    Caring staff and welcoming facility

I'm very pleased with Yorkview - the staff are caring, professional and attentive (nurses, aides, social worker Bekah and the business office showed real expertise and kindness). The building is clean, welcoming with good dining, rehab support and accommodating visits, and the smooth transition gave me real peace of mind.

    Summary of reviews

The reviews present a polarized picture of Yorkview Nursing and Rehabilitation, with distinct clusters of positive and negative experiences. Many families and visitors highlight committed, compassionate staff members, helpful reception and business-office personnel, effective rehabilitation services, and pleasant common areas and views. These positive accounts describe staff who are approachable, communicative, and able to provide reassurance and continuity for families during transitions and visits.

Conversely, there is a recurrent pattern of operational concerns that appear linked to staffing and consistency. Numerous accounts describe long waits for assistance, delayed medication delivery, missed or delayed personal-care tasks (including bathing and linen changes), and inconsistent responsiveness to call systems. These delays are frequently associated with perceived staffing shortages or scheduling gaps, which reviewers identify as a contributor to safety and comfort issues.

Sanitation and maintenance are another recurring theme with mixed reports. Some reviewers describe clean, well-kept areas and timely housekeeping, while others cite persistent cleanliness issues, odor concerns in parts of the building, and pest-control concerns. Facility-aging issues such as worn carpeting, broken blinds, yellowed fixtures, and climate-control variability are noted and are sometimes connected to the perception of an outdated environment.

Dining and nutrition received mixed feedback: several reviewers praised hot meals and satisfying dining options, but many others reported cold or late meals, limited protein or vegetable options, and inconsistent accommodation of special diets (for example, diabetes management). Families also reported variability in food-service reliability and in the replacement of missing items or preferences.

Safety and clinical-quality concerns recur in multiple areas: gaps in incontinence care, pressure-ulcer risk (requests for specialty mattresses), weak transfer-safety practices, and fall risk because of limited supervision or missing safety equipment. Medication timing and communication around therapy schedules were also described as inconsistent. A small number of serious individual claims — including an allegation of theft and concerns following a resident’s death — were raised; these appear isolated but warrant scrutiny given their severity.

Management and culture appear to be in flux for some reviewers. Several accounts praise recent leadership changes, improved communication, and staff accountability, while others describe variable professionalism, tone concerns in staff interactions, and limited family updates. Overall, the facility shows strengths in individual caregiver dedication, therapy services, and some administrative functions, but persistent operational weaknesses — particularly in staffing consistency, sanitation and maintenance, dining reliability, and safety processes — create a mixed quality profile. Prospective residents and families should consider an in-person assessment focused on staffing levels, sanitation practices, care-response times, dietary accommodations, and specific safety measures (fall prevention, pressure-area prevention), and should request current information on management initiatives addressing these concerns.

    About Yorkview Nursing and Rehabilitation

Yorkview Nursing and Rehabilitation sits in Grantley, PA, and has provided care since 1996, bringing local support for nearly three decades, and this facility blends skilled nursing, assisted living, memory care, and rehabilitation in one place, so older adults can stay even if their needs change over time, which many families find helpful. The community has a steady, smaller staff but still covers both short-term help, like after a hospital stay, as well as longer stays for chronic conditions, with skilled nursing available most hours of the day and a 24-hour call system for emergencies or help, and those recovering from illness, surgery, or injury can get therapies and round-the-clock support that helps them regain or keep skills needed for daily life, with care adapted to each person. They do offer programs tailored for people with Alzheimer's and dementia, with safe spaces and memory-focused activities and supervision, and this adds an extra layer of comfort because the environment is set up to protect seniors who may wander or need more structure.

The place has private suites with air conditioning, private bathrooms, and kitchenettes, and each room comes fully furnished, which makes moving in easier; plus, the common areas include a dining room, beauty salon, computer center, garden, fitness and gaming rooms, a small library, and an outdoor space where people can relax or join in programs and activities, and WiFi, cable, and telephone services are available, so folks can stay connected. Meals come made by professional chefs with restaurant-style dining that handles special diets or restrictions, and daily housekeeping and linen services help keep everything clean.

Yorkview has transportation options with shuttle service and arrangements for non-medical trips, making it possible for residents to go out for errands or enjoy local events, and the staff encourages social, emotional, physical, and mental engagement through scheduled daily activities, day trips, and resident-run programs, plus they offer help with activities of daily living and medication management for those who need it. For caregivers, the respite care option lets families get a break while their loved one receives care. Hospice is also available for those needing comfort and dignity as needs progress.

The focus at Yorkview stays on expert medical care, therapy, and nursing support with skilled staff, rehabilitation services, and memory care in secure settings, but the atmosphere puts a value on social life and personal well-being, and a variety of amenities support these priorities. The facility holds an average rating and is known for dependable care rather than luxury or promise of perfection, making it an option for families seeking stability, a secure environment, and qualified help for many different phases of aging or healing.
