    Clean, welcoming facility; excellent therapy

I visited and left very impressed: a clean, welcoming facility with kind, engaged staff and excellent therapy that helped my loved one regain independence quickly. Activities (shoutout to Coleen) keep residents motivated and happy, and new leadership has brought renewed energy and clear direction. I highly recommend this caring, well-run place.

    Summary of reviews

Reviews of Cabarrus Health & Rehabilitation Center present a polarized picture: many families and visitors praise the facility's therapy programs, activities, and several individual staff members, while others raise significant operational concerns. Strengths repeatedly cited include an effective therapy team (PT/OT/Speech) that supports short-term rehab goals, an engaging activities program led by a committed recreation director, and several compassionate nurses and CNAs who provide direct hands-on care. Admissions and social-work staff have been described as caring and communicative by numerous reviewers, and newer leadership has been credited with raising staff morale in some accounts.

Care quality shows a clear split. When therapy and direct nursing attention are available, residents frequently make measurable progress and families express satisfaction with clinical oversight for dementia and rehabilitation goals. Conversely, multiple accounts describe lapses in clinical follow-up, inconsistent medication administration and documentation, and cases where escalation to higher-level care was required. Reviewers also noted limited availability of some specialty services (for example respiratory/PEG/Trac support), which can affect residents with complex needs.

Staffing and conduct are recurring themes. Many reviews indicate the facility operates with chronic understaffing and high turnover; this correlates with delayed responses to call lights and assistance requests and variability in continuity of care. Positive comments about individual staff members coexist with reports of unprofessional conduct (for example, staff distraction during shifts, on-person device use, and smoking on premises) that some families felt undermined resident experience.

Dining and nutrition received mixed feedback. A number of families praised improved meal options and the ability to exercise dietary choices, while others described inconsistent food quality, problems with menu communication between nursing and kitchen staff, and lapses in kitchen processes. Observing meal service during a visit and asking about menu rotation and food-safety practices are advisable.

Activities and social engagement are clear strengths for many residents. Multiple accounts describe lively programming, high participation, and staff who meaningfully involve residents in recreational and social activities. This domain appears to be a consistently positive contributor to resident quality of life.

Facility condition and maintenance are variable. The building is described as older, with plumbing and heating issues and visible wear in some areas; at the same time, some reviewers note timely maintenance responses and comfortable rooms. Sanitation inconsistencies, odor concerns in certain areas, and pest-control issues were raised frequently enough to be a notable pattern and warrant direct inquiry. A low health-department score was mentioned by reviewers, which prospective families should verify with up-to-date public records.

Management and communications show signs of recent improvement under new leadership according to several reviewers, but administrative responsiveness remains inconsistent in other accounts. Families reported difficulty reaching staff by phone at times and expressed concern about follow-through on complaints. A small number of serious allegations (including a HIPAA-related personnel action) were reported; such items should be explored directly with administration during a tour.

In summary, Cabarrus Health & Rehabilitation Center offers strong rehabilitation and activity programming and has caring individuals on staff, but it also exhibits persistent operational challengesโ€”primarily around staffing levels, responsiveness, sanitation control, and some aspects of clinical coordination. Prospective residents and families should schedule an in-person tour that includes observing meal service and activities, asking for current health-department and staffing metrics, clarifying specialty-service availability, and requesting recent examples of corrective actions taken by leadership to address the issues above.

About Cabarrus Health & Rehabilitation Center

Cabarrus Health & Rehabilitation Center sits in Concord, North Carolina, not far from Concord High School, and people know it for its long-term and short-term senior care, and you'll hear a lot about the LifeWorks Rehabยฎ Recovery Program, which comes with custom therapy plans for each person and therapy available up to seven days a week for two to three hours a day if someone needs that, and they've also got a state-of-the-art gym with all the latest therapy equipment for people going through recovery from things like heart attacks, knee and hip surgeries, strokes, planned surgeries, accidents, or different illnesses. There are 120 certified skilled nursing beds, and the rooms come in both private and shared options, each one having its own bathroom with an easy-to-access shower or tub, so folks can be comfortable and safe, and the dining area is done restaurant-style with plenty of choices, and chefs prepare the food, making it a little more enjoyable for those who might be staying a while or just passing through for short-term rehab.

The center provides both long-term and short-term options, including respite care for caregivers who need a break, and the staff's made up of full-time employees-nurses, certified nursing assistants, physicians, physical therapists, occupational therapists, speech-language pathologists, dietitians, and culinary staff-rather than contractors, which some feel makes the care more personal, and the nurse staffing comes in at about 3.02 nurse hours per resident per day, which is less than the North Carolina state average of 3.8, and there's been a higher nurse turnover rate, sitting at around 72.3% when the state average is 50.9%.

The facility's Medicare-certified and owned by Universal Healthcare, managed by Choice Health Management, Serv LLC since 2001, with Emily Rowe as its administrator, and it's a for-profit company, which is a bit different from some places, and there have been inspection reports with 45 deficiencies as of June 2025, mostly with no actual harm but the potential for more than minimal harm, and these include citations about things like keeping residents and families informed, following treatment and care orders, making sure they honor resident choices, and preventing neglect or abuse.

Cabarrus Health & Rehabilitation Center offers memory care, skilled nursing, and a range of therapies, along with activities organized by an activities director, so there's a focus on social and recreational life, and the place uses tracking tools like the Recovery Mapโ„ข and Personal Report Cardโ„ข to help residents and their families keep up with progress and see what the next steps are. The center belongs to the North Carolina Health Care Facilities Association and has a steady census of about 104 residents most days, and even with the noted issues, many recognize the staff's compassion and their dedication to creating a caring environment for the seniors living there.
