    Excellent care and attentive staff

I've been very pleased with Patriots Ridge - therapists and medical staff provide excellent rehab and long-term care, caregivers are consistently warm, patient and attentive, and the community truly feels like family. The facility is very clean, offers activities, respite and VA/Medicaid support, staff keep me updated, and I'd recommend it, though there's always room for small improvements.

    Summary of reviews

The Trinity Community at Fairborn elicits strongly mixed impressions across reviewers. Many families and residents praise the caregiving staff, therapy services, and rehabilitative capabilities: clinical teams, therapists (physical, occupational, speech), and some nursing staff are described as skilled, compassionate, and effective at short-term rehabilitation and respite care. The community is noted for a family-oriented culture, activity programming (including arts), laundry and housekeeping services, and coordination with payers such as Medicaid and the Veterans Administration. Several accounts describe smooth admissions and transfers and consistent staff assignments that supported family communication and continuity of care.

Despite those positives, there are recurring operational concerns that prospective families should weigh. Staffing levels and turnover appear inconsistent; reviewers describe occasions of being short-staffed or experiencing high staff churn, which correlates with delays in direct care tasks. Several reviewers raised issues about incontinence care and bathing schedules that were inconsistent or delayed, and some linked those care gaps to skin integrity problems and unplanned hospital transfers. There are also accounts of medication-management and post-surgical assistance problems, including delayed medication communication or limited help with feeding and mobility after procedures.

Cleanliness and environment receive mixed assessments. Some reviewers describe clean common spaces and well-kept activity rooms, while others report sanitation and odor concerns in particular units (notably a memory-care area). Facility condition is uneven: communal areas and therapy spaces are generally functional, but resident rooms are frequently described as older, darker, and in need of updates; limited natural light in some areas contributes to a lower ambient brightness.

Dining and activities are commonly cited as strengths, with seasonal menus and a la carte options appreciated by many. At the same time, meal-service continuity and dietary variety have weaknesses: meal length and limited vegetarian choices were noted, and some residents required assistance at meals that was not consistently provided. Activity programming and social engagement—especially arts and religious services—are highlighted as positive contributors to quality of life.

Management and administrative issues are a frequent source of concern. Families referenced inconsistent communication between administration and nursing during clinical incidents, billing and refund-processing problems including delays in refunds or held funds, and complaints about personal-property safeguards. A small number of serious interpersonal and conduct concerns were raised regarding administrative behavior and staff relations; these point to potential gaps in human-resources handling and equitable treatment. When clinical issues escalated, some reviewers described gaps in incident-response and family notification that led to emergency department visits.

Overall, Trinity Community at Fairborn appears to offer robust rehabilitative services and many caring staff who create a supportive, family-like environment. However, variability in day-to-day staffing, lapses in some aspects of personal care and sanitation in specific units, and administrative inconsistencies (communications, billing, and property safeguards) create uneven experiences. Families considering this facility should plan an in-person tour focusing on staffing patterns for the prospective unit, hygiene and odor control in memory-care areas, meal-assistance capability, medication and post-surgical protocols, and clarity on billing/refund procedures before admission.
