    Attentive, respectful staff; trustworthy care

I'm very pleased with Heritage - the staff are incredible, attentive, and respectful; administration communicates and responds quickly, and medical and therapy care is consistently excellent. The building feels clean and home-like, meals are flexible, visits are accommodated, and my family feels well supported - not flawless, but I trust them completely.

    Summary of reviews

Experiences at Heritage Skilled Nursing & Therapy are mixed, with families and residents describing both strong, compassionate care and operational weaknesses that affect outcomes. Many families praise individual caregivers โ€” aides, nurses, and therapists โ€” for attentiveness, emotional support, and effective rehabilitation services. Several accounts highlight quick family notification, preservation of dignity, and a home-like atmosphere with tidy common areas and visitor accommodations such as chairs and window visits.

At the same time, recurring operational concerns are evident. Staff professionalism and conduct appear inconsistent across shifts; while some staff are described as deeply caring and skilled, others are described as brusque or unprofessional. Understaffing and variable staffing levels were cited frequently, and that shortage is associated with delayed personal care (including inconsistent bathing and toileting assistance), missed or irregular medication timing, and gaps in assistance to the bathroom. These failures in routine care are linked in some narratives to fall-safety problems and delayed medical attention.

Dining and activities show variability as well. Several families appreciated flexible meal options and alternatives, while others found the food bland or preferred more from-scratch preparation. Recreational engagement also varied: some residents and relatives described a friendly, home-like social environment, whereas others described limited activity programming and periods of isolation or insufficient engagement, which contributed to residents appearing inactive for extended periods.

Facility maintenance and sanitation receive mixed commentary. The entrance and many common areas are often described as clean and welcoming, and the facility was noted by some to lack a stereotypical institutional odor. Conversely, some reviews raise sanitation and cleaning frequency concerns in resident rooms and bathrooms. Room amenities vary; some rooms include visitor seating but others lack in-room entertainment such as a television.

Management and administrative practices are another area of inconsistency. Some families reported clear communication, responsive administration, and dignity-respecting processes during stays and during infection-control visits. Other families described problematic interactions around discharge paperwork, visitation restrictions, and consent/medication processes. Serious individual allegations โ€” including theft and inappropriate use of restraints โ€” were raised and warrant further inquiry through facility management or regulators.

In summary, Heritage demonstrates meaningful strengths in individual caregiver skill, rehabilitation services, and certain aspects of facility upkeep and family communication. However, persistent operational issues โ€” especially staffing variability, inconsistent personal-care routines, medication- and safety-process gaps, and uneven management practices โ€” create variability in resident experience and clinical outcomes. Prospective residents and families should tour the unit(s) of interest, ask specifically about staffing levels and shift coverage, medication-administration protocols, bathing and toileting schedules, fall-prevention procedures, dining menus, activity programming, and how the facility handles complaints, visitation exceptions, and allegations of misconduct. Following up with recent family references and observing care during different shifts can help assess whether the facilityโ€™s strengths are reliably delivered.

About Heritage Skilled Nursing & Therapy

Heritage Skilled Nursing & Therapy sits at 201 W Walnut St in Tecumseh, Oklahoma, and it's the sort of place where folks can get help when they need skilled nursing care, therapy, or support for daily living. This nursing home isn't in a hospital, but it offers 100 certified beds, and the rooms come in studio and semi-private layouts, plus options to share if somebody wants a companion room, and they keep the rooms fully furnished with private bathrooms and services like air-conditioning, cable, telephone, and Wi-Fi. Residents have access to different types of care like skilled nursing, long-term care, assisted living, and memory care, with round-the-clock nursing support, a 24-hour call system, and staff to help with bathing, dressing, transfers, and medication management, so there's help for those daily tasks that get a little harder with age.

The facility has served 149 discharges in the period ending December 31, 2023, with an average stay a little over 161 days, filling a total of 24,094 inpatient days, which shows a mix of both short and long-term residents, and they do participate in both Medicare and Medicaid, with a Medicare Provider Number 375446 for those needing that kind of coverage. Heritage offers physician services that include primary and preventative care, plus nursing services like cardiac care, respiratory support, diabetes management, wound care, and comprehensive rehabilitation. For those who need therapy, they provide occupational, physical, and speech therapy, and also cover mental wellness and specialty care programs.

Meals come with flexible options, including restaurant-style dining, balanced menus, and in-room service for those not able to get to the dining room, and the staff helps with dietary restrictions, meal prep, and clean up, while weekly housekeeping and linen service keep rooms tidy. Heritage also has a beauty salon, barber services, a computer center, fitness and wellness rooms, a garden, small library, game room, and other relaxing common spaces where residents can spend their time how they like. Folks can take part in activities like painting, movie nights, morning meditation, planned day trips, social groups, educational programs, or even resident-organized activities, so there are ways to socialize or enjoy some quiet if that's what's wanted, and community-operated transportation helps folks get around for non-medical trips too.

When inspectors came by, the facility had a fire survey on April 10, 2024, with 4 deficiencies, and a health survey on November 16, 2023, with 10 deficiencies, so there are areas for improvement even though staff aim to provide a safe place. It has a 2-star rating from the Centers for Medicare & Medicaid Services and a community rating of 5.4 out of 10, with a 2.9 star average based on 17 online reviews, meaning people have mixed experiences. Heritage Skilled Nursing & Therapy is proprietary with a focus on providing support for seniors with complex health needs, tailored care, and community engagement, so folks can expect a mix of dedicated nursing, therapy, recreation, and daily living support all in one place.
