    Excellent skilled nursing and rehab

I was very pleased with my stay at Grace - phenomenal doctors, compassionate nurses who kept my family informed, and an excellent PT/OT/speech team that got me back on my feet quickly. Dawn and the rest of the staff were professional, caring, and responsive, the meals were good, activities plentiful, and the facility felt clean and welcoming. I recommend this team for high-quality, attentive skilled nursing care.

    Summary of reviews

Overall impression: Reviews of this facility vary considerably, producing a mixed picture. Many families and residents praise the therapeutic services, medical clinicians, and individual caregivers for effective rehabilitation and attentive bedside care. At the same time, recurring operational weaknesses—especially around staffing, cleanliness, and communication—are frequently cited and appear to create uneven experiences across different stays.

Care quality and clinical management: Therapy services (PT/OT/speech) receive consistently positive mentions for facilitating functional improvements during short-term rehab. Several reviewers also described strong medical oversight from physicians and responsiveness from specific clinicians. Conversely, there are recurring concerns about inconsistent nursing coverage, missed or delayed medication administration, infrequent physician rounds in some cases, and uneven handling of clinical changes. A subset of accounts describes serious adverse clinical situations and delayed escalation; these underline a broader pattern risk related to incident response and care coordination that prospective families should monitor.

Staff and communication: Many reviewers highlight compassionate, dedicated caregivers and approachable management who communicate proactively about health changes. Staff were described as going above and beyond in numerous instances. However, other accounts describe variable staff professionalism, rushed or terse communication, and pressure toward early discharge. These contrasts suggest staff behavior and communication quality are inconsistent—often dependent on unit staffing, shift, or individual team members. Family engagement and responsiveness appear strong at times but unreliable at others.

Dining and activities: The dining team and menu receive positive feedback for pleasant service and satisfying meals for many residents; flexible dining options were noted. At the same time, there are reports of occasional food-safety and quality problems and interruptions to meal service. Activity programming is a clear strength for some residents, with organized events, games, religious services, and intergenerational visits contributing to a home-like environment. Other reviewers, however, described limited entertainment or insufficient activity frequency, indicating variability in program availability.

Facilities and sanitation: The building is described as older with small resident rooms and limited communal front space. While some described clean, well-maintained rooms and a cheery atmosphere, others raised sanitation concerns including odor issues, mold, and inconsistent housekeeping. Laundry and personal-belongings handling are additional areas of operational weakness, with multiple complaints about lost or mishandled items.

Management and notable patterns: Management is seen as responsive and supportive by many staff and families, and some reviewers praised a positive workplace culture. Nonetheless, systemic issues persist: inconsistent staffing patterns, lapses in infection-control practice during outbreak management, and pressure to discharge residents earlier than families expected. There are also allegations related to theft and privacy breaches; such claims should prompt families to seek clarification and review facility policies and safeguards. Prospective residents and families may benefit from in-person observation of staffing patterns, reviewing recent inspection reports, and discussing discharge planning and incident escalation processes with leadership before admission.
