    Attentive, compassionate staff and therapy

I'm very pleased with our experience - the staff are attentive, kind, and genuinely compassionate from admissions through nursing, CNAs and therapy. Tina and the dietary team went above and beyond, my mom loved her cheery room with a big window and plants, and the community is clean, welcoming, and well coordinated with doctors and rehab. I'd recommend this facility for responsive, professional care and strong therapy support.

    Summary of reviews

Overall assessment Bourne Manor Extended Care elicits strongly mixed impressions. Many families and residents praise the facility's admissions process, nursing staff, and rehabilitation teams as attentive, compassionate, and effective—particularly for short-term rehab goals. Reviewers frequently highlight coordinated transportation for dialysis and medical appointments, cheery and well-lit resident rooms, and an active activities program supported by a dedicated activities director. Administrative and ancillary staff (including dietary and therapy personnel) receive repeated commendations for responsiveness and personalized attention.

Care quality and clinical oversight Clinical care is described as high-quality in numerous accounts, with physical and occupational therapy singled out for producing measurable mobility gains. At the same time, there are recurring concerns about gaps in clinical monitoring and escalation: families described cases of missed medical deterioration, delayed transfers to higher-acuity care, and interruptions in therapy continuity. Medication management and tracking also surface as an operational weakness, with examples of long medication waits, lost prescriptions, and inconsistent documentation. Prospective families should ask about current staffing ratios, weekend coverage, medication-administration protocols, and how clinical changes are escalated to physicians.

Staff conduct, communication and culture Reviews show a broad spectrum of staff behavior. Many reviewers describe compassionate, family-oriented caregivers and clear, informative updates from lead nurses. Conversely, others describe variable responsiveness, an unfriendly tone from some staff, and instances of poor communication. There are serious community concerns about discriminatory behavior and hiring practices; such allegations warrant direct inquiry and, if relevant, confirmation of any facility-level corrective actions or external investigations.

Dining, activities and environment Dining impressions are mixed: several families praise the dietary department's ability to meet picky eaters and the quality of meals, while others note periods of inconsistent food quality and menu adherence. The activities program is generally viewed as a strength, offering frequent programming, outings, and social engagement. Physical environment strengths include comfortable rooms with natural light and plants, but some reviews raise sanitation and odor concerns in certain areas. Prospective residents should tour dining and common areas at different times of day to assess consistency.

Management, operations and documentation Operational issues appear concentrated around staffing consistency, weekend coverage, documentation practices, and inventory controls. Multiple reviews mention reliance on paper records or limited electronic documentation, which can complicate continuity of care. There are also mentions of lost personal items and limited flexibility around hospice visitation in certain circumstances. Families are advised to review the facility’s policies on personal property, hospice and visitation, electronic health record use, and remedial actions taken when staff shortages occur.

Conclusion and practical advice Bourne Manor demonstrates clear strengths in rehabilitation services, admissions coordination, and individualized attention from many members of the care team. However, variability in staffing, clinical monitoring, medication handling, cleanliness in some areas, and staff conduct create notable operational risks. If considering this facility, ask for current staffing ratios and weekend coverage, examples of recent clinical-quality improvements, medication safety procedures, policies on hospice visitation, and documentation practices. An in-person tour focusing on mealtimes, evening staffing, and common-area cleanliness will help families assess whether the facility’s strengths align with their loved one’s needs.

    About Bourne Manor Extended Care

Bourne Manor Extended Care sits at 146 MacArthur Blvd in Bourne, MA, pretty close to the Bourne and Sagamore Bridges, so getting to and from Route 6 or Route 28 is simple, and plenty of local physicians or hospitals are nearby if you need them, and the place has been around since 1989, so folks in Bourne, Falmouth, Marion, Wareham, New Bedford, and up and down Cape Cod might recognize the name. It's a skilled nursing facility focused on senior care, with options for independent living, assisted living, skilled nursing, memory care including a separate memory care unit for those with Alzheimer's or dementia, and programs for short-term rehab, long-term care, respite care, hospice, and palliative care-the basics and then some, really. Residents get 24-hour care, including support with bathing, dressing, taking medicine, and meals planned by a dietitian, and there are specialized rehab services-physical, occupational, and speech therapy-with programs that help people recover after surgery, stroke, joint problems, hospital stays, or even more complex issues like heart failure, diabetes, Parkinson's, MS, and ALS, and the Steps to Strength program encourages folks to keep up their physical abilities. Nurses and staff are always on hand, with an education program in place for them, and language variety's possible depending on who's working. The clinical team manages pain, wounds, medication, cardiac and respiratory care, dialysis, diabetes, and more, and the pulmonary rehab program is especially set up for COPD, pneumonia, and other serious respiratory problems, and there's help for orthopedic recovery too. The memory care unit provides safety and support for people living with Alzheimer's or other forms of dementia, and there are policies in place to reduce unnecessary medications and help people avoid falls. Bourne Manor Extended Care is open all day, all week, and offers respite stays when family caregivers need a break or residents need care right after a hospital stay. You'll find private and semi-private rooms, individual climate controls, free Wi-Fi and DIRECTV®, free phone service, and a beauty/barber shop. Visitors can eat meals there for free, and there's a steady calendar of activities like arts and crafts, music therapy, religious services, bingo, pet therapy, and community involvement. Resident councils give people a say about what goes on. The place aims for a warm environment where people feel welcome, encouraging friendship and daily connections. Care plans get tailored for each person with the goal to help them reach their best possible health and keep as much independence as possible, and family and residents get a voice in decision-making, while care transitions are supported so moves between settings go more smoothly. Bourne Manor Extended Care is accredited by the Joint Commission and is part of Berkshire Healthcare, and it takes referrals from the Providence Health Plan networks. Staff keep up with best practices, offering seminars and education on topics like flu prevention and end-of-life care. They update their online facility directory at least every month. At the moment, Bourne Manor Extended Care isn't accepting new patients, but it's known for thorough care and a full range of services for seniors in the area.
