    Caring staff restored my independence

I had a very positive stay at this clean, beautifully remodeled facility - the staff truly cared and often went above and beyond. The therapy team (PT/OT/speech) was excellent and helped me regain independence so I'm back home driving and shopping; activities and therapy dogs made it feel like a second home. Administrator Kelli and nurses Regina and TC, plus social workers Marilyn and LaToya and Ginger on admissions, were accessible and attentive - I highly recommend this place.

    Summary of reviews

Reviews indicate a facility with pronounced variability across clinical care, operations, and environment. A consistent strength is the rehabilitation program: physical, occupational, and speech therapy receive frequent praise for restoring mobility and independence, and several families credited therapy teams with successful discharges home. Activity staff and programs (including therapy dogs and group events) are also cited as positive contributors to resident engagement.

At the same time, nursing and direct-care quality is highly inconsistent. Numerous accounts describe delayed responses to call lights, missed or late medication administration, and gaps in basic personal-care routines. Several reviewers reported failures in documentation or care-tracking (for example, therapy sessions or lab results not recorded), which aligns with concerns about medication management and clinical follow-up. There are multiple references to limited after-hours clinical coverage and difficulty reaching a clinician outside regular hours, which contributes to family frustration around urgent issues.

Dining and nutrition present mixed feedback. Some families observed appropriate meal service and recovery-supporting nutrition, but many other comments cite cold meals, small portions, meals served in inappropriate textures, and interruptions to regular meal delivery. These inconsistencies appear to reflect uneven kitchen processes and communication between clinical and dietary staff.

Facility condition and environmental maintenance are recurring themes. Reviewers noted dated rooms, leaking ceilings, malfunctioning equipment (for example call buttons and sagging beds), and general maintenance shortfalls. Sanitation and incontinence-care concerns were raised frequently enough to be notable: delayed toileting assistance, personal-hygiene omissions, and related odor and cleanliness issues in particular areas. Loss or misplacement of clothing and personal items was also reported as an operational problem.

Family communication and management responsiveness show a split pattern. Several reviewers praised administrators and named staff for accessible, caring leadership and effective problem resolution. Others described unhelpful or argumentative interactions with social work and front-desk personnel, inconsistent visitation management, and inadequate discharge instructions or home-care follow-up. A small number of reviewers filed formal complaints and described serious clinical incidents; these accounts warrant attention from families and regulators and underscore the variability of experience.

Overall pattern: the facility appears to deliver strong, rehab-focused care for some residents through capable therapy teams and engaged activity staff, but persistent operational weaknesses—particularly inconsistent nursing coverage, medication and documentation controls, sanitation and maintenance issues, and variable communication—create risk for adverse experiences. Prospective families should weigh the strong rehabilitation capabilities against the reported variability in direct-care staffing, night coverage, meal consistency, and facility upkeep; when possible, visit the unit, ask about staffing levels and after-hours clinical access, and request recent care-plan and discharge processes before placement.
