    Kind professional home-like rehabilitation care

I placed my mom here and have had a very positive experience. The staff are kind, patient and professional, communication is excellent, therapy helped with rehab, and the facility is clean, welcoming and full of activities - it truly felt like home. They provided compassionate support through every stage and I gladly recommend this caring, family-centered community.

    Summary of reviews

Azle Manor elicits strongly mixed impressions. Many families and former residents praise the rehabilitation services and therapy teams, describing effective physical and occupational therapy that supported successful transitions home. Nursing assistants, therapists, and some nurses receive frequent recognition for compassionate, patient-focused care, one-on-one feeding assistance, and attentive end-of-life support. The facility's physical environment is commonly described as clean, bright, and well maintained, with spacious rooms, attractive indoor features (aquariums, birds), and pleasant outdoor spaces that support visiting and activities.

At the same time, recurring operational concerns appear across reviews. Staffing levels and consistency are a persistent theme: families describe variable nurse and aide availability, which relates to delayed responses to call systems, inconsistent personal-care routines (bathing and grooming), and uneven monitoring. Several comments indicate problems with medication administration and communication with prescribing physicians — including last-minute medication changes, inconsistent dosing administration, and inadequate family notification — which contribute to agitation, sleep disruption, and care transitions that required hospitalization in some accounts. Reviewers also raise infection-prevention and clinical-device management as an area needing attention.

Dining and daily programming draw mixed feedback. Activity offerings are a clear strength when staffed and supported: reviewers praise a robust calendar (bingo, art projects, library time, concerts, outings) and named activities staff who engage residents. However, some families note a reduction in activity frequency or scope at times. Meal quality and consistency are described variably; some families find the food satisfactory and good value, while others see room for improvement in menus and service continuity.

Facility operations and management produce divergent impressions. Several reviewers commend responsive administrators and coordinated social services that eased transitions and communicated well with families. Conversely, other reviewers describe defensive or adversarial responses when concerns are raised, delays in facility repairs or vendor payments, and uneven enforcement of workplace policies (including reports of staff smoking on duty). Housekeeping and laundry controls are generally effective but have occasional lapses (lost clothing, inconsistent room upkeep) reported.

Taken together, Azle Manor presents as a facility with notable strengths in rehabilitation, individualized caregiving, and an inviting physical environment, alongside operational weaknesses tied to staffing consistency, clinical-communication processes, and certain practice controls. Prospective residents and families should consider an in-person tour, ask specific questions about current staffing ratios and medication-administration protocols, review infection-control and transfer-safety procedures, and clarify the facility's complaint-resolution pathway before making placement decisions.

    About Azle Manor Nursing Home

Azle Manor Nursing Home has 142 certified beds and usually about 101 residents each day, so it's a good-sized place that focuses on long-term care and skilled nursing, and while it offers rehabilitation services for those needing therapy, people live here full-time too and get help with daily activities, medication, and regular personal care throughout the day. Owned mostly by Burmont, Inc., there's not much information about who actually runs it day to day, but they do have a team of registered nurses, licensed practical nurses, certified nursing assistants, and various specialists like podiatrists, dermatologists, psychiatrists, optometrists, and dentists, and they provide twenty-four-hour care and supervision. Azle Manor accepts Medicare, Medicaid, private, and hospice patients, and households choosing it can expect a clean environment with a call system that works around the clock, plus the rooms, whether private or shared, come fully furnished, and have things like kitchenettes, private bathrooms, big windows that look out at the countryside, and air-conditioning for comfort, and the facility itself has spacious lounges, TVs, outdoor patios, dining rooms, therapeutic whirlpool tubs, and even a beauty and barber shop, so residents can have some of the comforts of home.

The staff do medication management, laundry, and housekeeping, and there's organic food at mealtimes, along with counseling for residents and families, a social services department that helps with admissions and paperwork, and nurses help residents get to appointments when needed, making sure people stay as independent as possible. Azle Manor is part of the local business chamber but doesn't advertise its care services as part of that mission. Residents have some social and recreational activities, hobbies, and interests to keep them active, and there's spiritual support, exercise, and wellness programs, which all help keep people engaged.

There are a few things that aren't perfect though, since a recent inspection from November 2024 showed 4 deficiencies that included issues with nutrition and dietary practices-like not always getting food from approved places or following standards for handling and serving food-which affected many people and had the potential for more than minimal harm, though no actual harm occurred, and they also had environmental deficiencies like not keeping up with pest control, and a couple of infection control violations, so they're still working on following federal rules better. Altogether, the last count was 10 total deficiencies over different inspections, including one from a complaint. Nurse turnover is about average compared to the state at 51.3%, but nurse staffing hours are a little below state average at 3.18 hours per resident daily, which means sometimes there might be fewer staff around than in other places.

Azle Manor Nursing Home keeps things focused on the basics-safe, round-the-clock care, good medical oversight, social opportunities, regular therapy, and comfortable, clean living spaces, with an eye on safety, personal attention, and keeping residents steady and well looked-after, but it's important to know about the recent food and infection control findings and the generally standard staffing situation before making decisions.
