    Beautiful family-friendly community with amenities

I toured this recently renovated community and was impressed - beautiful, family-friendly, and clearly focused on residents' quality of life. The staff were generous and sincere, and the wide variety of amenities (movie theater, chapel, game room, spa) made me want to recommend a tour. Overall a very good place for loved ones.

    Summary of reviews

Magnified Nursing and Rehabilitation presents a mixed profile: physically appealing and activity-rich, but with operational gaps affecting day-to-day clinical care. The facility is frequently described as recently renovated and well appointed, with a clear emphasis on quality-of-life programming. Amenities cited include a movie theater, chapel, game room, and spa, and the overall environment is described as family-friendly and welcoming. Admissions and tour experiences are commonly recommended as positive first impressions.

Clinical-care descriptions are mixed. Positive comments highlight engaged, generous, and sincere staff members who contribute to residents' quality of life. However, reviewers also raised distinct concerns about responsiveness and the timeliness of personal-care tasks, particularly incontinence-care delays. There are indications of inconsistent service delivery across shifts and occasions, and at least one concern about limited overnight clinical coverage. These patterns suggest the need for clearer staffing models and response protocols to ensure consistent care across all hours.

Management and administrative processes appear to be another area of friction. Families noted delays in Medicaid and insurance processing, which affected transitions and placement timing. Several accounts also describe situations where family members had to intervene repeatedly to coordinate care or advocate for a resident, indicating that internal care-coordination and communication processes may be uneven. Communication tone from certain staff members was also called out as an area for improvement; while many staff are praised for warmth, there are isolated reports of less constructive interactions.

Information about dining was not prominent in the summaries provided; prospective families should request specific details during a visit. Strengths of the facility include robust activities and a well-maintained, renovated environment that supports social engagement. Areas for due diligence prior to placement include confirming 24-hour clinical staffing levels, asking about response-time expectations for personal care needs, clarifying how the facility handles Medicaid/insurance paperwork and timelines, and discussing how the care team communicates with families. Taken together, the facility appears to offer a high-quality living environment with important operational challenges that prospective residents and families should evaluate in person.

    About Magnified Nursing and Rehabilitation

Magnified Nursing And Rehabilitation has been operating for 25 years and is managed by Sulik Healthcare Services, and while it's not a BBB Accredited Business, the facility is Medicare and Medicaid certified and has 126 licensed beds, with 83 currently occupied, offering both short-term and long-term skilled nursing care for seniors who need medical support or help after hospital stays, and they do provide physical, occupational, and speech therapy services using their on-site rehabilitation gym. Sheridan Lane is a secure care unit for those with dementia, where residents get 24-hour skilled nursing care, and there's also help with daily activities like bathing, dressing, grooming, and taking medicine, and families can join counseling sessions if they want. Rooms come in both semiprivate and private options, more than 100 in all, featuring things like private bathrooms, cable TV, kitchenettes, air conditioning, and Wi-Fi, and folks can enjoy home-like touches in the decorated common spaces. Meals are made by professional chefs in a community dining room, and the kitchen can make specialized diets for allergies or diabetes, with snacks and meals available all day, and folks can choose to dine together or more privately if they like. Outdoor walking paths and gardens, a library, arts and game rooms, a movie theater, a fitness and spa room, and organized activities provide ways to fill the day, and there's help with scheduling and moving in, plus basic services like housekeeping, laundry, transportation, parking, and even a concierge for any needs that pop up. The staff includes nurses and aides on duty around the clock, and the facility can help with complicated health needs, medication management, and recovery after medical procedures. Health monitoring is available through an online portal, and there are special events and community or resident-run programs to get involved as much as you like. Medicare has given Magnified Nursing And Rehabilitation a 1 out of 5 star overall rating, based on staff, health evaluations, and care quality, and recent government reports show 24 deficiencies, one assessed penalty, and a fine of $2,772, so it's good to review inspection records if that's important. Support is available for mental health, dental, pharmacy, clinical labs, x-rays, and organized group meetings for residents. While the facility is known in the community and has some awards and recognitions, it's recommended to visit, talk with staff, and discuss your specific needs to see if it's the right fit.
