    Warm attentive care beautiful facility

I'm very happy with my dad's stay - the staff are warm, attentive, and genuinely caring, and the nurses and therapists provided excellent, professional care. The facility is beautiful and spotless, meals are pleasing, and activities/amenities like the garden, beauty shop and rehab program kept him engaged. Admissions and administration were helpful and responsive, and I would definitely recommend this community.

    Summary of reviews

The reviews for Wilshire at Lakewood Rehab Center present a markedly mixed picture. Many families and patients praise the facility's rehabilitation program and several long-term caregivers: therapy staff are frequently described as effective and professional, and individual nursing and CNA interactions are often called compassionate and attentive. The facility's aesthetic features and amenities — including a welcoming admissions process, garden/courtyard areas, beauty shop, and cleaner self-pay accommodations — receive positive comments. Activity programming and certain leaders or liaisons are also credited with good communication and hands-on support in some cases.

At the same time, a consistent set of operational concerns appears across reviews. Staffing levels and reliance on temporary/agency personnel are recurrent themes; reviewers link this to delays in responding to call lights, inconsistent assistance at night, and gaps in routine care. Medication-management shortcomings are another prominent pattern: reviewers describe medication timing errors, missed doses, and unsecured controlled substances. Related clinical issues cited include irregular vital-sign monitoring, inconsistent restorative-care or weight checks, and missed or delayed therapies for some residents.

Housekeeping, meal service, and facility maintenance are additional areas of divergence. While some areas and rooms are reported as clean and well maintained, others are described as having sanitation and odor concerns, inconsistent linen and trash management, and infrequent cleaning of rooms or bathrooms. Meal complaints center on cold or late meals, limited variety, and missed service; conversely, some reviewers found the food acceptable. Infrastructure problems noted include phone system outages, HVAC or duct issues, worn carpeting and finishes, and delayed maintenance responses — all of which affect daily comfort and perceived safety.

Management and communication present another mixed pattern. Several reviews praise administrators and admissions staff who addressed concerns promptly and improved operations, while others describe slow or inadequate follow-up on grievances, poor communication about clinical changes, billing disputes, and perceived dismissive responses by leadership. There are also serious, individual-level claims including regulatory complaints and allegations of staff or management misconduct; these are raised infrequently but are consequential and warrant verification through official surveys and regulatory reports.

For prospective residents and families, the salient pattern is variability: strong clinical rehabilitation and numerous caring staff coexist with operational weaknesses that can impact daily living and safety if not actively managed. When evaluating this facility, consider asking for recent state inspection results, turnover and agency-staffing rates, the facility's medication-administration and narcotics-security policies, staffing ratios for nights and weekends, documented cleaning schedules, how grievances are tracked and resolved, and how the facility ensures continuity of care across payor wings. An in-person tour that examines rooms on both private/self-pay and Medicaid wings, a conversation with therapy leadership, and written commitments on call-response expectations can help clarify whether this facility's strengths align with a particular resident's needs.

About Wilshire at Lakewood Rehab Center

Wilshire At Lakewood Rehab Center sits with 170 certified beds and usually has about 121 residents a day, offering both short-term rehab and long-term care in a building that aims for a warm, homelike feeling, and you'll notice right away that they've got a professional and friendly nursing staff that puts a focus on each person's health, comfort, and dignity, even though staff turnover is higher than average, at about 70.7%. Residents get skilled nursing care, help with medications, and support with daily tasks, while also having access to doctors for podiatry, optometry, and dental care, plus counseling and wound care, so there's a wide range of medical services right under one roof. For those wanting to stay active or keep their spirits up, the center brings in a good amount of recreational activities, social events, and therapy programs, all designed with the idea of helping people recover or maintain their abilities, and they really do try hard to match programs to each resident's needs, offering popular physical, speech, and occupational therapy using up-to-date equipment and methods. They also care about the little details, like making sure laundry is washed just for each person, keeping rooms clean, and having different floor plans so residents can find the space that fits them, which helps everyone feel a bit more at home. The dining options stand out with well-prepared meals three times a day and snacks in between, so you won't feel left out at mealtimes, and meal plans aim to help nutritional needs but some inspection reports noted problems with food sources and dietary standards, so not everything goes perfectly all the time. There have been several documented deficiencies in recent years-54 total, with some about infection control, nutrition, quality of life, and pharmacy services-though the most recent review found no actual harm, only the chance for more than minimal harm, and management does seem to handle these issues as they come up. The center's main focus remains on solid rehab programs and making sure people get what they need, whether for a few weeks of recovery or for ongoing care, and if you're looking for a place that balances medical treatment with an effort to keep things pleasant and personal, all the information suggests Wilshire At Lakewood tries to deliver on those goals, even if they've had some bumps along the way.
