive services, orthopedic and pulmonary rehabilitation, wound care, hospice, respite care for families who need a break, and even home health care for those who need it, plus a strong focus on tailoring care to fit each resident's specific needs which is something the Clinical Care Teams work on with folks, making sure there are personalized treatment programs and a goal to support as much independence as possible.

Stonehedge is a Genesis HealthCare facility, and it accepts Medicaid and Medicare, so it tries to make care easier for many. The center has a community review rating of 5.8 out of 10, which means some people have concerns while others have had positive experiences, and that's just the way things tend to go in these places. For rooms, you'll find studio layouts, some with kitchenettes, all with private bathroom amenities, and the place is furnished to keep things comfortable. The common spaces are pretty spacious, too, so there's a spot for socializing, whether it's in the community room, the library, fitness room, or out on the walking paths. There's air conditioning, high-speed internet, cable TV, and telephone service for people who want to stay connected, and a concierge service to help with daily needs.

When it comes to support, there's help with activities of daily living like bathing, dressing, transfers, and medication management, with supervision and care available around the clock thanks to a 24-hour call system and 12-16 hour dedicated nursing service. Nutrition's a focus, too, since the dining room offers restaurant-style meals that fit special diets, even for folks with diabetes or allergies, and there are snacks to suit preferences. Stonehedge offers community and recreational amenities, like arts and crafts rooms, movie nights in the theater, a game room, spa and wellness room, group and resident-run activities, and you can find family support services if needed. Laundry and housekeeping come standard, and there's transportation and parking, so family and visitors don't need to worry.

For locals, the place lets people explore the neighborhood, which might make adjusting easier, and there are outdoor spaces and programs for those able to go outside. The aim at Stonehedge Rehabilitation and Skilled Care Center is to provide a warm, safe, and nurturing setting, whether someone's there for short-term rehab after a surgery or needs long-term care due to chronic illness or memory needs, and everything is based on evidence-based practices. Specialized programs and units are in place for various health needs, and many activities run daily to help residents feel part of a community. The setting might not be fancy, but it's designed for comfort and safety, and the staff work to support well-being as much as possible for the folks who call Stonehedge home.
