    Father thriving; outstanding care, staff

I'm very pleased with this Minneapolis senior living facility - my father is thriving and our family had a wonderful visit. The nurses, therapists (and social worker Robin), receptionist and admin are kind, professional and went above and beyond; the therapy and rehab care is outstanding. The place is clean and pretty with nice outdoor space, comfortable rooms, engaging activities, well-balanced meals and 24/7 snacks - I feel cared for and plan to return.

    Summary of reviews

Reviews of Victory Health + Rehabilitation Center are highly mixed, showing clear strengths in therapy and supportive services alongside recurring operational concerns. Several accounts praise the facility’s rehabilitation program, therapists, and elements of nursing care; families and residents described a cohesive therapy team, helpful social-work assistance, comfortable rooms, accessible showers, and active outings and entertainment. The front entry and outdoor areas receive positive remarks, and some residents note that Medicare-covered stays are accepted and that the atmosphere can be welcoming and supportive.

At the same time, multiple reviewers describe operational gaps that affect clinical quality and resident safety. Key themes include inconsistent medication administration and clinical protocol adherence, delays in responding to resident requests, and deficiencies in wound care and infection-prevention practices. Reviewers also describe uneven staffing coverage—particularly on weekends—which correlates with concerns about supervision, missed clinical issues, and safety events such as prolonged absence of residents from the unit.

Cleanliness and environmental management emerge as repeat concerns. Several reviewers cite sanitation issues, odor concerns in common areas, and pest-control problems, which raise questions about the facility’s environmental services and routine maintenance. Relatedly, a number of family members reported problems with property management and accountability for residents’ belongings, as well as at least one allegation of financial misconduct; these claims warrant direct inquiry and verification with the facility and oversight bodies.

Communication and operational coordination are additional pain points. Families described inconsistent or poor communication from staff and management, missed or mismanaged external appointments and transport, and staff tone or conduct that some found unprofessional. Conversely, some families highlighted individual staff members and administrators who were responsive and advocacy-oriented; this suggests variability in staff performance rather than a uniformly positive or negative culture.

Dining and daily life received mixed feedback. While some residents enjoyed menu variety and snack access, others reported inconsistent food quality and meal-service execution. The activities program and outings were frequently praised, indicating strengths in social engagement despite variability in other service areas.

Given the polarized feedback, prospective residents and families should tour the facility, ask specific questions about staffing patterns (including weekend coverage), infection-prevention protocols, pest-control schedules, medication-administration safeguards, incident reporting, property-inventory controls, transportation processes, and how the facility addresses complaints. Confirming up-to-date regulatory inspection results and speaking with current families can help clarify whether praised clinical and therapy strengths are consistent across units and whether the operational concerns have been addressed.

About Victory Health + Rehabilitation Center

Victory Health & Rehabilitation Center is a medium-sized nursing facility with a capacity of 79 beds. The center operates under for-profit, individual ownership and is not part of a continuing care retirement community. It accepts both Medicare and Medicaid, enabling it to serve a wide range of residents with varying needs and financial resources.

The facility provides both short-term rehabilitation and long-term care services, earning an average rating in both areas based on recent assessments. Short-term rehabilitation at Victory Health & Rehabilitation Center focuses on post-acute care for individuals recovering from hospital stays due to conditions such as strokes, heart attacks, infections, or injuries. The goal of the center’s rehabilitation program is to help residents regain their strength and independence so that they can return home when possible. Long-term care is provided for residents who need ongoing assistance with daily tasks including medication administration, dressing, eating, mobility, and personal hygiene.

Staffing at the center averages about 3 hours and 11 minutes of nursing care per resident each day. Consistent nurse staffing ensures that residents’ medical and non-medical needs are attended to, although there have been noted differences in staffing levels between weekdays and weekends. The center has systems in place to monitor and support residents’ self-care abilities, with a significant percentage maintaining the capacity to perform daily activities independently. Preventive health measures such as annual influenza vaccinations are emphasized to safeguard residents’ well-being.

Victory Health & Rehabilitation Center has undergone regular health, fire, and safety inspections in accordance with relevant standards. These inspections cover food preparation, resident care practices, and overall environmental safety. The center has, at times, been subject to additional scrutiny to ensure standards are maintained, reflecting a commitment to addressing areas that require improvement.

In terms of resident safety and health outcomes, the center has a record of maintaining a low percentage of major injury falls among residents. Emergency room visit rates are also tracked and monitored to provide insights into resident health trends and the effectiveness of on-site care.

The center has been proactive in addressing challenges related to staffing, especially in specialized areas such as food and nutrition services, ensuring that residents’ dietary needs are met by appropriately qualified staff. Over recent years, the facility has addressed areas that required attention as identified through inspection reports, resolving issues and implementing needed changes to enhance the quality of care provided.

Victory Health & Rehabilitation Center strives to create an environment focused on the medical, physical, and emotional needs of its residents. By offering a combination of rehabilitation and long-term care, together with an experienced staff and established procedures for monitoring care quality, the facility is dedicated to supporting seniors and individuals who require ongoing assistance in their daily lives.
