    Spotless facility, compassionate staff, homey

I've been very pleased with this facility - spotless and odor-free, home-style meals, lots of activities, and clear communication. The nurses, CNAs, social worker, dietary manager, housekeeping and administrator Kevin are compassionate, responsive team-players who treat residents like family; it's reasonably priced and a great place to live and work.

    Summary of reviews

Reviews for this facility are mixed and polarized, with clear themes of strong direct-care relationships on one hand and operational weaknesses on the other. Many family members and residents praise individual caregivers, CNAs and certain nurses for compassion, attentiveness and teamwork; several reviews specifically commended administrators and nurses who were visibly engaged on the unit. Positive accounts highlight a warm, family-style atmosphere, active programming, home-style meals with feeding assistance when needed, helpful social services and consistently thorough housekeeping that keeps common areas odor-free.

At the same time, a recurring pattern of workforce and management issues emerges. Multiple reviewers described understaffing and staff instability that contribute to delayed responses to call lights and delayed assistance with personal-care tasks. Reviewers also conveyed low staff morale — including comments about undercompensation, high turnover and reliance on temporary staffing at times — which reviewers linked to uneven continuity of care. Concerns about staff conduct and communication tone were raised alongside operational observations of staff spending extended time off-floor during shifts.

Dining and activities are frequently cited as strengths, especially when kitchen leadership and dietary staff are stable; however, dining experiences appear inconsistent across reports, with some families noting meal-quality and service gaps (for example, missing condiments or restrictive practices). Housekeeping and cleanliness receive many positive comments, but there are also reports of specific sanitation and maintenance issues in resident rooms and certain common areas, as well as problems with laundry and personal-property management that warrant attention.

Management perceptions vary considerably. Some reviews point to an engaged administrator and improved responsiveness under newer leadership, while others describe managerial disengagement or a perception that corporate priorities overshadow bedside care. Safety and supervision concerns are mentioned in the context of unit oversight and response to clinical alarms or events; a few reviewers also described troubling individual incidents that prompted questions about clinical incident response and family communication.

Overall, the facility shows strengths in direct caregiving, social programming and housekeeping when staffing and leadership are stable. The most significant risks for prospective residents and families are operational: staffing levels and stability, management consistency, dining-service reliability, laundry/property controls, and unit-level maintenance and sanitation. Families considering placement should observe current staffing patterns, ask about administrator presence and turnover rates, review recent inspection histories, and discuss specific policies for call-response times, laundry accountability and supervision practices to determine whether the facility’s current operations align with their expectations.

    About Pacific Care Center Inc

Pacific Care Center Inc sits at 105 South Sixth St. in Pacific, Missouri, and's been around since 1986, running as a privately held company focused on long-term care, assisted living, and skilled nursing. The place's cared for seniors who need both short-term and long-term health services since at least 1998, adapting over the years to meet different needs-from memory care and behavioral health to physical therapy, occupational therapy, wound management, and speech therapy-all under physician supervision. The rehab and nursing options cover everything from post-hospital stays to hospice care and respite care, and staff like Director of Maintenance Larry Cerutti, Sheryl Woolf from Alternative Homehealth, Sabrina Hertel the certified nurse aide and medication aide, and Pat Robertson the health care professional help take care of daily needs and keep things running.

The facility houses somewhere between 51 and 200 employees, and it accepts Medicare and Medicaid to help with costs. Residents find furnished rooms and helpful services like house cleaning, laundry, and dry cleaning, as well as move-in coordination. For safety, there's an emergency alert system in place and supervision is available around the clock. Caregivers help with bathing, dressing, transfers, and other activities of daily living, and they also handle medication management, making sure residents get medicine as needed and that doctors' instructions are followed.

Pacific Care Center tries to give a home-like feel, keeping the building updated and clean, and offers common spaces like walking paths and gardens for fresh air, transportation with handicapped-accessible vans, and events like movie nights to help fill the days with something to do. There are also activity programs, scheduled meals including special diets, and an eye on changing needs as folks age, so whether someone needs just a little help or more hands-on nursing and rehabilitation, the team adjusts to match. Everything's run with the goal of supporting each resident's physical, mental, and psychosocial well-being, with the main focus on treating residents with respect and letting their preferences and choices matter every day. Ms. Gretchen Gullet serves as the administrator, keeping the community steady and connected, and the place sticks to principles of integrity and steady management. The facility shares photos and location maps online, so families can get a sense of the space and the surrounding neighborhood.
