    Caring staff excellent therapy inclusive

I'm very pleased with the care my mother receives here. The staff are incredible and always smiling; nurses and therapists provide excellent, skilled rehab (private therapy rooms) and quickly resolved a PT hiccup. In-house physicians are on site four days a week and available by phone, activities are plentiful and inclusive, and services like laundry, supplies and a beauty shop are included. The tour was pleasant and informative - I'd recommend this facility to family and friends.

    Summary of reviews

Reviewer input describes a facility with clear strengths in post-acute rehabilitation and certain resident services alongside persistent operational weaknesses. Therapy services are a consistent positive: skilled therapists, private therapy rooms, and generally strong rehab outcomes were highlighted. Several accounts also note an engaged and friendly caregiving staff whose interactions and greetings create a positive day-to-day environment for residents. Onsite conveniences such as laundry, included supplies, a beauty shop, multiple activities with inclusive participation, and in-house physician coverage contribute to continuity of care. The facility accepts a range of payment sources and has been recommended to family and friends by some correspondents.

At the same time, reviewers identify facility-level issues that affect the resident experience. The building is described as aging, with deferred maintenance needs (peeling paint, damaged surfaces, and window issues) and small, hospital-style rooms that limit private space. Cleanliness and sanitation concerns in both rooms and common areas were raised repeatedly. Private bathing options are limited, and some noted the absence of in-room communication amenities. Dining service appears inconsistent: several accounts cite variable meal quality and meal deliveries that arrive late or at undesirable temperatures.

Clinical operations and staffing are mixed. While some families praised clinical outcomes and the presence of onsite physicians several days per week, others described staffing shortages—particularly evenings and weekends—that manifest as delayed responses to call lights and assistance requests. High staff turnover and workload pressures were linked to inconsistent adherence to nursing procedures and gaps in clinical follow-up, documentation, and laboratory-result communication; one review referenced serious concerns following a resident's death and related questions about clinical follow-up. Therapy resources are generally strong but at times constrained by limited therapy space and equipment.

In summary, Southern Hills Rehabilitation Center appears to offer effective rehabilitation services and a caring staff presence for many residents, along with useful onsite amenities. Prospective residents and families should weigh these strengths against facility condition, cleanliness, meal-service reliability, and staffing patterns—especially during evenings and weekends. Given the polarized experiences described, prospective families may wish to tour the facility during different shifts, ask about recent staffing levels and turnover, review clinical follow-up protocols (including lab-result communication), and observe meal service and housekeeping practices before making a placement decision.

    About Southern Hills Rehabilitation Center

Southern Hills Rehabilitation Center sits in Tulsa, Oklahoma, in a neighborhood close to pharmacies, clinics, parks, and places of worship, and the location makes it easier for folks to run errands, go to appointments, or visit family and friends. The center is part of a Continuing Care Retirement Community, which means people can get assisted living, independent living, memory care, and skilled nursing care all in one place, and you'll often see residents involved in their own activities and daily programs that help keep them engaged and social. They've got certified beds-41 out of 106 were open in June 2025-and the usual census is about 64 residents, with both private suites and shared rooms, all furnished and offering private bathrooms, air conditioning, Wi-Fi, and cable TV, and you'll see amenities like walking paths, gardens, arts rooms, a library, and a fitness space.

For care, they've got a team made up of certified nurses, licensed medical practitioners, nursing assistants, and physical therapists, so you get help with bathing, dressing, transfers, and medicine management, and there's always supervision because staff are on site around the clock with a 24-hour call system. They're able to look after folks who need short-term rehab between the hospital and home, help people with memory loss in a secure area, and provide care to frail residents who need lots of hands-on help day and night. You'll find the dining is taken care of, with restaurant-style meals, diets for diabetes or allergies, and the option for all-day dining, plus housekeeping, laundry, and move-in help, so you don't end up worrying about chores.

They accept Medicare and Medicaid, which helps some folks pay, and help arrange trips to doctor's appointments when needed. There's a business room, community theater for movie nights or music, activity rooms for crafts or games, and they've got programs run by residents themselves. Southern Hills is managed by Selectis Management LLC since December 2020, overseen by Corporate Operations Director Jeanette Ross and administrator Christy Aubrey, and is affiliated with Global Healthcare REIT.

The environment feels friendly and the staff seem dedicated, but there have been some inspection reports showing deficiencies, including issues with resident rights, nutrition standards, food handling, and infection control, and there's been a nurse turnover rate of about 76%, with nurse staffing hours per resident at 3.79. Still, people say it's a place focused on dignity and self-respect, with family included in care decisions, secure areas for memory care, and a whole range of therapy options like physical, occupational, and speech therapy, so residents can work on recovery or keep as independent as possible, all while being in a place that tries to look after both medical needs and comfort.
