    Gorgeous campus, caring staff, excellent

I moved in recently and I'm very pleased - the campus and grounds are gorgeous and meticulously maintained, with a lovely chapel and peaceful open spaces. The staff are warm, service-oriented and genuinely caring; they made our move-in smooth and treat residents like family. Meals are excellent (in-house chef, appealing dining), activities are varied and engaging, and on-site medical/rehab support gives real peace of mind. Overall I'm very satisfied and would happily recommend this community.

    Summary of reviews

St. Leonard CHI Living Communities presents a notably mixed profile: many families and residents praise the campus, programming, and individual staff members, while others identify recurring operational and clinical weaknesses. The campus and grounds are consistently described as attractive and well cared for, with private cottages and apartment options, an on-site chapel, and a strong schedule of social activities, special events, and outings. Several reviewers emphasize the community feel, active volunteer involvement, and the value of small-unit layouts and intimate dining rooms. When services align, in-house dining and rehabilitation resources are appreciated; some reviewers highlight high-quality meals prepared by an on-site chef, engaging activity planners, and attentive maintenance staff.

Care quality and staffing present the clearest pattern of concern. Positive accounts describe compassionate, personable aides and nurses who provide dignity and responsive assistance; however, a substantial set of comments describe chronic understaffing, frequent staff turnover, and reliance on agency personnel. These workforce pressures are linked to delays in responding to call lights, inconsistent bathing and personal-care schedules, variability in medication timing and clinical monitoring (for example, blood-glucose checks), and missed elements of planned care. Reviewers identify lapses that have led to transfers to higher-acuity care and, in some cases, serious clinical consequences. Families advising prospective residents should ask specific questions about current staffing ratios, weekend coverage, and escalation procedures.

Dining and therapy services are described as uneven. Many residents enjoy varied menus, seasonal items, and occasional special nights; others describe unappealing meal experiences, limited variety, cold or poorly timed service, and inconsistent meal observation. Therapy and rehabilitation programs are available and effective in some cases, but several reviewers cite truncated therapy minutes, therapy that did not meet the documented plan of care, and inadequate post-discharge follow-through. Those seeking short-term rehabilitation should verify therapy schedules, documentation practices, and discharge planning procedures.

Facility condition and operational maintenance vary by building. The campus overall receives praise for landscaping, walking trails, and communal spaces, but parts of the physical plant are described as dated and in need of repair — examples include older furnishings, malfunctioning climate-control in units, window treatments or blinds in poor condition, and uneven housekeeping in certain wings. There are also specific concerns about pest control and sanitation in some rooms and dining areas. Transportation systems and vehicle upkeep have been questioned; prospective families should inquire about vehicle maintenance policies and transport safety protocols.

Management and communication are another source of mixed feedback. Some families commend individual leaders and staff for responsiveness and advocacy, while others describe difficulty reaching nursing staff, slow phone follow-up, inconsistent administrative responsiveness, and frustration with placement or billing decisions (including transfers tied to payer status). A few reviews raise serious allegations about staff conduct and financial irregularities; these are individual, high-impact claims that merit targeted inquiry and review of recent regulatory inspection findings.

Overall, St. Leonard offers strong community assets — attractive grounds, a full range of living options, active programming, and many dedicated employees — but reviewers identify recurring operational risks concentrated in clinical care delivery, staffing, sanitation, and maintenance in specific areas. Prospective residents and families should tour the exact unit of interest, review recent inspection and staffing records, ask for current staffing ratios and therapy schedules, and discuss maintenance and pest-control protocols before making a placement decision.

About St. Leonard CHI Living Communities

St. Leonard CHI Living Communities sits on 240 acres of well-kept countryside about 10 miles south of Dayton in Centerville, Ohio, and was founded in 1983. The campus has a lot of trees and walking paths, and residents can choose between independent cottages, garden homes, apartments, studio units, and two-bedroom, two-bath cottages-those large garden homes with big garages sometimes have a waiting list. The community has assisted living, independent living, memory care, skilled nursing, rehabilitation, and adult day services, as well as hospice and respite care, and you'll find private, semi-private, and shared rooms you can decorate how you like. There's help for simple daily tasks like bathing, dressing, and taking medicine, and trained nurses handle medical care and rehabilitation. Memory care programs help folks with Alzheimer's or dementia, and the campus provides brain-stimulating activities and personalized care plans, plus activities like crafts, outings, and music on a structured calendar led by dedicated staff. Families can get help with needs assessment and support from senior living advisors, and the staff, including chaplains and spiritual care managers, tries to involve families and caregivers whenever possible so everyone feels included.

St. Leonard has a Wellness Center, well-maintained grounds, and lets residents enjoy both indoor and outdoor community spaces, plus they can keep pets if they want. There's a Station House Restaurant open to the public with meals by an in-house chef, and a housekeeper service keeps living spaces clean. Transportation is available for errands or outings, and amenities also include complimentary rides, on- and off-site devotional services, activities, library fund, and activity funds to support art, crafts, and events like BoomerFest, which is a special music festival designed for the Baby Boomer generation. There's even a kids' area for use during events, so when families visit, young ones have a place to play.

Faith-based programs are a big part of daily life at St. Leonard, with ministries for Faith Formation, Community Life, Worship, Justice & Service, Membership, and Pastoral Care. Residents can take part in social activities like community picnics, ice cream socials, Holy Thursday dinners, and church events. Sacramental services, and a church facility are available, with chaplains, spiritual care managers, and the Pastoral Care committee offering prayers, sacraments, and visits during illness or loss.

The community belongs to CHI Living Communities, a nonprofit tied to CommonSpirit Health and Franciscan Services Corporation, and has been recognized in Dayton for its independent living, assisted living, and memory care thanks to friendly staff and a range of enrichment programs. The mission stays rooted in Christian values and helping families make choices with information and compassion. Adult day care supports family caregivers by giving seniors a place to stay safe and engaged during the day. Most of the staff knows residents and their families, and people often comment on how kind and joyful everyone is. There are funds and foundations for library use, art activities, support for veterans, pet therapy, and more. The campus feels secure and the atmosphere is relaxed and welcoming. The overall community rating is 3.0 stars, with reviews from residents and families, and while it's not perfect, people generally find comfort and a strong sense of belonging there. St. Leonard provides suites where residents can express themselves, offering affordable housing, independence, care, and friendly faces in a setting focused on dignity and spiritual growth.
