    Friendly caring community with therapy

I toured the assisted living and left with a very positive impression - the staff and director were friendly and knowledgeable, and everyone clearly cares. It's a nice, community-feeling facility with nursing and assisted living options, occupational therapy, and accepts Alzheimer's/dementia patients; waiver options include shared 2-bedroom suites.

    Summary of reviews

This facility elicits a mixed set of impressions. Several reviewers praised individual caregivers, the director on tour, and the overall small, community-like atmosphere. The site provides a range of care levels — independent living, assisted living, memory care, and skilled nursing — and has on-site occupational therapy resources, which are meaningful positives for families seeking continuity of care across needs.

However, a number of operational concerns emerge across reviews that affect care quality. There are recurring accounts of inconsistent staff visibility and low engagement on units, which reviewers linked to unmet basic needs and family members stepping in for routine grooming and assistance. Families described gaps in communication about changes in health and timing of clinical events. Related clinical-safety issues include concerns about medication-management practices and a pattern of resident falls that point to weaknesses in transfer and fall-prevention protocols.

Dining and daily care show variability. Some families noted attentive assistance when staff are present, while others reported that meals or meal assistance were not consistently offered without a resident request. Reviewers also raised security concerns for personal belongings and clothing, indicating that inventory and personal-item controls warrant attention.

Memory-care and facility-layout issues were specifically noted. The memory-care unit's location and design were described as having limited access to outdoor spaces and natural light, which can affect resident engagement and wellbeing. The building itself is described as older, with related maintenance and environmental-safety limitations; at least one family raised concerns about winter precaution measures.

Management and admissions interactions present another clear theme. While some frontline staff were characterized as kind and helpful, others described unprofessional communication from admissions and leadership, including curt or dismissive tones. Several reviewers perceived that service levels and certain safeguards varied by payer status, creating concerns about equitable access to supports.

For prospective residents and families: observe staffing patterns during multiple times of day, ask for written policies on medication management, fall prevention, and belongings/security, and request specifics about staffing ratios and outdoor access for memory-care residents. Clarify billing and service differences by payment type and seek references from current families. The facility offers useful rehabilitative resources and a community atmosphere, but attention to communication, safety protocols, and facility infrastructure would be important considerations before placement.

    About Crest View Senior Communities

Crest View Senior Communities sits at 4444 Reservoir Blvd NE in Columbia Heights, Minnesota, and has served seniors since 1952 as a nonprofit organization with a Christian mission, aiming to support people's spiritual, emotional, and physical needs through a continuum of care. There are several levels of housing and care to pick from, including independent living, assisted living, memory care, skilled nursing, and short- or long-term rehabilitation, and people can choose from places like The Boulevard Apartments 55+, Crest View on 42nd, and others, where choices include 1-bedroom or 2-bedroom apartments and, if eligible, help with funding through Elderly Waiver or other assistance programs is available. The community's three-story building has things people need to live comfortably and safely-such as private apartments, weekly housekeeping and linen service, cable television hookup, individual thermostats, urgency call systems, and a home health aide on duty any time night or day; some apartments allow small pets as well. The grounds include gardens, community spaces, and a library, and there are planned outings using the Crest View van, escorted trips to local stores or medical offices, access to golf courses and parks, and the chance to join group exercise, games, worship services, and wellness clinics if you want. Residents receive three meals daily with snacks in between, and the staff provide medication help, personal care assistance, customized housekeeping, and social and recreational activities designed to keep folks as independent as possible. The Ruths Way area is secured for people living with Alzheimer's or dementia, offering spaces with vintage furnishings and simple layouts for easy navigation. The Columbia Heights campus holds 122 skilled nursing beds licensed by the State of Minnesota and certified by Medicare and Medicaid, and the staff are always available for support. Crest View Senior Communities aims to make life easier by bringing certain care, therapy, and support services straight to people's rooms or even their homes, and spiritual support is provided by Director of Spiritual Care Robert Lyndes, with regular worship and Bible study sessions for anyone interested, but all faiths are welcome. Crest View also works with most managed care providers, accepts many assistance programs, and belongs to the Twin Cities North Chamber of Commerce. Shirley Barnes serves as the CEO, and the community makes a point of encouraging folks and their families to come tour, meet the staff and residents, and see for themselves what the place is really like.
