    Resident praises caring, responsive staff

I've lived at Maplewood Care Center four years under new ownership and I'm very pleased. Caring, responsive nursing and therapy teams (excellent SLP and dialysis support), strong leadership, and an active activities program keep me safe, engaged, and comfortable in a clean, renovated building with great meals and a scenic downtown view. Communication during COVID (window visits/FaceTime) and quick responses to concerns reassured my family - I recommend this home-like community.

    Summary of reviews

The review corpus for Maplewood Care Center is highly polarized: a number of reviews describe meaningful clinical and quality-of-life strengths, while a substantial set of reviews raise recurring operational and safety concerns. Prospective residents and families should weigh evidence of strong therapy outcomes and active programming against repeated concerns about clinical oversight, environmental maintenance, and administrative practices.

Care quality: Several reviewers praise the facility's therapy teams (physical and speech therapy) and describe measurable functional gains after skilled therapy. There are also reports of consistent physician access and effective specialty services such as dialysis. Counterbalancing those accounts are repeated concerns about medication management (including documentation and administration controls), wound care delays, and insufficient postโ€‘operative attention. Falls and transfer-safety issues are mentioned repeatedly, along with instances of delayed clinical response and unclear incident documentation and followโ€‘up.

Staff: Many individual staff members, therapists, activity leaders and some nursing leaders receive positive mention for compassion, responsiveness and clinical skill. At the same time, reviews describe uneven staff conduct, lapses in responsiveness to call bells, inconsistent assistance with toileting and bathing, and concerning communication tone in some interactions. Reviewers also raise workforceโ€‘level problems: high turnover, low morale, episodes of poor supervisory followโ€‘through, and confusion about staff roles that can impede continuity of care.

Dining and activities: Dining receives both positive and negative remarks. Several reviews note three daily meals, pleasant dining rooms, and active, social mealtimes with staff assistance. Others describe intermittent meal-service or dining-area concerns. Activities programming is consistently listed as a strength in many accounts, with regular events (bingo, karaoke, holiday programs), music, and social offerings that contribute to residentsโ€™ engagement.

Facilities and environment: Some visitors and families report renovated common spaces, updated flooring, and an attractive outdoor courtyard. Conversely, multiple reviewers cite sanitation and odor concerns in particular areas, pestโ€‘control issues, inadequate lighting in some rooms and hallways, and maintenance problems such as water damage or unfinished repairs. Security and buildingโ€‘access controls are also flagged as uneven, and some families describe poor room conditions or inconsistent housekeeping in parts of the facility.

Management and administrative patterns: Reviews portray a mixed picture of leadership. Several comments single out particular administrators, the director of nursing, or therapy leaders for strong, handsโ€‘on involvement and effective responsiveness. Other reviews raise issues about billing, financial transactions and document handling; there are explicit allegations concerning financial misconduct and forged documentation in a small subset of accounts. Families also describe difficulty escalating concerns, inconsistent incident reporting, and at times what they perceive as retaliatory treatment of staff who raise issues.

Notable patterns and guidance: The dominant pattern across reviews is variability โ€” several clear areas of strength coexist with recurring and specific operational weaknesses. Positive experiences are often associated with engaged therapy teams, active programming, and individual caregivers who provide attentive care. Negative experiences cluster around medication and woundโ€‘care processes, staffing shortages at offโ€‘peak times, environmental maintenance, sanitation and pest control, and serious administrative/financial allegations. Prospective families should consider an onโ€‘site, timeโ€‘ofโ€‘day visit (including meal and activity times), ask for current state survey and incident reports, review medication- and woundโ€‘care protocols, and seek clarity on billing and property-protection procedures before making placement decisions. Where serious financial or safety concerns are suspected, families may also consider verifying whether regulatory or protectiveโ€‘services actions are open and request documentation of corrective plans from facility leadership.

    About Maplewood Care Center

Maplewood Care Center sits at 6202 E 61st St in Tulsa, serving seniors who need nursing care, memory care, and support with daily living, and you'll find it's been open since 2002, run by Senior Care Group Inc and under Charles Rhoades' management since 2016, which means there's a long history and plenty of experience here. Residents with Alzheimer's or dementia get special attention in the Known Memory Care at Tulsa unit, and there's skilled nursing, comfort and palliative care, as well as all the services you'd expect in a nursing home, including long-term care, short-term rehab, wound care, and even dialysis. You'll find 180 certified beds and amenities for health and well-being, plus renovated spaces and friendly staff, and the team keeps a focus on making it home-like, clean, and comfortable, like with the new floors and fresh paint. Maplewood offers things like nutritional counseling, behavioral care, transportation, and help for family caregivers, and there's information and guidance on long-term care insurance. For people needing higher levels of care, Maplewood also provides high acuity care, hospice services, therapy, and dedicated staff trained on senior needs. While there's a focus on compassion and meeting each person's needs, the facility does have a history of serious quality problems, with nurse turnover higher than the state average, and past deficiencies on things like timely reporting of suspected abuse and keeping areas safe from hazards, but none were found to have caused actual harm, even though they had the potential for more than minimal harm, and government inspectors have not flagged Maplewood, though it's listed as a Special Focus Facility Candidate. Maplewood is a non-profit and holds high rankings and accreditations from health organizations, but it's not accredited by the BBB even though it follows BBB standards for trust, with an A+ rating and an active business profile. Staffing averages 3.73 nurse hours per resident per day, so there's a decent level of hands-on care, even though the nurse turnover is high. The environment gets described as high-energy and friendly, and reviews score it 3.5 out of 5, based on 66 accounts, so people have mixed experiences. Families and residents can expect common nursing home features, a team committed to patient well-being, and a continued effort toward improvements, innovation, and reliable care for seniors in Tulsa.
