    Warm caring staff, excellent care

I placed my mom here and had a very positive experience - the staff were warm, patient, and treated her like family. Nurses, CNAs and the therapy team were exceptional, social services and management were responsive, activities were plentiful, and the facility was clean with good meals. Staff consistently went above and beyond, and I would highly recommend it.

    Summary of reviews

Reviews of Forest Hills Care and Rehab present a highly mixed picture. Many accounts praise the facility’s rehabilitation services and therapy teams, noting effective physical and occupational therapy, helpful PT/OT staff, and successful short-term recoveries that enabled discharge home. Housekeeping, room cleanliness, and some aspects of facility upkeep are also frequently described positively, and an active activities program and pet-friendly visitation policy are cited as quality-of-life strengths.

At the same time, a persistent cluster of operational concerns appears throughout the feedback. Staffing levels and on-floor coverage are recurrent themes: families describe frequent delays in answering call bells, long waits for toileting or assistance, and limited on-floor help during meals. These delays are tied to broader safety and clinical worries, including inconsistent bathing and incontinence-care schedules, delayed wound and dressing changes, delayed or missed diagnostics and medication administration, and weaknesses in transfer and fall-prevention practices. Several accounts raise concerns about infection-control practices and hand-hygiene adherence.

Dining and dietary accommodation are uneven. Reviewers describe variability in meal quality, late meal service, and difficulty obtaining appropriate renal or diabetic menu options. Some families also reported insufficient portions or meals that did not meet special dietary needs. Communication and coordination with families and outside providers are additional areas of concern: delays or gaps in phone callbacks, discharge paperwork, medical-records transfer, and social-work availability were cited multiple times. There are also reports indicating restrictions on outside physicians and delays or denials for equipment prescriptions.

Management and administrative patterns are mixed. Reviewers describe periods of leadership turnover with new administrators and directors; some note clear improvement and compassionate oversight under new leaders, while others report defensive or unresponsive management behavior. High staff turnover, agency staffing use, and signs of inadequate documentation practices were described. More serious allegations appear in a subset of accounts, including claims of theft and problems with property accountability and billing; these items suggest the need for families to inquire specifically about inventory and financial safeguards.

For prospective residents and families: Forest Hills demonstrates clear strengths in rehabilitative therapy, engaged activity programming, and dedicated caregiving staff in many areas. Simultaneously, persistent operational weaknesses—especially around staffing consistency, call-response times, medication and wound-care reliability, dietary accommodations, and family communication—warrant careful, proactive assessment. Suggested due diligence includes asking for current staffing ratios, average call-bell response times, protocols for medication administration and wound care, recent inspection/citation history, procedures for handling personal property and billing, and references from recent families whose goals align with your needs (short-term rehab versus long-term skilled nursing).

About Forest Hills Care and Rehab

Forest Hills Care and Rehab sits in a quiet suburban area of Tulsa where you can see lovely country views, and the whole place feels peaceful which can help people feel calm when they're healing or settling in for a longer stay, and even though it's a nursing home, you'll notice thoughtful touches like safety features with fire and medical alert systems and some floor plans even have kitchenettes and bathrooms for folks who need them, so residents can keep some independence while staying safe. The staff offers different services that support seniors, including long-term and short-term skilled nursing care, and that means people who need help for a short while after being in the hospital, or those who need more constant support, can both find care here, and with things like 24-hour nursing, consultant care, and therapy rehabilitation, there's help day and night, and the staff tries to deliver care with respect and compassion, which helps everyone keep their dignity. Residents get meals made by a chef and cooking staff who use fresh produce, and the dining areas encourage conversation, while the rooms in some areas like the assisted living spots can have private spaces to give folks some quiet when they want it. Centers like Artesian Home and Aspen Health & Rehab are part of their network, so care can happen across a few locations if needed, and assisted living services can also be adjusted to suit what each resident wants or needs, whether that's just a little help with daily things or a bit more structure. Forest Hills Care and Rehab is close to shopping, banks, and other places people might need, so it's convenient for families and residents alike, and with amenities for daily living and a safe environment, the focus stays on helping seniors feel empowered while still having support around every day.
