    Compassionate attentive care, minor improvements

I moved my mom into this small, home-like community and have been very pleased with the compassionate, attentive staff, proactive nurses, excellent communication, clean well-kept spaces, engaging activities and great dining. Management is hands-on, the staff-to-resident ratio allows truly personalized, warm care, and this gem has made a positive difference for our family-very happy overall with just a little room for improvement.

    Summary of reviews

Overall impression The reviews portray Heart to Home, Inc. as a small, home-like senior care provider that emphasizes personalized attention and a family-style environment. Reviewers consistently highlight a clean, well-maintained facility and describe a warm, welcoming atmosphere. The small size and high staff-to-resident ratios are repeatedly credited with enabling individualized care plans, close observation by nursing staff, and frequent, personal interactions between staff and residents. Care quality and staff Care quality is frequently described as high, with proactive nursing and strong clinical and hygiene practices noted. Staff are characterized as compassionate, attentive, and knowledgeable; reviewers point to respectful interactions, personal attention, and a culture of warmth and laughter. Management and leadership are also singled out positively for professionalism and for setting high standards that translate into day-to-day care. These strengths appear to be a structural feature of the facility model: small scale, consistent staffing, and an emphasis on individualized services. Dining and activities Dining and meal service receive positive comments for attentiveness and quality. Activities and social programming are described as engaging and appropriate for resident needs, contributing to a lively, family-oriented atmosphere. These elements are viewed as complementary to the clinical care, supporting residents’ social and emotional well-being in addition to medical needs. Facilities and management The physical environment is described as spacious, well-kept, and homelike. Reviewers emphasize a sense of safety and comfort associated with the setting. Management is portrayed as active and effective, with several mentions that leadership ‘‘exceeds standards’’ and that staff and managers are responsive when families are engaged. Cost is described as comparable to other local options while delivering a more intimate care model. Notable patterns and caveats The dominant pattern across feedback is strongly positive, focused on individualized care, strong staff ratios, and a supportive home-like culture. However, there are isolated contrasting comments indicating occasional problems with staff responsiveness and the tone of particular staff interactions. A few reviewers sought direct contact from staff or noted delays in outreach; one comment referenced rude or unsupportive staff behavior. These items appear episodic rather than systemic in the broader set of comments, but they point to potential variability in individual staff interactions and communication follow-up. Recommendations for prospective families For families touring Heart to Home, Inc., the facility’s small scale, staffing model, and emphasis on personalized care are likely to be central strengths. Prospective residents and families should observe staff–resident interactions during a visit, ask about communication protocols and escalation pathways, and clarify expectations for outreach and responsiveness to ensure fit with their priorities. Overall, the reviews suggest a caregiving model focused on individualized attention and a homelike living experience, with a small number of occasional concerns about responsiveness and conduct that are prudent to verify during evaluation.

    About Heart to Home, Inc.

Heart to Home, Inc. is a senior living community in Mendota Heights, Minnesota, that has served residents since 2007, focusing on assisted living, memory care, and hospice in small, family-style homes. Each home, like Mendota Manor, Mendakota Manor, Hilltop Manor, and Lake Manor, holds 6 or 7 residents, with care team staff-to-resident ratios of 1:3 or 1:4 during the day, moving to 1:6 or 1:7 overnight, which means residents get more attention than at bigger places. Registered nurses and certified nursing assistants like Jessica Marcello and LaFonda Bradford watch over care and are trained in dementia, hospice, and geriatric needs. Residents have large private suites with handicap accessible bathrooms, all on one level for safety, and emergency alert systems, monitored alarms, and a 24-hour call system, which help everyone feel safe and supported.

The focus stays on the whole person-mind, body, and soul-so folks get care with daily activities, medication management, and tailored memory care and hospice waivers, plus the ability to receive more help as their needs change, so they can stay in the same home as they age. Housekeeping, laundry, dry cleaning, meal preparation, and dining options-also for special diets like allergies and diabetes-keep daily life comfortable, with home-cooked meals tailored to individuals, and rooms come furnished and have phones. Residents enjoy arts rooms, movie nights, gardening, walks in the neighborhood, trips out for ice cream, seasonal celebrations, and a garden where they can sit or take strolls. The homes are set up for easy access to nursing staff at all times, and community programs mean no one has to feel left out. Technology, including systems like Drupal and Cloudflare, supports information management, while features like walking paths, transportation, a barber/salon, group games, community-run activities, and libraries help residents stay active and social.

Founded by Nick Cesaro-Moxley and his son Josh Cesaro-Moxley, the community's leadership keeps the environment warm, friendly, and focused on each person's dignity and independence, giving everyone both a private room and a sense of belonging in a smaller, family-like setting. House activities, like arts and library programs, encourage friendships and engagement, and the move-in process offers help with coordination. The staff handles management, operations, and healthcare coordination, supporting older adults at every stage, and the homes offer a neighborly feeling with easy access to amenities and healthcare in the area. Residents can receive both on-site and remote consulting services, and as needs change, services adapt, letting residents age in place for the long term.
