    Beautiful facility, excellent dementia care

I had a very positive experience at this beautiful, modern facility - scenic grounds and a gorgeous courtyard, clean and organized spaces, and a large physical therapy area. The staff are warm and attentive, activities and improved meals are plentiful, and the dementia care is secure and well run; I highly recommend it.

    Summary of reviews

Reviews for Forest Manor Health Care Center present a mixed picture with distinct strengths and recurring operational weaknesses. Positive accounts highlight a recently renovated, bright facility with attractive outdoor areas and a secured dementia wing that includes wandering alarms. The center also receives praise for its physical and occupational therapy resources and for individual staff members who families described as attentive and compassionate. Some families found the long-term care lock-down unit to be organized and appreciated the availability of purposeful activities and a pleasant courtyard setting.

At the same time, a number of reviews identify systemic issues that affect day-to-day care. Staffing instability and what reviewers described as chronic understaffing emerge as a central concern; reviewers linked this to long call-bell response times, delays in assistance with personal care and bathing, and reports of inconsistent medication and pain-management delivery. These responsiveness problems appear to be operational rather than isolated events, and they directly influence perceived quality of care and family trust.

Dining and nutrition are another area of variability. Several accounts describe limited kitchen staffing, inconsistent meal preparation, and a lack of appropriate diet accommodations (for example, limited low-sodium or diabetic options). While some reviewers noted improvements with a new chef, others described poor meal temperature and limited dietary tailoring. Activity programming received mixed feedback as well: some families appreciated structured recreation and therapy offerings, while others found activities minimal or repetitive.

Facility maintenance and environmental concerns were frequently raised. While parts of the building are described as modern and well-kept, other areas were associated with sanitation and odor concerns, occasional laundry or housekeeping lapses, and at least one report that raised pest-control worries. In addition, reviewers noted recurring maintenance and equipment problems such as malfunctioning call bells and issues with beds or laundry that reduced resident comfort and safety.

Communication and management oversight are recurring themes. Several reviewers described poor communication between staff and families and inconsistent coordination among clinical staff, social work, and external providers. Some families reported delays in advance care coordination (including hospice assessment) and frustration with managerial follow-through. Conversely, a few reviewers acknowledged positive leadership changes and caring staff members who attempted to address problems.

In summary, Forest Manor offers meaningful clinical and environmental strengthsโ€”particularly in rehabilitation services, a secure dementia unit, and attractive outdoor spacesโ€”but it also exhibits persistent operational weaknesses around staffing, responsiveness, dining consistency, environmental maintenance, and communication. Prospective residents and families should weigh the facility's therapy and dementia-care capabilities against reports of variability in daily care and management responsiveness. Visiting at different times of day, speaking directly with therapy, nursing leadership, and the administrator about staffing patterns, dietary accommodations, and incident follow-up procedures will help assess current performance and fit.
