    Warm remodeled facility, caring staff

I toured the beautifully remodeled community and was impressed by the warm, home-like atmosphere, lovely decor and fresh paint, private rehab rooms and a well-equipped therapy gym. Staff and leadership were friendly, caring and knowledgeable - hands-on administration, prompt assistance, organized systems and secure reception made me feel confident. I would recommend this facility for its attentive, compassionate care and strong activities and dining support.

    Summary of reviews

Reviewers portray a facility with a mixed performance profile. Strengths are concentrated in rehabilitation services and select staff members: the therapy team, therapy gym, and short-term rehab accommodations receive consistent positive mention for professionalism and effectiveness. Several families also note attractive, recently remodeled interiors and private rehab rooms, and some front-desk and kitchen staff are described as helpful and attentive. Under recent management changes some reviewers described more engaged leadership and improvements in organization and cleanliness.

Care-quality observations are highly variable. Multiple accounts point to inconsistent staffing levels, particularly on nights and weekends, producing delays in assistance and delays in medication administration for some residents. There are repeated concerns about staff responsiveness and the tone of communication from certain shifts. Reviewers indicate gaps in clinical follow-up and documentation review (for example, chart-review practices and discharge coordination), which have contributed to difficult hospital transitions and family anxiety.

Dining and daily-living feedback is polarized. Some visitors praised kitchen staff and enjoyable meals, but a number of families described inconsistent food quality and issues with meal temperature and nutrition. Activities and programming for long-term residents are cited positively; however, operational problems such as delayed bathing or mobility assistance were also described, implying uneven implementation of care plans.

Facility condition and safety concerns are mixed. Many reviewers appreciated recent remodeling and an attractive interior, while others raised sanitation and odor concerns in certain areas and noted maintenance delays. Several accounts raise security and personal-property issues; there are allegations regarding missing items and financial handling that families considered serious and sought investigation of. The facility’s visitor controls and phone connectivity were also mentioned as inconsistent, complicating family communication during post-acute stays.

Management and workforce themes recur across reviews. Staffing levels, turnover, and low wages were cited as factors that may undermine consistent care and staff professionalism. Some reviewers praised new leadership and described tangible improvements, while others reported slow administrative responsiveness to complaints and billing or transfer communications. Prospective residents and families should verify current staffing ratios, medication-safety protocols, background-check practices, family-communication procedures, and recent inspection or investigation outcomes during tours. Observing multiple shifts, asking about weekend/night coverage, and confirming therapy capabilities and property-safeguarding policies will help form a clearer, up-to-date impression.

    About Highlands Guest Care Center

Highlands Guest Care Center in Nocona, Texas, has 116 certified beds with both private and semi-private suites that have been newly remodeled, and those private rooms help residents who want a more comfortable rehab-to-home experience, and while there's a strong clinical team here skilled in wound care, IV antibiotics, and post-surgical care, plus a rehabilitation staff to help folks with recovery, the place has faced some serious problems in the past; inspection reports show 26 total deficiencies, including troubles with safe pain management, accident safety, and protection from abuse or neglect that even amounted to immediate jeopardy for residents' health or safety, and, on top of that, there've been a couple of infection-related deficiencies too, so there's room for improvement when it comes to patient safety. The nurse turnover rate runs high at 65.2%, and the facility provides about 2.91 nurse hours per resident each day, which is something for families to think about, but residents do get 24/7 nursing care and a comprehensive set of services like Dialysis, Medication Management, Long Term Care, Medically Complex Care, Occupational Therapy, Physical Therapy, Skilled Nursing, Speech Therapy, Tracheostomy Care, Traumatic Brain Injury care, Wound Care, and more specialized therapies, so there's a lot they can handle right at the center if someone needs help regaining independence after a hospital stay or just needs ongoing medical support. Highlands Guest Care Center belongs entirely to the Nocona Hospital District and has been managed by The Highlands Guest Care Center LLC since July 2018; it's still affiliated with the hospital district, so there's some sense of local oversight. Amenities like WiFi and arts and crafts activities give a bit of comfort, and the building is laid out to create a cozy, supportive setting for seniors, plus there are smoking areas for residents and visitors, since some folks like that, and the place is known to have responsible and attentive staff, with leadership under a respected administrator named JP, and there's even a long-term care ombudsman in place to help resolve resident complaints confidentially. Staff members shape care around each resident's needs and focus on recovery, well-being, and maintaining quality of life day to day. The general feel aims to be both comfortable and practical, with therapies and services meant to help people feel respected and looked after, but those inspection problems can't be ignored, so families might want to visit and see firsthand whether the environment and services are the right fit.
