    Warm professional staff, excellent care

I placed my loved one here and have been very happy: the staff are warm, attentive and professional-from nursing and rehab teams to social worker Jenn and Diane in Finance-and the care is excellent. The dining is delicious, the facility immaculate and home-like, with plenty of activities that keep residents engaged. Communication is responsive, the atmosphere feels family-like, and I have real peace of mind knowing my relative is treated with dignity and kindness; I'd recommend this caring, well-run community.

    Summary of reviews

Overall impression Reviews present a facility with many strengths centered on people and environment. Families and residents frequently describe compassionate, attentive caregiving and praise specific clinical and therapy staff for skilled rehabilitation and supportive bedside manner. The social-work team and admissions/finance staff receive positive mention for helping families navigate placements and paperwork. The physical plant and grounds are often characterized as clean, hotel-like and well maintained, with an active activities program and a variety of entertainment options that contribute to a home-like, family atmosphere. Care quality and clinical safety Clinical and rehabilitative care earns repeated praise for effectiveness and for restoring function; several reviewers singled out therapists and nurses for notable positive outcomes. However, a cluster of more serious concerns also appears: reviewers identified instances implying gaps in medication and clinical-safety processes, delayed responses to call lights or requests, and fall- or transfer-related safety issues. One review described a medication error with a severe outcome; while this appears to be an isolated but serious account, it flags the need for prospective families to verify current safety protocols, medication-administration checks, and incident-resolution practices. Staffing, training and management Staff are broadly described as caring, upbeat and family-oriented, and many caregivers and nurses are praised by name. At the same time, recurring themes include understaffing or inconsistent staffing patterns, variability in staff competency or training, and occasional delays in attending to resident needs. Management presence and communication receive mixed feedback: some families found administration responsive and engaged, whereas others perceived management as less present or insufficiently communicative following adverse events. These mixed reports suggest variability in operational leadership and the potential value of asking about current staffing ratios, turnover, and supervisory routines during a tour. Dining and activities Dining is a mixed area. Numerous reviewers praised meals as delicious and varied, contributing to resident satisfaction and social dining. Conversely, other reviewers found meal quality inconsistent, with limited heart-healthy selections and some preparation issues. The activities program is consistently highlighted as a strength — regular events, entertainment, and therapy-supported social activities are cited as promoting resident engagement and mood improvement. Facilities, cleanliness and infection control Housekeeping and the appearance of public spaces receive high marks; custodial staff are credited with maintaining a sparkling presentation. However, several reviews raise odor concerns on particular units and note ventilation or infection-control weaknesses that may increase risk of respiratory illness spread. Room sizes are sometimes described as small, limiting private space for residents. Prospective families should inquire about current infection-prevention protocols, HVAC maintenance, and the availability of larger or private-room options if space is a priority. Notable patterns and recommended due diligence The dominant pattern is of a facility with strong interpersonal care, effective rehabilitation services, and a welcoming environment. Counterbalancing this are repeated operational concerns around staffing consistency, certain clinical-safety processes, variable meal healthfulness, and localized odor/ventilation issues. For decision-making, families should verify: up-to-date staffing ratios, recent quality and safety reports, medication-administration safeguards, fall-prevention protocols, infection-control measures, sample menus that include heart-healthy options, and management’s process for communicating and resolving incidents. Touring different units at varied times of day and speaking directly with therapy, nursing leadership, and the social worker can clarify whether the strengths align with a given resident’s priorities and whether the operational concerns have been addressed.

About Salem Hills Rehabilitation and Healthcare

Salem Hills Rehabilitation and Healthcare sits at 537-539 Route 22 in Purdys, NY, and you'll notice right away how peaceful things feel with the Muscoot Reservoir and those gentle North Salem hills in sight, making it a nice spot if you like tranquil views when you're feeling under the weather or looking for care over the long haul. Now this place mainly focuses on long-term skilled nursing care, but it also gives a lot of help for folks who need short-term or sub-acute rehab services after a stay in the hospital or after surgery, serving as a bridge so people can get back home or into the community when they're ready. The staff has a reputation for being warm and compassionate, which goes a long way, and the personalized care plans really try to match what each person needs instead of expecting everyone to be the same.

Salem Hills has a high occupancy rate, estimated around 98%, so it stays pretty full. It accepts both Medicare and Medicaid. As a for-profit partnership, it works to balance care with business, though it's put up very good marks with the government, getting an A+ for inspection and a Better Than Average rating in U.S. News & World Report, while the overall score from past inspections sits at a B-, which shows some room to improve. It got high-performing marks for its short-stay care, which is good for families looking for reliable short-term rehab.

Even though there's no resident or family council here, the facility offers a range of specific services, including complex medical management, comprehensive wound care, pain management, IV antibiotic therapy, and speech, physical, occupational, respiratory, and cardiac therapies. Extra care comes in areas like dentistry, podiatry, pharmacy, nutritional counseling, recreational therapy, homemaking, transportation, medical transportation, and medication management. The registered staff hours are set up to support these medical and therapeutic services.

Residents can get post-acute care, respite care, and care for tough medical needs. Salem Hills has amenities and luxuries that aim to make recovery more comfortable, so rooms and areas feel somewhat like a hotel rather than a regular care facility. The admission process is flexible with "Anytime Admission," which can work well if you're moving in on short notice. The staff treats residents with dignity and respect, and the care is considered patient centered-meaning each person's plan fits their own medical and daily needs. For more, the facility's website, www.salemhillsrehab.com, lists all the services and more details about what living there looks like. Salem Hills works together with Waterview Hills Rehabilitation and Nursing Center at Purdy Station, so there's some connection between the two. Overall, it's a straightforward, comfortable place with a strong focus on rehab and nursing care in a quiet setting.
