    Compassionate dementia care; highly recommended

I placed my loved one here and have been very pleased. The staff are caring, compassionate, and respectful - nurses, techs, and the administrator go above and beyond and keep us regularly updated. Daily assistance, dementia-focused care, emotional support, and activities like art, manicures, and outdoor time have noticeably helped. The facility is clean, well-maintained, and resident-centered. I trust the team and would recommend them.

    Summary of reviews

Overall impression: Reviews show a facility that many families and residents praise for its compassionate caregiving, dementia-aware approach, and well-maintained physical plant, but also exhibit a distinct minority of strong operational concerns. A majority of comments emphasize attentive nurses, caring assistants, and a management team willing to assist with administrative matters such as Medicaid paperwork. Several reviewers explicitly recommended the facility and contrasted it favorably with previous placements.

Care and staff: Strengths cited include consistent emotional support, daily assistance, and staff who understand dementia-related needs. Many families noted improvements in their loved ones’ condition under the current care team and described specific staff members (nurses, techs, and the administrator) as dedicated and resident-centered. However, a number of critiques point to inconsistent staff conduct and responsiveness. These critiques cluster around slower response times, variability in attitude and attentiveness among staff, and occasional delays in addressing resident needs — suggesting staffing or supervision variability between shifts or units.

Facilities and cleanliness: The facility is frequently described as clean, modern, and well-maintained. That said, a subset of reviews raised sanitation concerns and incontinence-care issues in certain areas, along with isolated comments about odor concerns and delays in room or bedding maintenance. There were also mentions that bed- or transfer-related safety practices could be improved, indicating potential gaps in fall-prevention and transfer protocols.

Activities and dining: The facility offers dementia-focused activities, outdoor time, art projects, and personal-care services like manicures; an incoming Activity Director was noted as a planned improvement. At the same time, some reviewers said activity programming could be expanded or more consistently delivered. Reviews provided limited information about dining quality and menu variety, making it difficult to assess food service from the available comments.

Management and communication: Leadership receives mixed feedback. Several families praised an administrator who is actively involved and responsive, and multiple comments highlighted helpful office staff and fast service. Conversely, there are isolated but notable remarks about poor communication, missed appointments, and unprofessional conduct by management in some cases. These divergent accounts point to unevenness in administrative follow-through rather than a single uniform experience.

Notable patterns and guidance for families: The dominant pattern is one of generally good, dementia-capable care and a clean facility with engaged staff; however, there are clear signals of inconsistency in operational execution — particularly around responsiveness, sanitation/incontinence care in specific areas, and safety practices for transfers. Prospective families should tour the unit, ask for current staffing ratios, request documentation of cleaning and incontinence protocols, review fall-prevention and transfer procedures, and obtain a sample weekly activities schedule. Asking how the facility handles family communication, missed appointments, and Medicaid paperwork will help clarify whether the strengths noted in many reviews are consistently applied to a given resident’s care.

    About Green Country Care Center

Green Country Care Center is a skilled nursing facility with a 3-star rating from the Centers for Medicare & Medicaid Services and a 4.8 out of 10 rating based on reviews, so it's not the highest rated but it is recognized for the services it does provide, and it's a place that offers memory care, skilled nursing, rehabilitation, and assisted living for folks who need help with daily life, and it stands out as one of only three licensed specialized facilities for Alzheimer's care in the state of Oklahoma, giving support to people with dementia or similar conditions, and the community takes care of people who can't live alone safely anymore but don't always need around-the-clock skilled nursing, though they do have 24-hour supervision and a call system for emergencies. Residents live in either private suites or shared rooms, which are fully furnished and include private bathrooms, air conditioning, and cable and Wi-Fi, plus there's access to meal services including restaurant-style preparation, help for different diets, and a dining room, and they've got community areas like a game room, library, fitness room, salon, and wellness spa, along with green spaces and quiet spots for people who need rest or recovery. Nurses, medication aides, nurse aides, dietary staff, and maintenance keep the facility running, while each resident gets help with bathing, dressing, and getting around, along with medication management, laundry, housekeeping, and transportation to doctor's appointments, banks, malls, and hospitals, so folks have what they need even if they can't drive or walk far. The medical team includes RNs, LPNs, CNAs, appointed physicians, and consultants in pharmacy and diet, and the staff watches for changes in residents' health, offering physical, occupational, and speech therapy as needed, and anyone with complex medical needs or in recovery from illness, accident, or surgery can get support to stabilize and regain health. The building's secure, with special areas designed for people at risk of wandering and regular activities organized to keep everyone active and social, plus they offer hospice care for people with advanced illness and provide emotional support through memory care teams. The facility accepts Medicaid and Medicare, and both English-speaking and bilingual caregivers are available, so people from different backgrounds feel comfortable and understood. Green Country Care Center aims to help older adults who can't live alone anymore, providing care, meals, supervision, and a safe environment with regular medical oversight and activities to bring neighbors together.
