    Strong rehab; compassionate, organized nursing

I placed my mom here for rehab and am very pleased - therapy was strong and she steadily improved, staff (D.O.N., nurses, aides) were responsive, professional and compassionate, and management felt organized. The building was clean, odor-free and orderly, with good food and a pleasant private-room view; activities were limited due to COVID but amenities exist. I recommend this center for rehab and long-term care.

    Summary of reviews

Overall impression: Reviews describe a facility with a mix of clear strengths and notable operational weaknesses. Positive elements include a strong rehabilitation focus, attentive therapy follow-up for some residents, consistently praised dining, and a generally clean and orderly environment in many areas. At the same time, families and visitors describe recurrent operational issues that affect care reliability and resident safety.

Care quality and clinical coordination: Clinical-care experiences appear uneven. Several reviewers indicated slow nurse response times and concerns about medication handling, suggesting lapses in medication-management processes and timeliness of care. There are also indications of delayed plan-of-care meetings and gaps in care coordination, which can complicate family communication and discharge or therapy planning. Rehabilitation services were highlighted as a strength by some families, with ongoing therapy and follow-up reported, but other comments noted insufficient therapy frequency for certain residents.

Staff and responsiveness: Staff descriptions are mixed. Many comments praise helpful, friendly, and organized staff members and note improvements in staff organization under new management. Conversely, other accounts raise concerns about staff conduct, limited time for resident interaction, and overall understaffing. These staffing constraints are linked in reviews to delayed assistance, long waits for responses to call buttons, and reduced time available for routine care tasks.

Dining and activities: Dining receives consistently positive feedback; reviewers describe high-quality meals and satisfied residents. Activities and amenities exist on-site, but programming has been curtailed or underutilized during infection-control periods. Families should expect limited group programming during outbreaks or strict visitation/infection-control phases, although activity spaces are present.

Facilities and environment: The physical plant shows contrasts. Some areas are described as clean, orderly, and recently refreshed, and private rooms with pleasant views are available. Other comments point to an older building with cosmetic wear and some outdated equipment and furnishings. Odor-control and general cleanliness are praised in multiple accounts, but there are also specific sanitation and incontinence-care concerns that merit attention.

Safety, management, and notable patterns: Reviews highlight several operational patterns that could affect safety and trust: inconsistent staffing, delayed hygiene assistance, questioned transfer and fall-prevention practices, concerns about emergency and incident handling, and billing/charge-transparency issues. Several families noted management-driven improvements and cosmetic turnover, indicating leadership attention to facility condition and organization. Prospective residents and families should weigh the documented strengths in rehabilitation and dining against reported variability in staffing, responsiveness, and clinical coordination when evaluating this facility.

About Fairfield Nursing & Rehabilitation Center

Fairfield Nursing & Rehabilitation Center sits in Crownsville, Maryland, with 96 certified beds, offering both private and semi-private accommodations so folks can find some comfort and privacy, with 38 private rooms and a range of semi-private suites available. The center provides skilled nursing, supportive living, and specialized care programs, from short-term rehabilitation with physical, speech, and occupational therapy to long-term care and memory care for those with greater needs, and residents get help with things like pressure ulcers, infections, and wound care, too, which is managed with support from a Certified Wound-Care Physician and specialty equipment, like pressure-relieving mattresses. The nursing staff is described as professional, capable, and friendly, with 24-hour care and individualized care plans for each resident, while the therapy center comes equipped with up-to-date tools for therapy assessment and treatment, and there's a full recreation therapy department, too, with programs focused on social connection and self-esteem, plus activities for enrichment and community outings using a dedicated van.

Fairfield Nursing & Rehabilitation Center serves meals with a restaurant-style table service, changing up the menu to keep things fresh, and works with a registered dietitian to make sure nutrition is looked after, which can be important for folks needing diabetic management or nutritional support. Residents can enjoy free Wi-Fi, cable television, inviting outdoor spaces, and there are salon and barber services on site, along with assistance in managing medications, blankets, and arranging transportation. Seasonal flu and pneumococcal vaccines are offered, and there are programs to manage antipsychotic medications where needed, fitting the care to each person's situation. The facility welcomes people who need care for issues such as long-term nursing needs, rehabilitation, and even post-surgical recovery requiring prolonged antibiotics, and is equipped with piped-in oxygen and wall suction for those who need it. With English spoken and possibly other languages, the staff aims to be accessible to many residents, keeping the atmosphere warm and supportive.

The center holds an accreditation and is committed to healthcare standards, having served the community for many years as part of a community health system, and offers both Medicare and Medicaid for eligible individuals. The location's had recent renovations for improved comfort and care, and there's an ongoing mission to treat residents with dignity and respect, making the place a home away from home, but there have also been health inspections with some noted deficiencies, and it's been involved in several nursing home lawsuits, including wrongful death claims and issues like falls, pressure sores, dehydration, and financial concerns, so families may wish to review inspection histories and public records. With visiting hours recommended between 8:00 a.m. and 8:00 p.m. and admissions accepted any time, the facility tries to support families and new residents, though at the moment it isn't currently accepting new patients but provides a phone number for updates on availability.
