    Unreliable, unsafe care; avoid facility

    I placed my mom here and it's been a mixed, mostly negative experience. A few staff and admins were clearly dedicated and the holiday/entertainment programs were nice, but care was unreliable - meds were often wrong, late, or withheld for weeks, meals arrived cold and awful, and communication was terrible (phones often out, staff unavailable or rude). The facility was filthy at times, had pests, high staff turnover, theft of a laptop with no resolution, and I had to involve the Ombudsman and state after complaints about assaults and unsafe practices. I do not recommend this place-investigate thoroughly before placing a loved one.
