    Caring attentive staff home-like facility

I placed my aunt here and have been very pleased - the nurses and CNAs (Sandra, Miata, Corrinne and Dolores stood out) are caring, attentive and skilled, and therapy helped her regain independence. The small, home-like facility in a woodsy setting by a lake is very clean, has good-sized shared rooms, very good food, a nice outdoor area, beauty parlor and lots of activities and day trips. Staff check in daily and the admissions/office staff are friendly and helpful; overall a supportive, trustworthy place I gladly recommend.

    Summary of reviews

The reviews present a mixed and polarized picture of Hebert Health & Rehabilitation Center. Many families describe strong clinical strengths โ€” notably a high-quality therapy team, compassionate nursing assistants and nurses, focused wound care and restorative therapy, and an engaging activities program. The facilityโ€™s smaller size and woodsy setting, along with responsive maintenance and an accessible salon, contribute to a home-like environment that some residents and relatives consistently praise.

However, recurring operational concerns are evident across reviews. Clinical-care consistency is an important pattern: reviewers describe problems with medication administration and allergy management, missed repositioning and bathing, and delays in responding to falls and acute changes. These clinical-process gaps are described alongside reports of infection-control and cleaning-process shortcomings in select areas, which families associate with increased risk to vulnerable residents. Several reviewers also identified prolonged or delayed therapy coordination and discharge planning that affected recovery timelines.

Staffing and culture issues are a central theme. While individuals and teams (therapists, some nurses and CNAs, social work, and select administrators) receive strong praise for their professionalism and compassion, many reviews describe chronic understaffing, frequent management turnover, and variable staff conduct. Families report delayed communication from leadership, voicemail callbacks not returned, and inconsistent transparency around care decisions. Management and billing transparency is another repeated concern, including questions about financial handling and unresolved invoice disputes; there are also isolated allegations regarding lost items and financial mishandling that families found serious.

Dining, activities, and amenities show both strengths and inconsistencies. The facility offers varied menus, a dining-room atmosphere that some residents enjoy, and regular entertainment and outings; activities staff receive positive comments. At the same time, meal-service continuity and diet-texture accommodation (pureed/nectar-thickened diets) are described as inconsistent by some families, and there are accounts of meal trays arriving cold. Laundry and personal-belongings controls are another operational weakness: several families report items going missing or being mixed in laundry, which undermines confidence in daily-operations processes.

Communication, privacy, and family access are additional areas of concern. Reviewers cite limited in-unit telephone access, difficulty using cell phones, and instances they considered privacy/HIPAA breaches. Visitation and hospice coordination are described as inflexible by some families, including situations where families felt hospice involvement was remote rather than in-person. There are also mentions of regulatory complaints by relatives to oversight agencies.

For prospective residents and families: the facility demonstrates clear strengths in therapy, individualized caregiving by certain staff, and a comfortable, small-campus environment. At the same time, there are persistent operational issues around staffing levels, medication and clinical-process consistency, communication, cleanliness controls, personal-belongings handling, and administrative transparency. When evaluating this facility, visitors should verify current staffing ratios, review medication and incident-response protocols, ask about infection-control and laundry tagging procedures, confirm visitation and hospice policies, and seek recent inspection or survey results to understand whether previously raised concerns have been addressed.

About Hebert Health & Rehabilitation Center

Hebert Health & Rehabilitation Center sits in Smithfield, RI, right by the tree-lined roads and Stump Pond, and people find the setting calm and pretty, which most seniors tend to like, since it makes looking out the windows a little more enjoyable. The place mainly helps people who need skilled nursing care, long-term care, or rehabilitation after an injury, surgery, or a medical setback, and they take on people who might stay just a short time or who need an extended stay. There's help for families, too, with information about caregiving and Long-term Care Insurance, so families don't have to feel like they're left guessing about what comes next. The staff pay a lot of attention to long-term needs, offering support for folks with mobility problems, memory issues, or more serious medical conditions, and they've got 24-hour skilled nursing if someone's health gets complicated. Rooms have wheelchair-accessible showers, and there's a mix of indoor and outdoor areas, with parking and free rides when someone needs to get out. The chefs and meal planners put together nutritious food, which is helpful for people with all sorts of dietary needs, and activities include things to do inside, chances to go out, devotional time, and many recreational options, so days feel a little fuller. For anyone facing memory loss, there's a dedicated memory care program that works to keep things safe and structured, and the staff are known to be familiar faces around the area, which makes it feel more like a small community. The facility offers help with pain management, diabetes, IV therapy, wounds, and incontinence, and there are programs for stroke, heart, or orthopedic recovery, which is important for folks bouncing back after tough health events. Physical, occupational, and speech therapy are available, aimed at helping residents get back as much independence as possible. While Hebert Health & Rehabilitation Center scores a 2.4-star average from 18 reviews, people still choose it for its convenience to Smithfield and the surrounding area, as well as the support from a team that's spent a lot of time serving the local community.
